Because the etiopathogenesis of the Waldeyer's ring hypertrophy is not fully recognized, evaluation of adrenal cortex function in this disorders was made. Serum cortisol level was determined by sulfuric acid fluorescence under resting conditions and 3 hours after ACTH injection. Investigations were performed on 20 children, aged 5-9 years, in all of them a marked hypertrophy of palatine and pharyngeal tonsils was found. These children were divided into two groups, the first one included 11 children with frequent
angina
and the second group consisted of 9 children with serious difficulties in breathing, but without
angina
. In these groups serum cortisol levels were estimated before and 3-4 months post tonsilectomy or adenotomy. 11 healthy children of the similar age were included into a control group. It was found that in the first group (hypertrophy and frequent
angina
) initial resting and ACTH stimulated cortisol levels were statistically significantly higher than in the control group. In the same group 3-4 months post surgery serum cortisol levels were lower and similar to those found in control group. On the contrary, in children of the second group (hypertrophy without
angina
) the resting and ACTH stimulated serum cortisol levels were lower than in control group and these levels did not change 3-4 months after surgery.

The author describes a 47-year-old patient in whom lingual hemiatrophy developed one month after
angina
. The development of hemiatrophy was preceded by occipital headaches and pain behind the ear on the side of hemiatrophy. In the differential diagnosis the author excluded inflammatory processes neoplasms and developmental anomalies and thinks that the cause of this short-lasting hemiatrophy might have been tonsillitis with compression of the nerve by the oedematous inflamed tissues with action of bacterial toxins on the nerve.

Clinical presentation and course were studies in 45 consecutive patients (p)--39 males, 6 females with angiographically proven left main coronary artery stenosis (LMCA) > 50%. Mean age was 54.7 years. Three (6%) had no history of chest pain, 2 p (4%) had atypical chest pain, and the remaining (90%) had typical
angina pectoris
. 19 p (42%) had unstable angina, 20 p (44%) had suffered a myocardial infarction in the past. Outside an episode of chest pain most of the patients had an abnormal ECG with ST-T segment depression 2 mm or more in leads V3-6 and ST-T elevation in leads V1 and aVR. No significant differences were found when the abnormalities of the ST-T segment were compared to severity of LMCA obstruction. A symptom limited exercise test was performed in 17 (37%) p. It was abnormal in 13 p (29%). Thirty eight patients (85%) underwent bypass surgery and the mean number of bypass graft was 3.3. Seven patients were treated medically. In the surgical group four p (10.5%) died perioperatively. All of them had subtotal occlusion of LMCA, without significant lesions in the remaining coronary arteries, the ejection fraction (EF) was above 66%. Among thirty four living patients thirty have been asymptomatic. In the medically treated group 3 p (42%) died and only two of four survivors were asymptomatic at a mean follow-up 35.7 months. Left ventricle of all died patients were severely damaged (EF mean 28%), right coronary artery (RCA) was totally occluded and all had rythm disturbances. We conclude, that patients with significant LMCA stenosis had a good prognosis when treated surgically.(ABSTRACT TRUNCATED AT 250 WORDS)

We studied interactions between propranolol and nitrates, used in subjects with stable
angina pectoris
on effort. Basal pharmacokinetic indexes were estimated on measured levels of propranolol concentration. It was found, that nitrates used with propranolol at the same time decrease its concentration in serum and bioavailability, and increase propranolol clearance calculated for whole body, its distribution volume and shorten propranolol life half-time in the blood serum.

A pathogenetic outline of oesophageal inflammatory lesions with special emphasis on Barrett's oesophagitis has been presented. As recorded in a history of a 65-year old patient at first the
stenocardia
-mimicking pains occurred a few days after appendectomy. Gastrobulboscopy performed (after coronary insufficiency had been excluded) revealed extensive inflammatory and erosive lesions of the oesophagus and multiple ulcerations of the duodenum. According to the several authors the rapid manifestation of the inflammatory lesions of the oesophagus may be related to the intubation necessary for laparotomy. It may refer particularly to diabetic patients. Control endoscopies showed that a six-week treatment resulted in a total disappearance of mucosal changes. It might be thoughtful to take into consideration a possibility of oesophageal inflammation in clinical differentiation of
stenocardia
mimicking pains in diabetics who underwent the intubation.

