The article presents a review of the evolution of indications for laparosocpy during the years 1973-1977. Laparoscopy is always indicated in case of chronic pelvic pains, ovarian malignant tumors, tubal infertility,

or ectopic pregnancy. In such cases the accurate inspection of the pelvic cavity can allow precise diagnosis and therapy. Laparoscopy can also be repeated to control the effectiveness of therapy, especially in the surveillance of ovarian cancers. On the other hand, laparoscopy should not be systematically performed in cases of isolated pelvic pains, in case of
ovarian cyst
or fibroma, or in case of amenorrhea, where clinical examinations and biological explorations are sufficient to determine diagnosis and therapy. Laparoscopy should be used only when there are contraindications among biological findings, or in cases of therapeutic failures.

Experience is recorded in the treatment of acute gynecological diseases based on a series of 61 patients operated in the Clinic of Propedeutics of Surgical Diseases at the Research Institute of Surgery in Sofia for the period Jan. 1983-April 1990. In the light of the current trends in gynecological practice, the authors specify the indications and contraindications for surgical interventions in these diseases.

Between 1976 and 1986 laparoscopy was performed in 3,584 patients to establish the cause of acute and chronic lower abdominal pain. The most frequent indications for laparoscopy were lower abdominal pain of unknown etiology,

, and adnexal tumors. The overall level of agreement between the clinical and laparoscopic diagnoses was 56.8%. In 50% of the cases with unknown etiology there was no correlation with the complaints. It also proved impossible to establish any organic cause in 8% to 32% of the other cases clinically diagnosed. The principal advantage of laparoscopy is, that the diagnosis is definite, rendering medication and laparotomy unnecessary in many cases. Using mAb ICO-160, we have studied Fas (APO-1/CD95) antigen expression on blood lymphocytes from healthy women, pregnant women and patients with chronic

, ovarian and uterin cancer. In peripheral blood from healthy women Fas antigen was detected on 23.42 +/- 2.9% of lymphocytes. The healthy donors were divided in two different subgroups with low and high Fas expression. Expression of Fas antigen on lymphocytes was elevated (high expression subgroup) in all the groups of investigated patients, excepting the ovarian cancer ones with Fas expression similar to that in healthy donors. Comparison of expression of other differentiation antigens between healthy donors and the above groups of patients elucidated in the patients a kind of pronounced imbalance of the immune status and activation of the immune system. Additionally, in patients with ovarian cancer the imbalance of the immune status was reflected as altered ratio between helper and suppressor cells (the ratio was 0.73 in contrast to that 1.08 in group of healthy donors). As follows from the summarized results of all the trials, Fas antigen expression correlated with expression of other activation antigens as CD71 and CD25 (r = 0.05 and r = 0.52, respectively). Consequently, Fas (APO-1/CD95) antigen may be considered as the activation antigen and its expression may be used for assessing the immune status.

Sonography can reduce the high rate of false positive clinical examinations concerning acute appendicitis. It has to be stated that an exclusion of appendicitis can only be made sonographically if the normal appendix can be seen in its full length and/or an other differential diagnosis can be depicted that explains the clinical symptoms. Mucoceles are rare cystoid lesions of the appendix. They exhibit a typical onion skin sign structure caused by different mucus viscosities. In large mucoceles a tumor causes this lesion.
