With this study including 9 patients with angiographically proven coronary heart disease it could be shown that a monotherapy with nifedipine in the galenic preparation as Adalat SL (Bayer AG, Leverkusen, FRG) is efficient in reducing myocardiac ischemia and concomitantly raising working capacity during bicycle ergometry as well after first time application of this drug as after continuous therapy with 1 tablet twice daily during 4 weeks. This findings are true both at normal environmental conditions (1000 ft above sea level) and at conditions of altitude induced hypoxia (9000 ft above sea level), simulated in a hypobaric chamber. There was no development of drug tolerance during continuous therapy with 1 tablet of Adalat SL twice a day. Out of these findings the conclusion can be drawn that in patients with coronary heart disease receiving protective therapy with Adalat SL there is no remarkably higher risk of episodes of angina pectoris or even myocardiac infarction at altitudes up to 9000 ft above sea level than is to be expected during daily life.

Repeated treadmill exercise tests were used in 20 patients with stable exercise-induced angina in order to comparatively examine various dosage forms of isosorbide-5-mononitrate (IMN) and isosorbide dinitrate (ID) during their single administration. The antianginal effect of conventional IMN tablets (Monisid, Bulgaria) lasted about 5 hours and 1-2 hours longer than that shown by long-acting tablets (Olicard, FRG) and lasted on an average of 12 hours. There was a correlation between the antianginal effect of IMN and its blood concentration. A significant individual variability was observed in the potency of all the dosage forms of IMN and ID under study. The dosage form of IMN had antianginal effects that were similar and even superior to those exhibited by ID.

Using clinical and instrumental findings, the therapeutic effectiveness, hemodynamic effects and tolerance of the three nifedipin derivatives adalate (FRG), corinfar (GDR) and fenigidin (USSR) were studied in 51 patients. The data provided by questioning, examinations and bicycle ergometric tests showed an antianginal effect in 68.5% of the patients treated with corinfar and adalate, and in 61% of those given fenigidin. The best response was observed in patients with angina of effort and those with the angiospastic variant of angina pectoris. The drugs were well tolerated and brought about an insignificant decrease in the arterial pressure and the pulse rate attended by no substantial hemodynamic alterations.
