This retrospective case review sought to report the findings of a health screening of 100 international adoptees. Included were the 1st 100 children who attended the Outpatient Inter-Country Adoption Clinic at the Royal Children's Hospital in Melbourne. A medical history was taken and a thorough physical examination and series of screening investigations were performed on all children. The children range in age from 2 months-16 years (median=5 months; mean=2.8 years). The countries most represented were Korea with 36 children and India with 21. 30 children fell below the 3rd Australian centile for both height and weight. Abnormalities on physical examination included hepatosplenomegaly (13 children), scabies (6), and severe dental caries (6). Parasitic infestation of the stools was found in 23 children, a positive Mantoux test result in 9 (3 with active tuberculosis), anemia in 12, elevated hepatic transaminase levels in 9, and positive hepatitis B surface antigen and e antigen in 2. I child had acquired syphilis. Screening for human immunodeficiency virus antibody gave negative results in all children. These international adoptees constitute a special pediatric subgroup which require assessment and screening as soon as possible after arrival in their adoptive country.

Eighteen patients with a variety of refractory solid tumors received infusions of IgM class monoclonal antibody directed against tumor surface antigen and raised in a nonviral transformed human-human system. Three other patients with refractory hematologic or solid tumors had autologous marrow purged with antibody, regrown, and reinfused following high dose chemotherapy. A molecular weight of 200,000 was demonstrated for the whole antibody produced by the fusion line. Each antibody failed to react with nonspecific tumor tissue. No fetal antigen reactivity was noted. Antibody was cytotoxic. Complement dependence was not demonstrated. Chloroquine phosphate pretreatment was employed prior to each antibody infusion to block receptor recycling and host-antigen processing. Antibody dose was doubled with each treatment. Self-limiting chill and fever (with tachycardia) was noted in 12-hour infusions. Tumor biopsy following infusion demonstrated good binding for as long as 48 hours after dosing. Tumor regression was noted during infusions. Pulmonary metastases responded rapidly following infusion. Maximal shrinkage of skin metastases was noted at 4 days with some regrowth at 7 days. Reduction in size of liver metastases was the most durable response. Responses correlated with survival. There were no antibody-related deaths. Antibody used ex vivo to purge autologous bone marrow-cleared marrow of tumor. Marrow engraftment was noted between 7 and 21 days. Follow-up showed no antihuman antibody formation, alteration in renal or hepatic function, alteration in thyroid or adrenal function, or circulating antigen-antibody complexes. Natural killer cell activity and complement-activated clearing of immune complexes increased following infusion. The time for complete clearing of immune complexes from the circulation was 10 days. Mild anemia was seen after several courses of infusion. Reticulocytosis was unimpaired. Long-term follow-up showed no antihuman antibody formation or organ dysfunction.

The iron status of 203 Zairian pregnant women--38 with chronic hepatitis B virus (HBV) infection (HBsAg[+]), 94 with antibodies to the surface antigen (Anti-HBs[+]), and 71 without HBV markers (HBsAg[-]/Anti-HBs[-]) -- was assessed. Participants ranged in age from 15 to 42 and had parities of 1-12; they were recruited from Mama Yemo Hospital in the summer of 1983. Hemoglobin (Hb), serum iron, total iron binding capacity, and transferring saturation (TS) were determined by standard techniques and serum ferritin (FERR) by radioimmunoassay. To rule out inflammation and/or infection which increase FERR levels, C-reactive protein (CRP) and alpha1-acid glycoprotein (AGP) were also measured. There was no significant difference in the mean levels of any of the hematologic measurements, FERR, CRP, and AGP between the 3 HBV groups. Women who took iron supplements had slightly higher mean levels of Hb but no serum FERR or TS than those who did not. Women with inflammation and identical HBV markers had higher mean FERR levels than those without inflammation. Neither the prevalence of anemia, which varied between 32-35%, not that of iron deficiency, which varied between 52-59%, differed significantly between the 3 groups of women. It is concluded that in pregnant women, chronic asymptomatic HBV infection is not associated with a lower prevalence of iron deficiency and/or anemia.

We describe a staining technique, using Ponceau S in very mild conditions, by which proteins can be visualized on nitrocellulose replicas without being permanently fixed to the membrane itself, thus allowing subsequent procedures such as immunoblotting or preparative elution of the proteins to be performed. This staining technique can detect 250 to 500 ng protein, which is essentially the same sensitivity seen for Coomassie blue staining of proteins on nitrocellulose. The Ponceau S staining technique was used to locate proteins on nitrocellulose replicas for subsequent in situ radioiodination and trypsin digestion, followed by separation of the resultant digests in two-dimensional peptide analysis. Staining proteins with Ponceau S did not interfere with either the radioiodination or trypsin digestion, as indicated by essentially identical peptide patterns being obtained for the internal protein p26 from equine infectious anemia virus, regardless of whether the digests were prepared from polyacrylamide gel slices or nitrocellulose sections. The combination of preparation of radioiodinated tryptic digests on nitrocellulose and subsequent two-dimensional analysis is sensitive enough to detect peptide additions and deletions occurring in the surface antigen gp90 recovered from two antigenically distinct strains of equine infectious anemia virus. Thus these procedures provide a relatively simple, inexpensive, and highly reproducible technique for the analysis of as little as 250 nanograms of protein after separation by electrophoresis in polyacrylamide gels.

