The achillean reflexogram has been recorded according to Gilson's photo-electric technique among two male populations during routine cardio-vascular screening. The first population concerns 706 men aged 21 to 55 and the second one 4 437 subjects aged 46 to 52. The frequency of bilateral areflexy increases with age (from 3% before 30 to 6% after 40); it is significantly associated with the presence of
angina pectoris
. However, no relation is observed with diabetes or clinical suspicion of myocardial infarction. Whereas fat body mass is positively associated with the reflex duration the latter is reduced with heart rate, systolic blooc pressure and number of blood erythrocytes and leucocytes increased. These results and the interest of systematic reflexogram recording during health check up are discussed.

The authors report 25 cases of patients, average age 67 years with severe coronary or valvular heart disease, with conduction disorders. The conduction disorder occurred alone in 8 cases and was associated with a disorder of excitability in 17 cases. It was either obvious, as in 14 cases, or latent, as in 11 cases, and precipitated by various forms of treatment, the disadvantage of which was the negative dromotrope effect. This treatment was prescribed for permanent resting
angina
(amiodarone and prenylamine), heart failure (digitoxin) or excitability disorder (beta-blockaders or procainamide). 11 patients had one or several fainting attacks. Permanent electro-systolic pacing with stimulation on demand, is necessary in all patients to palliate the consequences of treatment. In 11 cases out of 25, prior temporary pacing permitted the authors to assess the efficacy of treatment. The high post-operative mortality (40%) is not due to the apparatus but depends on the severity of the coronary heart disease or heart failure in these patients, In 60% of cases, the result of stimulation was excellent and was maintained permanently.

The authors report 4 cases of hyperlipemia and show the great benefit which results from ileal exclusion when there is an atherogenic risk. The considerable reduction in total lipid, cholesterol, triglycerides, and prebeta-lipoproteins is constant together with clarification of the serum. In one case,
angina pectoris
regressed considerably, as did arteritis of the lower limbs (unlimited walking became possible, oscillometry in the leg improved from 2 to 7). Surgery is indicated whenever by lack of will power, the diet and medical treatment cannot be followed, when social and economic conditions make proper medical treatment impossible, or when the latter has failed. The existence of arterial lesions, cardiac or cerebral complications makes surgery even more urgent. Gall-stones were observed in gall bladder. The authors raise the problem of oxalate stones. Only type II familial hyperlipemia in homozygotes should be excluded: and end-to-side portacaval anastomosis seems to be preferable.

In 34 cases, corresponding to 3,7% of all patients underwent coronary arteriography for evaluation of anginal syndrome in our laboratory, it was observed a congenital coronary anomaly on their angiograms. The purpose of this paper is to verify the frequency of coronary anomalies and to evaluate relations existing between these anomalies and anginal syndrome. Coronary arteriograms, in 22 cases (2,44%), revealed exclusively; the presence of a coronary anomaly without occlusive coronary disease, which might per se justify
angina
. They were distributed as follows : 5 with coronary fistula, 2 with coronary aneurysms, 2 cases with single ostium and finally, 13 subjects with hypoplasia of one of the three major coronary arteries. One infant, 14 months old, had a hyperplasic left discending artery (LDA) draining in right ventricle cavity. Her ECG revealed signs of right ventricle overload. All the other cases had a typical
angina
syndrome with positive stress test. Left cineventriculography demonstrated left ventricle asynergy in 16 patients. We concluded that typical
angina
syndrome in our 22 patients, may be attributable to coronary anomalies observed at their coronary arteriograms.

but with "normal coronarography" results, are not a rare finding during angiographic investigations, more especially in the case of women. The authors review the clinical, electrical, hemodynamic and metabolic characteristics of these cases, and discuss the principal known etiologies, and limits of "normality" with respect to coronary artery angiography results.

30% of diabetics have arterial hypertension. 25% of patients with hypertension are diabetics. The rapid development of atherosclerosis is the main cause of morbidity and mortality among diabetics. Occasionally, prescription of a beta-blocking agent may seem rational in diabetics (hypertension,
angina pectoris
, etc.). But is such a medication acceptable considering the patient's fragile metabolic control? After a brief review of the pharmacology of the various beta-blocking agents, we discuss the choice of the best medication for the diabetic patient. Cardioselective beta-blockers seem to be best suited to diabetics. After insulin-induced hypoglycemia, rise in blood sugar level is less delayed and symptoms of hypoglycemia are less attenuated.

During 2588 coronarography examinations conducted over a period of 4 years, assisted circulation was employed in 63 cases to reduce the risks of the examination procedure in particularly debilitated patients. These high risk cases included 48 patients with unstable angina resistant to medical treatment and 45 cases of recent infarcts with complications. Mortality with assisted circulation was very low, in spite of the severe nature of the affections, but the authors use this technique in only a limited number of cases, mainly because of the risk of lower limb ischemia, and the possibility of using intravenous nitroglycerin and analgesia from neuroleptics for examination. For this reason the number of cases examined in this way has dropped from 4% in 1977 to 2% in 1978, though assisted circulation is still employed in certain particularly severe cases of
angina
, and for infarcts with complications.

Aorto-coronary by pass, widely accepted in the treatment of patients with coronary artery disease is still controversial in the treatment of Prinzmetal variant

. Recurrence of attacks, occlusion of grafts and post-operative infarction were frequent by described and seem to be related with the persistence of coronary spasm. Authors propose the association of myocardial revascularization and coronary denervation by resection of periaortic plexi. Results of 50 surgical plexectomies associated with coronary by pass appear far better than by pass alone (mortality 4%, myocardial necrosis 4%, persistence of attacks 4%).

Forty-six patients presenting typical or non-typical
angina pectoris
were all submitted to analysis of effort
angina
, traditional or automatic, and to analysis of the amplitude of the R wave and to an effort myocardial scan using thallium 201 (taken as reference substance) during maximal muscle exercise. The following conclusions may be drawn from this study. 1) Autoanalysis of the ECG improves considerably the sensitivity without changing the specificity of the method (92 and 82 p. cent for 79 and 82 p. cent). Analysis of the variations in amplitude of the R wave gave no further information. It should be reserved for patients whose effort ECG was difficult to interpret: e.g. left or right bundle branch block, digitalis effect, very abnormal resting ECG, or average age female population.

Bepridil, a new anti-anginal drug, was given in a daily dosage of 400 to 600 mg to twenty patients with unstable angina pectoris. The trial was designed to evaluate the effectiveness of bepridil in this indication and to determine the optimal dosage. A positive response evidenced by the arrest of progression towards infarction was recorded in 17 patients. Tolerance was satisfactory in 19 cases: in one diabetic patient under insulin symptoms of hypoglycemia resolved once insulin was discontinued. With the dosages used the effectiveness of bepridil in unstable angina pectoris is similar to that recorded in effort
angina
with 300 mg per day.
