272 cases of schistosomal egg induced polyps collected from mass-screening for colorectal carcinoma in Jiashan County were divided into 3 types: fibrous type (FT), mixed type (MT) and epithelial proliferative type (ET). The ET was characterized by crypt elongation, hypertrophy, variation in size, disorderly arrangement, as well as the high percentage of atypical hyperplasia (64.9%). The coexistence and transition between inflammatory reactive hyperplastic and neoplastic crypts were noticed. By HID/AB staining, sialomucins were increased in ET, CEA and PNA receptors detected by the ABC method were present in 18/20 and 6/18 of ET respectively. The mucin expression of ET was similar to that of adenomas. It is likely that a spectrum of ET, atypical hyperplasia and adenoma may exist. We suggest that the patients with ET, especially those with atypical hyperplasia, should be regarded as "risk group", and close follow-up is needed.

Six pleomorphic adenomas were studied histochemically by electron microscopy after staining with ruthenium red, high iron diamine-thiocarbohydrazide-silver proteinate (HID-TCH-SP), and tannic acid-ferric chloride (TA-FeCl3); the latter two are quite specific for sulfated glycoconjugates. With ruthenium red staining, the proteoglycans of the myxochondroid stroma could be visualized as numerous extracellular 25 to 50-nm polygonal matrix granules with fine projecting filaments. Similar positive ruthenium red-stained intracellular granules were observed within the Golgi-derived vacuoles of the modified "myoepithelial" cells in the myxochondroid region. Secretion of these granules by exocytosis was occasionally observed. The modified myoepithelial cell in the myxoid region exhibited strong HID-TCH-SP- and TA-FeCl3-reactive sites within the Golgi-derived vacuoles. The intercellular space or microcyst of the cellular clusters and the extracellular matrix granules stained strongly positive with both HID-TCH-SP and TA-FeCl3. Some cells located at the periphery of the cellular clusters also showed similar but weaker reactive sites. No staining was obtained in neoplastic ductal epithelium. The above cytochemical observations indicate that the modified "myoepithelial" cell probably derives from the periphery of the cellular clusters and is the cellular source of the stromal matrix. Moreover, the observations are compatible with the histogenetic model recently proposed by Dardick et al. that the neoplastic, modified myoepithelial cell is the principal proliferating cell in pleomorphic adenoma.

The epithelial hyperplastic type (E type) was characterized by gland elongation, hypertrophy, variation in size, disorder in arrangement and high percentage of atypical hyperplasia (64.9%). Coexistence and transitional change of hyperplastic and adenomatous glands were noticed. By HID/AB staining, sialomucin increased prominently in E type. CEA and PNA receptors by ABC method were detected in 18/20 and 6/20 of E type respectively. The results of mucin expression were similar to those of adenoma. It is likely that the spectrum of E type polyp, atypical hyperplasia and adenoma exists. The authors suggest that patients with E type polyp of schistosome egg, especially those with atypical hyperplasia, should be treated as "risky" population of colon carcinoma.

Using HE, AB/PAS, HID/AB, PAT/KOH/PAS stain and CEA monoclonal antibody BA stain techniques, composition of mucin and distribution of CEA in the tumor tissue were detected in 65 cases of colorectal tubular adenoma, 7 adenoma with malignant change and 14 established tubular adenocarcinoma in order to study the functional changes of colorectal adenoma and its relation with the colorectal adenocarcinoma. According to Konishi's criteria, colorectal tubular adenomas were divided into three groups: mild, moderate and severe dysplasia, added with two of ours: adenoma with malignant change and established adenocarcinoma, totalling 5 groups for comparison. The results showed that CEA positive rate increased with the grade of dysplasia in these 5 groups (33%, 57%, 67%, 86% and 100%). Similarly, the percentage of non-acetylated acid and C7 C9 O-acetylated sialomucin also gradually increased with the grade of dysplasia, showing a parallel tendency. Epithelial cells, secreting a heterogeneous mucin, could be taken as a signal of abnormal cellular differentiation. The increase of non-acetylated acid and C7 C9 O-acetylated sialomucin in colorectal adenoma implies a likelihood of malignant change.
