Twenty-seven patients with peritonitis to whom a drain was applied were given sulbenicillin (SBPC), a broad-spectrum antibiotic, which has so little hepatic and renal toxicity that massive doses may be feasible, and examination was made as to its therapeutic effects and concentrations of the antibiotic in the ascites. Daily dosage of SBPC was 10g in two divided doses in most cases given by the intravenous infusion. Medication was continued for 3 approximately 15 days. The highest daily dosage was 20g and the largest total dosage reached 190g, but there was no adverse reaction except for one case of a slight anemia. Peritonitis complicated appendicitis, adnexitis, duodenal ulcer perforation, intestinal obstruction or trauma as its primary disease. No difference in the therapeutic effect existed among the primary diseases. The response to SBPC treatment was excellent in 8 of the 27 patients and good in 17. Two patients failed to respond to the therapy. When SBPC was given just before operation, the SBPC concentration in ascites obtained at operation was 112 microgram/ml in 2 cases. The SBPC concentrations in ascites were examined following intravenous infusion of 5g over an hour, and a peak concentration of 94.7 microgram/ml was obtained at the completion of infusion (an hour after the start of infusion), which gradually decreased thereafter. In the ascites excreted from the drain after operation, a high concentration of 12.7 approximately 90.2 microgram/ml (mean: 51.7 +/- 7.7 microgram/ml) was obtained on the day after the operation day, but the concentration was lower thereafter. The SBPC concentrations in ascites were compared as regards the sites of drainage (Winslow's foramen, ileocecum and Douglas' fold), but no particular difference was observed. The SBPC concentrations in ascites after operation were in inverse proportion to the alleviation of peritonitis. They were higher when the inflammation was severer.

Using data from the 17th Congress of the Polish Gynecological Society of 1968, an analysis of complications following abortion at 2 obstetrical clinics in Poland is presented; the analysis covers a 10-year period beginning 2 years after the legalization of abortion in 1956. In the 1st clinic there were 3386 abortions (95% for social reasons), almost all performed during the first trimester. There were direct complications in 345 (10.2%) cases, delayed complications in 52 cases, but no deaths. At the 2nd clinic, 14,257 abortions were performed, and out of these there were 21 (.14%) cases of uterine perforation, 97 (.68%) cases of hemorrhage, 125 (.87%) of endometritis, 320 cases of adnexitis, 287 (2.01%) retained placentas, and 35 (.37%) cases of anemia. The use of a vacuum aspiration technique has demonstrably reduced complications. Since the legalization of abortion the mortality rate from complications is over 5 times less. Abortion is not without consequence to the organism, however, and stress is being placed on family planning and birth control.

The levonorgestrel-releasing intrauterine system (LNG-IUS) is a highly reliable contraceptive device. It is a suitable method of contraception also for breast-feeding mothers, and for women who have completed their family it is a real alternative to sterilisation. In comparison with the copper-releasing intrauterine devices, the rates of ectopic pregnancies and adnexitis/pelvic peritonitis (PID) are reduced. In menstruating women the menstrual blood loss is lower and, correspondingly, dysmenorrhoea and/or iron-deficiency anaemia occur less frequently and when they do occur they are less severe. In hypermenorrhoea/menorrhagia the LNG-IUS is a therapeutic alternative to the surgical procedure. In the menopause and postmenopause the LNG-IUS is suitable for protection of the endometrium in women undergoing continuous estrogen replacement therapy.

Morbidity data from the dispensary of the district of Sangha, Mali, are analyzed. The morbidity data do not precisely mirror the health situation of the district because some villages located 2 or 3 hours away have less recourse to the dispensary. Data were collected for May, when the dry season ends, and September, when significant rainfall occurs. 578 persons were treated in May. The parasite Schistosoma haematobium, which is present in shallow and stagnant water and around onion fields, was the largest single cause of consultation. Schistosoma haematobium causes genital bilharziasis leading to sterility in women, genitourinary infections, and neoplastic bladder disease. A number of women had combined vaginitis-adnexitis causing tubal obstruction, and some also had uterine fibromas. Almost 20% of consultations were concerned with pathologies of the urinary or genital tracts. In all, 47.2% of consultations were for parasitic diseases including 28.4% for bilharziasis; 17.8% for bacterial diseases including 8.1% for gonorrhea, 5.7% for anemia, 5.3% for gastroenterologic problems, 3.1% for ophthalmologic problems, and 20.9% for other or poorly defined causes. The dusty air in the end of the dry season was the cause of colds which sometimes led to bronchopneumopathies. Intestinal parasites accounted for less than 3% of consultations. There were 537 consultations in September 1991. Parasite infections again accounted for 45.7% of consultations, with 15.1% due to malaria. Bacterial diseases accounted for 16.9%, with gonorrhea responsible for 9.2%. 4.3% of consultations were for anemia, 9.8% for gastroenterological problems, 4.5% for ophthalmologic problems, and 18.8% for other or poorly defined causes.
