Histochemical methods were applied to determination of salomycin, dermatansulfate, keratosulfate and neutral mucopolysaccharides in the goblet cells of rabbit appendix. Sialomycin and neutral mucopolysaccharides were found in the glands. Immunization with human gamma globulin was followed in the productive phase of immunogenesis by a rise of the secretory activity of the mucosa of the appendix. An increase of the mucus formation in the appendix occurred after laparatomy and during the initial period of experimental appendicitis. However, surgical trauma caused an opposite effect against the background of increased functional activity of the secretory elements of the immunized animals, and during experimental appendicitis no activation of mucus formation was seen. The influence of the abovementioned factors on the secretory activity of the mucous membrane was accompanied by qualitative changes in the synthesized mucus.

Non-neoplastic gastrointestinal disease in South America is largely related to environmental conditions. Parasitic disorders, including Chagas' disease with megacolon, predominate in endemic regions. Common enteritides of various etiologie are frequent, whereas appendicits, diverticulosis of the colon, ulcerative colitis and Crohn's disease are extremely rare. There was no appendicitis in native Indians of Paraguay.

Ninety-three postoperative patients 1 day to 13 years of age had blood cultures, limulus lysate assay, determination of fibrin degradation products, white blood cell and platelet counts. Seven groups were studied. The limulus lysate assay was often positive (64%) in the presence of gram negative septicemia but there were false positives and negatives. The tests for fibrin degradation products were inconsistent. The white blood cell count was low in babies with gram negative septicemia. One hundred per cent of the infants with gram negative septicemia had a platelet count below 150,000; 71% below 100,000 (average 67,000 septic babies, 257,000 non-septic babies). The drop in platelet count with gram negative septicemia was abrupt---as much as 222,000 in 24 hours. Platelets increased when therapy was effective. Two children with gram negative septicemia had platelet counts of 50,000 and 20,000. The platelet count for patients with gram positive septicemia was 299,000, and above 150,000 in all children with ruptured and non-ruptured appendicitis and major surgery without gram negative septicemia. It was concluded that serial measurements of platelet count in the postoperative infant and child was a rapid and reliable method for early detection of gram negative septicemia and changes in platelet count in response to treatment was an indicator of the effectiveness of therapy.

The authors compared the clinical and pathoanatomical diagnoses in 1399 patients, operated upon for catarrhal appendicitis. In 123 children the clinical diagnosis was revised as a result of further observations following surgery. In 1276 patients the clinical diagnosis of catarrhal appendicitis was remained. An exploration of the appendicular process in these patients supported the diagnosis in 748 cases (58.6%). In 16 cases chronic appendicitis was recognized, in 111 -- phlegmonous and gangrenous appendicitis (8.7%).

The relationship between the nematode Enterobius vermicularis and appendicectomy in three age-groups: 0--15, 16--20 and 21--25 years was followed during the 1968-1973 period. Following the compaign against enterobiosis, there was a significant decrease in the number of operations for appendicitis in the 0--15 years age-group; the incidence of appendicectomy was greater in the 16--20 years age-group than in the reference 21--25 years age-group.

Fibrosis of the intestinal lymphatic vessels, produced in one case by tuberculosis and, in the other, by appendicitis and peritonitis, caused blockage of the main lymphatic vessels causing, clinically, a protein-losing enteropathy similar to that noted in congenital lymphatic diseases of childhood. In the laboratory, there was noted a fall in serum protein, lipid and cholesterol. A fat absorption test was very abnormal showing a flat curve. During laparotomy, there was discovered on the small intestine, the same layout of lymph vessels, resembling a lace network, as that observed in congenital malformations. Intestinal lymphography showed considerable stasis of the opaque substance and absence of injection of the lymph vessels in the mesentery.

In 27 years, 64 patients were operated upon for ovarian lesions, 43 for tumors, 12 for nonneoplastic cysts and nine for ovarian torsion. Of all the girls in this age group who were operated upon for appendicitis, 2 percent had ovarian lesions. Of the girls referred for isosexual precocity in the last ten years, 4.7 per cent had ovarian lesions. Of the girls in this age group operated upon for malignant abdominal lesions, excluding hepatic, in the last 15 years, 10.8 per cent had ovarian tumors. Of the 11 tumors with endocrine function, four were malignant. This association is statistically significant. There were no operative deaths. There were no postoperative deaths among six patients with Stage I and II lesions. Mortality was confined to the two patients with Stage III and IV lesions. The surgical approach was designed to conserve ovarian tissue, and adjunctive chemotherapy and radiotherapy were infrequently used.

Two cases of perforated retrocecal appendicitis presenting with the roentgen finding of an air containing subhepatic abscess are discussed. Although it is an unusual cause for air in the right subhepatic space, retrocecal appendicitis should be considered in the radiologist's differential diagnosis.

Phosphatase activity of neutrophils was studied in 342 patients directed to the hospital with the diagnosis of acute appendicitis, and in 56 healthy children. The index of phosphatase activity of neutrophils (IPAN) was found to be considerably increased in all forms of appendicitis in childhood. An intensity of the reaction depends on the form and terms, elapsed since the disease onset. The determination of IPAN in the complex with clinical findings could be utilized for the differential diagnosis purposes in investigation of patients suspected of acute appendicitis, while in the postoperative period - for estimating a gravity of the course and prognosis of the disease.

This report is a retrospective evaluation of 12 case histories. All patients had been subjected to laparotomy on suspicion of acute appendicitis. The operative situations seemed to corroborate clinical expectations: an inflamed ileocecal process was assumed to be a result of performation of a gangrenous appendicitis. In three patients a primary adenocarcinoma arising from the appendiceal base was the incidental histologic finding postoperatively. In three other patients this inflammatory-looking process was related to a cecal carcinoma. In six patients a carcinoma existed unrecognized as the basic disease, these patients being subjected to intra-abdominal drainage only or in connection with appendectomy. In five instances a fecal fistula was the main persisting symptom of postoperative morbidity, in one patient even as long as two years after laparotomy. The problem in diagnostic verification of the initially unidentified carcinoma is illustrated. Attention is directed towards avoiding a false feeling of security in the presence of inflammatory manifestations in the right lower abdominal quadrant, which may misleadingly suggest a ruptured appendix.
