and stiffness in osteoarthritis was assessed over a two-week period in a multicentre general practice open study. In this trial it has been demonstrated that a correlation exists between

Of all the signs of Whipple's disease, the joint manifestations are among the most constant and the earliest to indicate the enteropathy, appearing long before the digestive and general signs. Essentially they consist of painful, peripheral joint manifestations: either simple
arthralgia
, or true arthritis differing in the degree of
pain
, the degree of the clinical signs accompanying the
pain
, the mode of evolution, and the number and the grouping of the joints affected, thus occurring in numerous clinical forms of which the two principal ones are subacute oligoarthritis with a tendency to migrate and chronic polyarthritis that gives rise to few definitive deformations. The radiographic appearance is usually normal. There is nothing specific about the laboratory aspects of the inflammatory syndrome. Synovial histology may in some cases clarify the diagnosis by demonstration of histiocytes with positive PAS granulations. The axial joint manifestations, which are always associated with the preceding ones, are infrequent and practically limited to unilateral or bilateral sacro-iliac lesions, with little or no clinical expression, that are discovered by standard radiology. Jejunal biopsy can be the key to early diagnosis. The pathogenesis remains obscure. The treatment is the same as for Whipple's disease, long-term antibiotic therapy.

Industrial overexposure to chlordecone, an organochlorine insecticide, caused tremor in 76 of 148 exposed workers. Chlordecone was absorbed through oral, respiratory, and dermal routes, the last possibly the most significant. Epidemiology of this incident disclosed low-level, widespread environmental exposure of man to chlordecone. In 23 workers with chronic chlordecone intoxication, tremor was associated with opsoclonus, pleuritic

. No seizures were reported. The site of action of chlordecone on the central nervous system is unknown. It concentrates in human adipose and hepatic tissue but is not biodegradable, either in humans or elsewhere in nature.

Nine patients presenting temporo-mandibular

-dysfunction syndrome were treated with occlusal bite splints made of acrylic resin, which eliminated premature contacts and occlusal interferences. Results were obtained with an extra-oral apparatus, of personal conception, which allowed to amplify and register maximum opening and lateral deviation in the frontal plane. The study has shown that this treatment brings: - an elimination of

and muscle spasms - an increase in maximum opening - a decrease in lateral deviation - a marked but partial decrease in articular noises.

-dysfunction syndrome, a term first suggested by Schwartz (1959) and now probably the most widely accepted. Another relatively common problem is degenerative disease or osteoarthrosis, and although this generally affects an older age group than

-dysfunction syndrome, its occurance in the younger patient is by no means a rarity and is further evidence that the two conditions are expressions of the same problem, that is repetitive overloading of the joint. It is proposed that this should be termed mandibular stress syndrome.

dysfunction syndrome. The authors present a short review from the literature that supports the multifactorial etiology of temporomandibular joint dysfunction and provide, in two case studies, a combination of conservative dental treatment and hypnotherapy. The two case studies show a successful resolution of temporomandibular joint dysfunction with hypnosis in one case and failure of hypnotherapy to relieve the

in a second case. Reasons for the success and failure are discussed. The article closes with some thoughts and ideas that dental practitioners can use in treating temporomandibular

Forty-six patients were admitted to an open general practice study of clomipramine (Anafranil) in the treatment of long-standing rheumatic
pain
; forty-one patients completed the trial. The duration of the trial was 56 days during which patients received daily doses of either 10 mg or 25 mg of clomipramine. Assessments were made at fortnightly intervals. Patients were assessed for
pain
using a visual analogue scale, analgesic requirement,
joint pain
, other
pain
and morning stiffness. At the completion of the study patients were asked whether the addition of clomipramine to their treatment was better, the same or worse than no additional treatment. Twenty-one patients (57%) felt that it was better, four felt that it was the same and twelve said that it was worse. The doctor also similarly recorded his preference at the end of the trial. In twenty-two cases (60%) the doctor felt it was better, in eight that there was no difference and in seven that it was worse. There was no difference between the 10 mg and the 25 mg regime. It is suggested that better control of rheumatic symptoms can be achieved in some patients by the addition of small daily doses of clomipramine to their standard anti-rheumatic therapy. The author expresses the view that further research in this field is merited.

The occurrence of "objective" symptoms of TMJ -disorder, as measured by clinical examination, and self-reported "subjective" symptoms, were investigated in a survey of 241 persons ages 65-79. This group constituted the participants of a sample which was drawn to cover all persons in this age category living in the county of Troms in Norway. The participation rate was 86. Participants were examined and interviewed by dentists in graduate training. Eight per cent of the group reported to have had TMJ
pain
recently. Fourteen per cent said they had clicking or creptiation. These symptoms were twice as commonly reported by women as by men, and they were also most frequent among people complaining of rheumatism or general
joint pain
. By the clinical examination, clicking and/or crepitation was found in 27% of the individuals. Limitation of jaw movement was registered in only 3 persons. These "objective" symptoms were evenly common with both sexes. Dental/prosthetic status had seemingly no distinct influence of the findings.

The effect of transcutaneous nerve stimulation (TNS) on
joint pain
was studied in patients with rheumatoid arthritis. Three different forms of TNS were used in an attempt to evaluate the influence of placebo effects. The results were evaluated by means of loading tests and from the patients' own assessments of
pain
relief. High intensity TNS near the painful joint gave
pain
relief of varying degree in 95% of the patients, while TNS of low intensity and TNS applied at a remote site gave an improvement in 75% and 5% respectively. It seems quite clear that TNS is effective in reducing
joint pain
. There are a number of factors which intimate that this improvement is not merely a placebo effect.

Aseptic necrosis developed in 11 (6%) of 171 recipients of renal allografts who underwent transplant operations at Childrens Hospital of Los Angeles between February 1967 and August 1977.
Pain
was the predominant presenting symptom and preceded roentgenographic evidence of aseptic necrosis by as long as seven months. Initial symptoms occurred two months to four years posttransplant. Limited weight bearing and reduction in the dosage of prednisone failed to prevent the progressive destruction of five femoral heads in three patients. Hip replacement led to an amelioration of the symptoms and a resumption of normal activity in each patient. Two patients with involvement of multiple osseous structures have persistent knee and elbow

and effusions, and one of them has required prosthetic replacement of the proximal humerus. No therapy was required for patients with aseptic necrosis of single bones of the hand and foot. There was no statistically significant difference in the total steroid dose received during the first posttransplant year between patients in whom aseptic necrosis developed, and those in whom it did not develop.
