Eight patients were seen with idiopathic
chondrolysis
in nine hips, with from one to eight years' follow-up. The adolescent patients showed characteristic diagnostic patterns--patients in the hip and a limp. The disease occurs most frequently in black girls and in them it tends to be more severe. . The pathological process in our patients mainly consisted of chronic synovitis and loss of articular cartilate in the weight-bearing surface of the femoral head. developed in 6% of the hips, occurring only when there was persistent intraarticular protrusion of the transepiphyseal fixation device. The chondrolytic process resulted in a relatively painless fibrous
ankylosis
of each hip. Avascular necrosis developed in 7% of the hips, and occurred only in the presence of a valgus reduction of an acute slipped epiphysis. Each case of avascular necrosis was of the whole-head type and was severely progressive. Biplane subtrochanteric femoral osteotomy was employed as a reconstructive procedure in the severely-displaced slipped epiphysis; it was delayed until after epiphyseal plate closure, and restored satisfactory hip function in all cases. Black children are not predisposed to poor results after slipped capital femoral epiphysis contrary to a commonly held impression.

appears as a complication of preexistent Protrusio Acetabuli.

The authors have seen 41 instances of
chondrolysis
following slipped capital femoral epiphyses. Most of the cases were followed-up for more than five years. The main clinical features are reviewed. There is joint stiffness and generalised narrowing of the joint space during the year following the initial lesion. The evolution of the lesion is unpredictable. A number of possible radiological changes may develop including renewal of the joint space, modifications in the femoral head with a tendency towards protrusio, and osteophyte formation at the lateral margins of the acetabulum. In five cases a biopsy was performed which showed a non-specific dystrophy of the acetabular and femoral cartilage, with sub-chondral bony change different from that found in epiphyseal necrosis. The synovium did not show inflammatory change. The treatment should not be surgical at an early stage but the effectiveness of traction was uncertain. Later, corrective osteotomies may be indicated. Definitive surgical procedures such as
arthrodesis
or arthroplasty should not be made early because late clinical recovery has been noted. Cup arthroplasties gave deceptive results. The etiology of this disease is discussed.

With reference to 5 recently observed cases, and after a review of the literature, the rare and hitherto inadequately clarified complex of stiffness of the hip in puberty is considered. The primary condition--
chondrolysis
, not necrosis of the head--is typified by destruction with subsequent layered fibrosis of the tissue at the cartilage/bone transition point, mainly at the head of the femur and the acetabulum. The clinical picture is an independent one, accompanied by synovitis and in severe cases leading to fibrous
ankylosis
. It is not only seen in females with deep and protruding acetabuli, beginning usually between the ages of 10 and 16; it also occurs as a complication of femoral head epiphysiolysis in boys and occasionally also after trauma and inflammations in the hip region. In severe cases the treatment of these events, which even occur occasionally on the "healthy" side, is unsatisfactory. Early relaxation surgery still seems to be the most effective treatment, by reducing the pressure on the base of the acetabulum which probably occurs in these cases. All treatment should be performed in carefully timed and measured stages, "sneaking up" on the irritation.

The real cause of this disease is still unknown. When a preschool child, especially a girl, complains of pain and limitation of movement at one hip-joint without evident predisposing cause, plus narrowing of inter-articular space revealed by roentgenogram and normal values in hematology and ESR, this disease should be ruled out. Owing to lack of ideal therapy up to now, pain can be alleviated and

be prevented by performing active and passive functional exercises of the joint persistently under light load or no load conditions. Besides, immobilization of the joint by brace and plaster bandage are contraindications for this disease.

for an incapacitating painful and stiff hip. Clinical diagnoses included avascular necrosis (AVN) associated with slipped capital femoral epiphysis (SCFE) (7 patients), posttraumatic AVN (6), septic arthritis (4), complication of treatment of developmental dysplasia of the hip (DDH) (4), pathologic fracture of femoral neck through bone cyst with resulting AVN (2), Perthes disease (1), and idiopathic
chondrolysis
(1). Preoperative motion was minimal or absent in 13 patients, limited in 12, and very painful in 23 patients. A two-incision surgical approach was utilized, providing for an intra-articular fusion technique and internal fixation with precise positioning. The surgical technique described avoids any dissection of the hip abductor musculature or a deforming osteotomy of the proximal femur. Twelve complications occurred in 10 patients, 9 of which required additional operative treatment. At an average postoperative follow-up of 6 years, 10 months, the overall activity level was greatly increased over the preoperative activity level secondary to the relief of pain. Hip
