In a phase II clinical trial to test the ability of recombinant human erythropoietin (r-HuEPO) to reverse the anemia of patients undergoing hemodialysis, the changes of enzyme activity in red blood cells were evaluated in 5 hemodialysis anemic patients who were treated with r-HuEPO. Concerning the activity levels measured, the following conclusions are drawn. 1) HK, ALD, TPI, G6PD and 6PGD were statistically significantly increased at the time when the hematocrit has risen by 8% with the use of r-HuEPO. 2) The enzyme activity levels of PFK, GA3PD, MPGM, ENOL, PK, GR and ADA were higher than normal already before the r-HuEPO treatment. 3) The increases of HK and G6PD by r-HuEPO, as age dependent enzymes, may reflect the generation of young red blood cells. 4) In view of the fact that they are related to ATP production in the glycolysis cycle, we infer that increases of red blood cell enzymes by r-HuEPO may play at least some part in bringing a sensation of "well-being" to severely anemic patients undergoing hemodialysis.

Changes of the density, MCV, MCHC, RNA-concentration and the activities of the enzyme G6PD and ASAT due to maturation and aging of rbc were investigated by repeated density gradient separation of these cells during a bleeding anaemia of the rabbit. The mean density of reticulocytes and erythrocytes decreases during the course of the anaemia by 14 and 10 g/l respectively. The difference between the density of both cell types increases from 3 to 6 to 12 g/l during the anaemia. Maturation and aging of rbc are characterized by an increase of the MCHC from 200 to 350 g/l cells and the decrease of MCV by 10-25%. The RNA concentration and the ASAT activity show an almost parallel behaviour in cells of different densities during the anaemia. Remarkable differences are observed between the ASAT activity and the G6PD activity: The G6PD activity decreases mainly during the aging process and almost not during the maturation, while the ASAT activity falls constantly during maturation and aging. As observed in former studies we could distinguish and isolate 4 characteristic populations of reticulocytes in certain density fractions during the anaemia: Normal young reticulocytes, line 2 reticulocytes, megaloreticulocytes and reticulocytes of high density.

A child suffering from G6PD deficiency developed a severe haemolytic crisis without an apparent trigger. The possible pathogenetic role of the ingestion of unripe peaches was studied biochemically in this anaemia. We show that an extract from the unripe peach exerts an oxidative challenge on normal as well as on asymptomatic G6PD-deficient erythrocytes. This effect is analogous to that of the favism-inducing agents. The effect of the extract on the patient's red blood cells was more pronounced than on other asymptomatic G6PD-deficient erythrocytes, particularly during his haemolytic crisis. The chemical nature of the deleterious component was not identified. It is suggested that unripe peaches be added to the list of hazards for G6PD-deficient subjects in combination with other factors.

A new G6PD variant, designated Gd (+) Laguna, was found in a 9-year-old Brazilian boy of Portuguese ancestry suffering from an iron-refractory anemia. The red cell enzyme activity of the subject was 64%. The mutant enzyme showed slower electrophoretic mobility, increased affinity for glucose-6-phosphate, decreased affinity for NADP+, elevated utilization of substrate analogues, decreased inhibition of NADPH, normal heat stability and a biphasic pH curve. The occurrence of the variant in two non-anemic relatives of the propositus indicates that the association between this G6PD type and anemia may be coincidental.

The anaemia was induced by injection of PHH in guinea-pig and rabbit and also by bleeding in guinea-pig. 3. Red cell GSH increased from 115.5 to 141.8 mg/dl RBC in the guinea-pig and from 108.5 to 133.5 mg/dl RBC in the rabbit during anaemia. 4. There were significant rises in the level of all the three enzymes activities in both the species of animals. However, the changes in rabbit were more pronounced than in the guinea-pig.

A case of viral hepatitis A with G6PD deficiency is described. The condition should be suspected in the presence of indirect hyperbilirubinemia, and anemia in a patient with viral hepatitis; repeat G6PD testing 8 weeks later is recommended for diagnosis.

