CT-documentation of skeletal muscular lesions caused by neuromuscular diseases implies an essential contribution to conventional techniques in the macroscopic field. Size, distribution and degree of lesions as well as compensatory mechanisms are proved thereby. We report about the different effects on muscle appearance referring to 106 patients of our own experience in amyotrophic lateral sclerosis, spinal muscular atrophy, poliomyelitis, polyradiculitis, polyneuropathy as well as peripheral traumatic nerve lesions.

The peripheral nervous system was stimulated over the vertebral column by brief, high-voltage condenser discharges. Potentials from proximal and distal muscles were recorded. We examined healthy subjects (N = 28), patients suffering from ALS (N = 5), from polyradiculitis (N = 6), and polyneuropathies (N = 8). In ALS latencies were slightly (in most cases less than 20%), in polyradiculitis latencies were markedly increased (30-40% on average). The latencies of patients suffering from polyneuropathies showed a variable pattern of vulnerability. Moreover, we could show that this method is frequently capable to quantify deficits in the peripheral nervous system when F-waves are absent.

Neuromuscular diseases occur in as many as 50% of patients infected with human immunodeficiency virus type 1 (HIV-1). All forms of neuromuscular disease have been reported, including axonal neuropathy, demyelinating neuropathy, mononeuropathy multiplex, polyradiculitis, ALS-like syndromes, disorders of neuromuscular transmission, myopathy, and toxic neuropathies due to medication side effects. Neuromuscular disease is often the presenting manifestation of HIV-1 infection. Infection with cytomegalovirus (CMV) is associated with different types of neuropathy including mononeuritis multiplex and polyradiculopathy. There is effective treatment for many of the associated disorders including chronic inflammatory demyelinating neuropathy, CMV-mediated neuropathies, and myopathy. Treatment of CMV-mediated mononeuropathy multiplex may be life saving. The different neuromuscular syndromes associated with different stages of HIV-1 infection may be due, in part, to different levels of immunocompetence.
