Indices of nutritional state were measured in 105 surgical patients. The indices were chosen to give information on protein-calorie malnutrition,

, vitamin deficiency. Abnormal values for the various indices were common in the group as a whole and most frequent (50%) in patients who were still in hospital more than a week after major surgery. These patients had a high frequency of

, vitamin deficiency, weight-loss, loss of arm-muscle bulk, and low plasma levels of

and albumin. These abnormalities had gone almost entirely unrecognised, even in patients with sepsis after major surgery, who would benefit from improvement in nutritional state.

Parenteral and enteral nutrition are being used as adjuncts to cancer therapy. A liquid diet formulation containing a 27% solution of glucose and 3.9% crystalline amino acids with electrolytes and vitamins was given continuously for a week via parenteral (iv), and via intragastric (ig) routes and also was given ad libitum via the oral or per os (po) route to groups of Buffalo rats with and without a Morris No. 7777 transplantable hepatoma to find out how these feeding procedures affect tumor-host interactions. Other groups of rats with and without the hepatoma were given solid food ad libitum. The following parameters were examined: mortality, carcass and organ weights, body and tumor growth, nitrogen balance, energy intake, fluid balance, urinalysis, hematology values, and serum protein levels. The results are considered with respect to the influence of the tumor on the host and the influence of the feeding procedure on the animal with and without a tumor. The presence of the hepatoma was associated with: higher mortality, a decrease in carcass mass, leucocytosis,
anemia
, a decrease in serum IgG,
transferrin
and albumin, and an increase in serum alpha fetoprotein. The iv and ig feeding procedures alone resulted in some mortality which was exacerbated by the presence of the tumor. Mortality was especially high in the tumorous rats on the ig feeding procedure. The degree of positive nitrogen balance and carcass mass was similar in non-tumorous rats fed the same liquid diet formula when given iv, ig, or po. Tumorous rats fed the liquid diet ad libitum showed anorexia and a significantly lower nitrogen balance. The iv and ig feeding of tumorous rats at a level which was well above those of the tumorous rats given solid or liquid diet ad libitum maintained the same degree of positive nitrogen balance as non-tumorous rats. Even though the iv feeding of tumorous rats maintained about the same degree of positive nitrogen balance as non-tumorous rats, these tumorous rats still suffered loss of carcass mass. It appears that the large rapidly growing hepatoma has priority for available nutrition over the host. It is further suggested that the rapidly growing hepatoma places an ever increasing demand on the available nutrients. Thus, a point is eventually reached where even supplemental nutritional support can no longer meet the needs of the growing hepatoma and the host.

The concentration of iron (III)-
transferrin
(IT) in whole blood and serum, along with another high-spin (five unpaired electrons) iron complex (probably IT) accumulated by tumor tissue, was investigated by electron paramagnetic resonance (EPR) spectroscopy during the development of Murphy-Sturm rat lymphosarcoma. The observed changes in concentration (microgram/ml) of IT in sera/blood were generally complementary to those from tissue and the character of the modifications suggested the existence of three distinct phases of systemic response to the implantation: (1) an initial response, evidenced by a sharp reduction in serum IT and somewhat high tissue-IT concentration (microgram/g); (2) a period in which the tumor is (2) a period in which the tumor is becoming established, indicated by relatively constant tissue IT levels and near normal serum IT; and (3) the onset of rapid cell multiplication, characterized by increased total tissue-IT accumulation that rises to above 200% of normal available serum iron, increasing tissue-IT concentration, and rapidly declining serum-IT concentration. The results suggest that, in the face of an implanted tumor there are two detectable abnormal serum-IT responses: (1) an initial change, probably due to systemic blockage of iron reutilization; and (2) extraction of IT from serum by multiplying tumor cells, which is probably a major contributor to reduced serum-IT levels and ultimately

The duodenal mucosa of genotypically normal iron replete and iron deficient mice and mice with sex-linked (sla) and microcytic anemias (mk) was examined for the presence of iron-binding proteins. Following continuous, 15 or 120 minute, in vivo intraenteric exposure of a closed duodenal loop to 59Fe, a high speed supernatant of homogenized mucosal tissue was chromatographed on G-200 Sephadex. Two major peaks of 59Fe activity were observed. The molecular weight, and immunological properties of peak I were similar to ferritin whilst those of peak II were similar to
transferrin
. The distribution of 59Fe between peaks I and II in mk/mk animals was similar to that in genotypically normal iron deficient animals indicating that the intramucosal mechanisms for iron transport were reacting appropriately to the iron deficient state of mice with microcytic anemia. In contrast, the distribution of 59Fe between peaks I and II in sla/Y animals was the reverse of that found in genotypically normal iron deficient animals suggesting the possibility of an intramucosal iron binding protein defect in sex-linked

was studied in the carcinoma of the human respiratory tract and in the Morris hepatoma 5123 C of the rat. In both types of tumors a positive tumor imaging by 67Ga-

scintigraphy was evident. The intracellular distribution pattern of the radioactivity showed that the incorporated 67Ga-

is accumulated within the lysosomes of the tumors. The uptake of 67Ga-

by the tumor cells resulted in a faster disappearance of the tracer from the blood. The accelerated disappearance of 67Ga-
transferrin
from the blood showed a direct correlation to the mass of tumor cells. The loss of circulating 67Ga-
transferrin
from the blood showed a close parallelism to the grade of the
anemia
observed in the tumor bearing rats. We conclude from these findings that the uptake of
transferrin
into the tumor cells is one of the factors which are responsible for the

observed in malignant diseases.

The special supplemental food program for women, infants, and children administered by the United States Department of Agriculture, was evaluated nationally. Participating infants, children under 4 years old, and pregnant and nursing women were investigated initially, and after receiving food supplements. The supplements were iron-fortified infant formula, iron-fortified infant cereals, and fruit juices for the infants, and milk, cheese, iron-fortified cereals, eggs, and fruit juices for the children and women. Initially, the average birth weight was lower and the infant mortality rate was higher than expected in a well nourished population. There was also evidence of slight growth retardation, a high
anemia
rate, and a high percentage of participants having saturation of
transferrin
values less 15%. The program had no effect on the prevalence of unsatisfactory values for saturation of

. There was an increase in weight gain during pregnancy, and increase in birth weight, an acceleration of growth, and a reduction in the

1. An epidemiological study of iron status in general practice has been undertaken. An age-sex register was established and a 10% stratified random sample (194 males and 220 females) of the patients above the age of 15 years was studied. A full blood count, serum Fe concentration,
transferrin
saturation and serum ferritin concentration were measured and correlated with clinical features associated with Fe deficiency. 2. The geometric mean for serum ferritin for males was 77 micrograms/l (20-196 micrograms/l) and for females 37 micrograms/l (8--177 micrograms/l). A low serum ferritin (male less than 30 micrograms/l, female less than 20 micrograms/l) in the absence of
anaemia
was found in fifteen males and thirty-five females. In this group forty patients had clinical features which are aetiologically associated with Fe deficiency. The incidence of similar features in a control group of thirty-three patients was seven. 3. It is suggested that this is indirect evidence that serum ferritin concentration is the most sensitive monitor of Fe status enabling the detection of pre-anaemic Fe deficiency.
