Pouch reconstruction after resection of the rectum. Functional results after low anterior resection of the rectum and straight end-to-end coloanal anastomosis are often poor. Patients are frequently troubled by the so called "anterior resection syndrome" which comprises faecal incontinence, urge, frequency and fragmentation. To investigate the potential functional benefit of a coloanal pouch reconstruction we conducted a prospective study. Some 55 patients [29 men; median age 66,4 (30-85)] underwent low anterior resection of the rectum followed by coloanal J-pouch reconstruction. Pouch size was 5-7 cm, a temporary stoma was fashioned in 32 patients. Indications for the operation were: 1. rectal cancer: n = 51, 2. recurrent
adenoma
n = 2, 3. recto-vaginal fistula n = 1 and 4. severe non-specific
proctitis
n = 1. We observed 5 anastomotic leakages of which two patients required reoperation. A pelvic abscess was encountered in three patients as well as one pelvic haematoma. Micturition was impaired in five patients. To date 47 patients have completed 12 months follow-up after reversal of the ileostomy. Patients were seen at three-monthly intervals. Mean stool frequency/day was 3.9 after 3 and 2.6 after 12 months. After 3 months 38 patients (69 %) reported complete continence which increased to 83 % at 1 year. 18 Patients (33 %) complained of urge defaecation initially, which decreased to 14.5 % after 1 year. Fragmentation was observed in 40 % at 3 and and 22 % at 12 months, respectively. In conclusion, our initial experience with colonic J-pouch reconstruction shows good functional results.

Patients with longstanding ulcerative colitis (UC) and Crohn's disease (CD) have an increased risk of colorectal cancer (CRC). CRC accounts for approximately 15% of all deaths in patients with inflammatory bowel disease (IBD). The molecular pathway leading to CRC in IBD appears to differ from the well-known
adenoma
-to-CRC sequence, given the fact that these cancers appear to arise from either flat dysplastic tissue or dysplasia-associated lesions or masses. The risk of CRC for patients with IBD increases by 0.5-1% yearly, 8-10 years after diagnosis. Patients with a young age at disease onset, more extensive colitis, greater inflammatory burden, concomitant primary sclerosing cholangitis, and a family history of CRC are at greatest risk. Most cancers arise in pancolitis and there is little or no increased risk associated with
proctitis
while left-sided colitis carries an intermediate cancer risk. The CRC risk in patients with colonic CD is similar to that of UC. Colonic dysplasia is a precursor to CRC in IBD. There is no clear evidence that surveillance colonoscopy prolongs survival in patients with extensive colitis. Newer endoscopic and molecular techniques are being assessed for their effectiveness in augmenting conventional surveillance.

Fragments of anorectal mucosa are frequently present in transrectal prostate core biopsy specimens. Pathologic findings within the anorectal mucosa are easily overlooked, in part because of their rarity and because anorectal mucosa is not the focus of examination. We reviewed pathologic reports of 96,656 prostate biopsies diagnosed in our consultation service from 1997 to 2010. Anorectal lesions were detected in 30 cases, which were summarized as follows: (1) Neoplasia and polyps (n = 10). A flat rectal
adenoma
with high-grade dysplasia was identified, for which the patient received hemicolectomy. One patient was found to have a rectal carcinoid tumor. Other neoplastic findings included 1 with low-grade B-cell lymphoma, 3 with hyperplastic polyps, 3 with tubular adenomas, and 1 with anal mucosa with high-grade dysplasia that was found to be a contaminant from another case. (2) Inflammatory changes (n = 20). Acute proctitis was found in 12 patients, including 1 with acute radiation

, 2 with inflammatory bowel disease, and 9 with nonspecific

probably due to bowel preparation. Pulse granulomas and nonnecrotizing granulomas were seen in 5 patients, including 3 with diverticular disease, 1 with history of perirectal fistula, and 1 with unknown etiology. Three patients had miscellaneous findings including muciphage reaction, increased eosinophils, and features of collagenous colitis. In conclusion, it is exceedingly rare to find significant pathology in the rectal fragments associated with prostate needle biopsies. Nevertheless, pathologists should evaluate the rectal mucosa for both neoplastic and inflammatory changes to avoid missing clinically significant anorectal diseases.

Background. i-scan is a software-driven technology that allows modifications of sharpness, hue, and contrast to enhance mucosal imaging. It uses postimage acquisition software with real-time mapping technology embedded in the endoscopic processor. Aims. To review applications of i-scan technology in clinical endoscopic practice. Methods. This is a case series of 20 consecutive patients who underwent endoscopic procedures where i-scan image enhancement algorithms were applied. The main outcome measures were to compare mucosal lesions with high-definition white light endoscopy (HD-WLE) and i-scan image enhancement for the application of diagnostic sampling and therapy. Results. 13 cases involving the upper GI tract and 7 cases of the lower GI tract are included. We report a case of multiple flat adenomas and cancer of the rectum that occurred 15 years after pelvic irradiation following surgery for uterine cancer.

borders were diagnosed accurately by magnifying chromoendoscopy, leading to their adequate excision using endoscopic submucosal dissection. This enabled minimal dissection of the irradiated pelvis that would have otherwise been difficult. Furthermore, our approach probably helped minimize loss of bowel function, thereby preserving the patient's quality of life as much as possible. . Pelvic irradiation of the bowel carries a high risk of causing flat adenomas and cancer. Close and long-term surveillance may be useful in such cases, using not only conventional colonoscopy but also chromoendoscopy with indigo carmine dye spray and magnifying endoscopy.