Transfusion-acquired malaria in a neonate is uncommon and factors such as drug resistance and concomitant G6PD deficiency can cause treatment difficulties. We report a 26-day-old premature infant with chloroquine-resistant malaria who underwent exchange transfusion. The aim was to correct anaemia, decrease parasitaemia and remove G6PD-deficient cells to allow successful use of quinine.

The aim of this study was to determine the frequency of G6PD deficiency and assess its impact on morbidity, especially anemia, in preschool-aged children in Cambodia. A total of 151 children including 82 boys and 69 girls from the Kandal province near Phnom Penh were studied. Ages ranged from 8 to 69 months. Blood was collected in EDTA-coated tubes. Blood counts were performed with an ABX Micros 60 system and G6PD in red blood cells was measured with a Roche Cobas Mira Plus system using Gamma reagents. G6PD deficiency was found in 14 cases (13.4% of boys and 4.3% of girls). Deficiency was complete in 7.3% of children and partial in 2%. Anemia defined as hemoglobin concentration less than 110 g/l was detected in 29.1% of children. No case of anemia could be attributed to enzyme deficiency since no sign of hemolysis was observed in any of the three children presenting both conditions. Further study is needed on G6PD deficiency in Cambodia including malaria-endemic areas and on the frequency and severity of jaundice due to enzyme deficiency in newborns.

Dapsone, used for prevention of Pneumocystis jirovecii infections, has been reported to cause hemolytic anemia and methemoglobinemia; its tolerability in solid organ transplant recipients is not well described. We investigated dapsone-related adverse events in patients undergoing solid organ transplantation from 1999 to 2004. Transplant providers identified patients for the investigators who then reviewed the patients' hospital and outpatient records. Sixteen solid organ transplant recipients fit case definitions for dapsone-related hemolytic anemia (n = 11) or methemoglobinemia (n = 5). Median time from event to dapsone discontinuation was 15 days; all patients improved after drug discontinuation. G6PD enzyme activity was normal in all patients whose test results were available. Dapsone may be associated with hemolytic anemia or methemoglobinemia, even with normal G6PD levels. These events are often not promptly recognized, and drug discontinuation is delayed. Dapsone-related hemolytic anemia or methemoglobinemia should be considered in solid organ transplant recipients with unexplained anemia or hypoxia.

To assess the antioxidant status and oxidative stress in bovine theileriosis due to Theileria annulata blood samples were collected from 35 clinically affected cattle referred to Veterinary Teaching Hospital, School of Veterinary Medicine, Urmia University, Urmia, Iran. Complete blood count, piroplasm parasitemia percentage, erythrocyte glutathione peroxidase, superoxide dismutase, catalase and glucose-6-phosphate dehydrogenase activities, malondialdehyde concentration, osmotic fragility test and median corpuscular fragility were determined and the results were compared with those of 50 healthy controls. Of 35 affected cattle, 12 (34.28%) had severe anemia and 23 had mild to moderate anemia and parasitemia varied from 5 to 40%. The activities of erythrocyte glutathione peroxidase, superoxide dismutase and glucose-6-phosphate dehydrogenase were significantly lower (P<0.0001) and the activity of catalase was significantly higher in the affected cattle than in healthy ones (P<0.001). Malondialdehyde concentration in erythrocytes of affected cattle was significantly more than those of healthy cattle (P<0.001). The affected cattle showed increased fragility of erythrocytes, so that median corpuscular fragility (MCF) in affected group was significantly lower than those of healthy group (P<0.0001). Median corpuscular fragility showed a positive correlation with the severity of parasitemia (r=0.81, P<0.0005) and a negative correlation with the activities of GSH-Px (r=-0.78, P<0.0001), SOD (r=-0.71, P<0.0005), catalase (r=-0.53, P<0.018) and G6PD (r=-0.58, P<0.0005). The results of this study suggest that oxidative damage to RBCs may contribute to the pathogenesis of anemia in bovine theileriosis.