A 43-year-old male with a phenotypically homogeneous, expanded subset of T cells presented in 1981 with anemia and neutropenia. The surface antigen phenotype of 99% of the peripheral blood lymphocytes was T3+, T8+, T4-, and they were morphologically large granular lymphocytes (LGL). The same cells comprised 37% of the marrow nucleated cells. Eight months after he presented, the peripheral blood T8+, LGL diminished spontaneously, and the anemia and neutropenia completely resolved. The patient remains hematologically normal as of October 1984. To determine if the T8+, LGL represented a clonal expansion, DNA from peripheral blood lymphocytes collected and cryopreserved when the patient was neutropenic and anemic, and when he was hematologically normal, was analyzed for clonal T-cell antigen receptor gene rearrangements. Using Southern blot analysis, a clonal DNA rearrangement was demonstrated, and this clone diminished but was still demonstrable in peripheral blood lymphocytes collected in 1984. The above observations implicate the expanded T8+, LGL in the pathogenesis of the neutropenia and anemia, yet the exact mechanism remains to be elucidated.

Although 50% of Indochinese refugees are under 18 years of age, previous studies have emphasized the prevalence of parasites, anemia, tuberculosis, and hepatitis, with few addressing age-related health care needs. In this study the specific health care needs of 80 Indochinese refugee teenagers, evaluated during a 4-year period, were determined. The Centers for Disease Control's suggested screening measures were used, and it was found that 52% had positive purified protein derivative skin tests, 38% lacked immunizations, 35% had stool specimens positive for parasites (prevalence and number of parasites greatest among Cambodians), 14% had blood tests positive for hepatitis B surface antigen, and 10% were anemic. Additional evaluations showed that 19% had hemoglobinopathies, 14% were in or below the fifth percentile for height and weight, 12% had goiters, 12% had skin disorders, 8% had positive hepatitis B surface antigen, 5% had visual defects, 5% had hearing loss, 5% had psychosomatic illness, and 4% had idiopathic scoliosis. Although suggested Centers for Disease Control screening measures may be adequate for younger Indochinese children, these data suggest that additional studies are necessary for teenagers. For the sexually active adolescent, identification of and counseling for hepatitis antigenemia and hemoglobinopathies are crucial. In addition, early identification of emotional and physical problems during screening may enhance assimilation into a new society and facilitate completion of the psychosocial tasks of adolescence.

There are reports in the literature that infection with hepatitis A virus in hepatitis B carriers can result in resolution of the carrier state. In an attempt to induce clearance of the carrier state of hepatitis B virus in two persistently infected chimpanzees, the chimpanzees were infused with documented non-A, non-B infectious material. Biochemical and histopathological evidence of hepatitis was accompanied by the unique abnormalities of endoplasmic reticulum associated with non-A, non-B hepatitis in the chimpanzees. Elevation of alanine aminotransferase was accompanied by fourfold reduction in one chimpanzee and sixfold reduction in the other in the plasma levels of HBV-associated DNA polymerase activity and simultaneously by twofold reduction in the concentration of hepatitis B surface antigen in both chimpanzees. A mediator may account for these changes in markers of hepatitis B virus infection, and this mechanism may also explain the occurrence of spontaneous regression in some persistently infected carriers. The significance of transient red cell anaemia in non-A, non-B hepatitis, which was observed in one of the chimpanzees, is yet to be established.

Findings on health screening of immigrant Southeast Asian adolescents enrolled at a Federally funded vocational training facility were reviewed for the years 1979-1982. History of or positive skin test for tuberculosis was found in 55%. A heavy parasite burden was found in 46% of the recent Laotian immigrants, but was infrequent in Vietnamese who had been in the United States for several months before testing. Anemia was found in only 5.5%, though an additional 6.5% had findings suggestive of Thalasemia trait. Hepatitis B surface antigen positivity occurred in 16.8% but none had abnormal liver enzymes. One case of leprosy and three cases of idiopathic microscopic hematuria were detected.

Forty-one Egyptian schoolchildren (36 boys and five girls; age, 12-16 years) who were heavily infected with Schistosoma mansoni were studied. Symptomatic subjects had swimmer's itch and hematochezia. Hepatomegaly was found in 39 and splenomegaly in 31 children. All subjects had eosinophilia, 13 had anemia, 31 had elevated levels of serum globulins, and nine had elevated levels of alkaline phosphatase. All but one subject had antibody to hepatitis A virus, and 26 had antibody to hepatitis B core antigen, antibody to hepatitis B surface antigen, or both. Oxamniquine was given in single daily doses of 20 mg/kg of body weight for either two or three days and cured 50% and 85%, respectively, of subjects treated; ova excretion was reduced by 86%-93% for up to 12 months. Morbidity was associated with heavy S. mansoni infection. Therapy with oxamniquine was safe and efficacious.

A patient with leukemic reticuloendotheliosis had splenomegaly, neutropenia, and a severe underproduction anemia. During a three-year period, the hematocrit was never in the normal range, and periodic transfusions were required. However, after an episode of hepatitis that was positive for B surface antigen, the spleen became smaller, the number of neutrophils increased, the transfusion requirement disappeared, and the hematocrit rose to normal. Several mechanisms for this observation are proposed.
