Massive tumours of the parotid are uncommon as due to their site, they are usually removed at an earlier stage. We present a bizarre case of a carcinoma ex-pleomorphic
adenoma
which mimicked an advanced breast cancer, complete with a 'nipple-like' extension and
peau d'orange
changes in the overlying skin as a result of a dense dermal inflammatory response. A procedure akin to a mastectomy with facial nerve preservation was carried out for removal. To our knowledge,
peau d'orange
has not been reported before in parotid tumours.