Isolated critical ostial stenosis of the left main coronary artery (LMCA) without narrowing in the distal parts of coronary vessels is rather rare cause of
angina
. It was observed in 7 our patients: 5F and 2M aged 42-55 yrs (mean 47.5). Five of them were in unstable condition. In all of them a direct surgical angioplasty of the LMCA was performed. Cardiopulmonary bypass with moderate hypothermia were used in all patients. The LMCA was approached from behind. A curved incision was made into the right lateral aortic wall toward the LMCA. Care was taken to stay away from the commissure between the noncoronary and the left coronary cusp. The posterior aspect of the LMCA was incised across the stenosis and prolonged through bifurcation. A venous onlay patch was used to enlarge not only the LMCA but also the adjacent 2 cm of aortic incision, so as to give the LMCA ostium a funnel shape, which favors a homogeneous blood flow. The mean aortic cross clamping time was 46 min. The patients were easily weaned from cardiopulmonary bypass. The early and late results are good--all patients were discharged from the hospital free of symptoms. In 6 patients a perfect patency of the left main stem was documented during control coronarography. In our opinion direct surgical angioplasty of LMCA is better then the conventional surgical treatment because normal geometry of LMCA ostium and normal blood flow can be restored using this method.

The groups of subjects were submitted to multipolar electrocardiographic recording from chest surface by means of a multi-electrode network (diamentoid) introduced to vectorcardiography by the Kowarzyk method. The QRS complex divided into 8 equal sectors served as the basis for calculating, among other things of the non-ipolarity index (km) and its graphical representation. The aim of the study was to evaluate the clinical usefulness of transesophageal echocardiography in the imaging of the left main coronary artery. The group tested--consisted of 26 patients with
angina pectoris
and 8 controls. The identification and adequate images of the left main coronary artery were obtained in 33 of 34 (97%) patients. In 5 of 26 patients with
angina pectoris
we observed stenosis of the left main coronary artery. The diameter stenosis of the lesion was judged to be greater than 50%. In addition, we tried to asses Doppler-derived parameters of blood flow. We have demonstrated that the pulsed Doppler study of the left main coronary artery is feasible. Comparison of patients with
angina pectoris
with the control group showed lower systolic flow velocities (35 +/- 10 cm/s vs. 55 +/- 15 cm/s) and slightly higher diastolic flow velocities (75 +/- 20 cm/s vs. 70 +/- 10 cm/s). This transesophageal examination holds promise for detecting pathologic vascular status. Although the sensitivity and specificity of this findings cannot be determined by our study, we believe that transesophageal echocardiography can provide a reference for the evaluation of patients with known or suspected ischemic heart disease. Further studies including larger groups of patients are recommended.

The relation between exercise test, echocardiography and clinical course of acute myocardial infarction was investigated. 17-34 days after an acute myocardial infarction, before hospital discharge, 58 patients underwent exercise test and 17-28 days- echocardiography. Low exercise capacity was significant related to
angina
before infarction, maximal CKNAC and complications during clinical course. There was inverted correlation of asynergy index calculated from echocardiography with maximal workload achieved during exercise test.

The aim of this study was to investigate the value of continuous ECG monitoring in early diagnosis of coronary heart disease. 312 patients (59 women, 253 men, aged 20-60 years) with chest pain since 3-6 months were studied. All patients underwent clinical examination, serum lipid determination, exercise ECG test, ambulatory ECG monitoring and coronary angiography. 194 patients had normal or nonsignificant irregularities of coronary arteries, in 118 patients significant atherosclerotic stenoses were found. Sensitivity and specificity of ECG monitoring were: 46 and 54% respectively. Significantly higher frequency of changes and longer duration of depression or elevation of ST segment in patients with significant coronary artery stenosis was found. ST segment changes without
angina
were observed in 23,7% of patients.
