Utilizing the stathmokinetic principle of timed vincristine and bleomycin, we combined these two agents with Mitomycin-C. The dose schedule included vincristine 0.5 mg/m2 intravenously (i.v.) geginning on day 1 and repeated twice weekly for 12 weeks; each injection was followed in 6-12 hours by bleomycin 6 mg/m2 for 12 weeks. Mitomycin-C was administered as a 20 mg/m2 bolus beginning on day 2 and repeated at 6-week intervals. Thirty patients were entered into this study, 27 were fully available for response. Thirteen patients (48%) met criteria of response (greater than 50% reduction in volume of measurable tumor). Significant myelosuppression resulted from this therapy. Median leukopenia nadir was 3.8 X 10(3) cells/mm3 and median thrombocytopenia nadir was 116 X 10(3) cells/mm3. Additional toxic reactions included anemia, lassitude, anorexia, peripheral neuropath fever, and skin rash. Despite significant, but manageable, toxicity, this combination appears to represent an improvement in the chemotherapy of a traditionaly refractory solid tumor.

Parenteral and enteral nutrition are being used as adjuncts to cancer therapy. A liquid diet formulation containing a 27% solution of glucose and 3.9% crystalline amino acids with electrolytes and vitamins was given continuously for a week via parenteral (iv), and via intragastric (ig) routes and also was given ad libitum via the oral or per os (po) route to groups of Buffalo rats with and without a Morris No. 7777 transplantable hepatoma to find out how these feeding procedures affect tumor-host interactions. Other groups of rats with and without the hepatoma were given solid food ad libitum. The following parameters were examined: mortality, carcass and organ weights, body and tumor growth, nitrogen balance, energy intake, fluid balance, urinalysis, hematology values, and serum protein levels. The results are considered with respect to the influence of the tumor on the host and the influence of the feeding procedure on the animal with and without a tumor. The presence of the hepatoma was associated with: higher mortality, a decrease in carcass mass, leucocytosis, anemia, a decrease in serum IgG, transferrin and albumin, and an increase in serum alpha fetoprotein. The iv and ig feeding procedures alone resulted in some mortality which was exacerbated by the presence of the tumor. Mortality was especially high in the tumorous rats on the ig feeding procedure. The degree of positive nitrogen balance and carcass mass was similar in non-tumorous rats fed the same liquid diet formula when given iv, ig, or po. Tumorous rats fed the liquid diet ad libitum showed anorexia and a significantly lower nitrogen balance. The iv and ig feeding of tumorous rats at a level which was well above those of the tumorous rats given solid or liquid diet ad libitum maintained the same degree of positive nitrogen balance as non-tumorous rats. Even though the iv feeding of tumorous rats maintained about the same degree of positive nitrogen balance as non-tumorous rats, these tumorous rats still suffered loss of carcass mass. It appears that the large rapidly growing hepatoma has priority for available nutrition over the host. It is further suggested that the rapidly growing hepatoma places an ever increasing demand on the available nutrients. Thus, a point is eventually reached where even supplemental nutritional support can no longer meet the needs of the growing hepatoma and the host.

Neocarzinostatin (NCZ), an acidic polypeptide antibiotic, was given to 47 patients with cancer and leukemia, and tolerance to two schedules, a single dose given as a 2 hour infusion and a continuous infusion over 5 days was investigated. Immediate reactions, including fever, chills, rigor, hypertension and mental confusion, were dose-limiting for the 2 hour infusion schedule, occurring at 3000 U/m2 and higher. Continuous administration for 5 days eliminated the immediate reactions and then hematological toxicity--often prolonged leukopenia and thrombocytopenia--became dose-limiting. Other toxicities of NCZ at both dose schedules included anemia, fever and chills, anorexia, nausea and vomiting, hepatic dysfunction, azotemia, hypophosphatemia, aminoaciduria, stomatitis, phlebitis and/or cellulitis at the venous infusion site and pruritus. Patients with solid tumors who had received little or no prior chemotherapy and had good bone marrow reserve tolerated up to 6000 U/m2/24 hours X 5 days. One patient with previously treated acute myelocytic leukemia was induced into a good partial remission lasting 10 weeks.

Current chemotherapy of malignant brain tumor bases on cell kinetics. Chemotherapeutic agents are devided into two, cell cycle specific (CCS) and cell cycle non specific (CCNS) agents. A case of malignant glioma successfully treated by chemo-radiotherapy using a new combination of the two agents , Carboquone (CQ) as CCNS, which has not appeared in literature, and FT-207 as CCS is reported. A malignant glioma in the right frontal lobe in a case of 51-year-old male was removed subtotaly on Dec. 10th, 1971 in our clinic. Three years and five months after the surgery, the patient was diagnosed as having a recurred malignant glioma in the left frontal lobe from the clinical symptoms. This was supported by a positive brain scan and carotid angiography. A total dose of 57mg of CQ was continuously into the left internal carotid artery during two months. Simultaneously, 16g of FT-207 as a total dose was given orally and 4,550 rads of Telecobalt-60 were irradiated. One month after the beginning of these treatments, clinical symptoms improved obviously. Four months later, the size of the tumor shadow on the brain scan decreased remarkably and the shifted anterior cerebral artery returned to normal position on the carotid angiogram. Anemia, leucopenia, thrombocytopenia, nausea, and anorexia were the side-effects of these treatments. But these complications disappeared six weeks after the termination of the treatments.

Experimental infections of Babesia microti in laboratory-reared Clethrionomys glareolus revealed that approximately 15% of the erythrocytes were infected with single ring forms during peak parasitemia. Infected erythrocytes could be detected in blood smears up to one month post infection. C. glareolus treated with a single injection of Depo-Medrol i.m. two days prior to infection displayed a four-fold increase in number of infected erythrocytes at peak parasitemia, 35% of which contained more than one Babesia, and a prolongation of the infection. B. microti infections in 35 laboratory--reared Lemmus lemmus were fatal. Multiple invasion of erythrocytes, anemia, icterus, hemoglobinuria, anorexia and weight loss, and adrenal and splenic hypertrophy were characteristic for B. microti infections in Norwegian lemmings.

Cancer cachexia is characterized clinically by anorexia, early satiety, weight loss. anemia, and marked asthenia. The syndrome is not the result of semistarvation alone but it represents a complex metabolic problem. In the host there are abnormalities in metabolism of energy, carbohydrate, lipid and protein, in water content, in acid-base balance, in electrolyte, mineral and vitamin concentrations, alterations in the activity of host tissue enzymes and changes in endocrine homeostasis and immunologic mechanisms. The cancer initiates and contributes to the genesis of the syndrome but complications of the disease and the treatment may also play a role. Only the control of the cancer can reverse completely the syndrome. It was proposed that cancer peptides throw the host metabolism into a chaotic biochemical state by activating and inactivating host enzymes. This results in increased energy expenditure; the released host metabolites and trapped by the growing cancer.

In 8- and 12-week feeding trials, channel catfish fingerlings were fed purified diets containing four levels (0, 10, 30 and 60 mg/kg) and six levels (0, 5, 10, 20, 40 and 80 mg/kg) of supplemental calcium pantothenate, respectively. The dietary pantothenate level required for maximal growth, feed conversion and prevention of gross deficiency signs was approximately 10 mg/kg of diet. Fish fed the unsupplemented diet showed severe anorexia, loss of weight, clubbed gills, anemia, high mortality rates and eroded skins, lower jaws, fins and barbels. Fused gill filaments were not obtained as reported in pantothenate deficient salmonids.

Bacterial endocarditis is an elusive disease that challenges clinicians' diagnostic capabilities. Because it can present with various combinations of extravalvular signs and symptoms, the underlying primary disease can go unnoticed.A review of the various extracardiac manifestations of bacterial endocarditis suggests three main patterns by which the valvular infection can be obscured. (1) A major clinical event may be so dramatic that subtle evidence of endocarditis is overlooked. The rupture of a mycotic aneurysm may simulate a subarachnoid hemorrhage from a congenital aneurysm. (2) The symptoms of bacterial endocarditis may be constitutional complaints easily attributable to a routine, trivial illness. Symptoms of low-grade fever, myalgias, back pain and anorexia may mimic a viral syndrome. (3) Endocarditis poses a difficult diagnostic dilemma when it generates constellations of findings that are classic for other disorders. Complaints of arthritis and arthralgias accompanied by hematuria and antinuclear antibody may suggest systemic lupus erythematosus; a renal biopsy study showing diffuse proliferative glomerulonephritis may support this diagnosis. The combination of fever, petechiae, altered mental status, thrombocytopenia, azotemia and anemia may promote the diagnosis of thrombotic thrombocytopenic purpura. When the protean guises of bacterial endocarditis create these clinical difficulties, errors in diagnosis occur and appropriate therapy is delayed. Keen awareness of the varied disease presentations will improve success in managing endocarditis by fostering rapid diagnosis and prompt therapy.

Twenty five cattle were infected with T. vivax, 25 with T. congolense, and 25 served as controls. Pathogenic clinical signs of trypanosomal infection were not observed. Secondary bacterial infections were common. Fever, increased heart and respiratory rates, anorexia, and emaciation developed. Elevations in parasitemai and body temperature were positively correlated in the T. vivax group. Infected groups were affected non-uniformly, with some animals in each group remaining asymptomatic and tending to have lower parasitemias. The T. vivax parasitemia was cyclic and the organisms had a genaration time of 7.9 SD 2.5 hours. The first peak of parasitemia in both infections was closely associated with the development of pancytopenia, i.e. anemia, leukopenia, and thrombocytopenia. The bone marrow erythroid response in the T. congolense group was significantly greater thn that in either the T. vivax or control groups. Leukopenia was due to concomitant neutropenia and lymphopenia.

Canine ehrlichiosis was successfully transmitted from the domestic dog to three Wild Dogs Lycaon pictus and three Black-backed Jackals Canis mesomelas. Wild Dogs showed symptoms of anorexia and depression as well as anaemia, leucopaenia and mild thrombocytopaenia. Black-backed Jackals were asymptomatic. Morulae of Ehrlichicia canis were found in peripheral blood smears from all experimental animals. The disease was also successfully transmitted from Black-backed Jackal to the domestic dog.
