Chronic abdominal pain is a common symptom with an extensive differential diagnosis. The work-up is frequently costly, yet many patients elude definitive diagnosis. We describe 12 obese women with long-standing
abdominal pain
, many of whom eluded diagnosis but who met criteria for abdominal wall pain. Each patient underwent a focused history and physical examination which included checking for Carnett's sign and performing a "pinch test". All patients had positive Carnett's sign and pinch tests. An injection of local anaesthetic, with or without corticosteroid, completely relieved the pain within 10 min. Of the six patients seen in follow-up, four remained pain free and two responded to a second injection of local anaesthetic.
Abdominal wall pain
is an under-appreciated cause of chronic
abdominal pain
. Diagnosis is often straightforward and treatment with a local injection of anaesthetic is both diagnostic and curative.

Chronic abdominal pain is common in children and adolescents but challenging to diagnose, because practitioners may be concerned about missing serious occult disease.
Abdominal wall pain
is an often ignored etiology for chronic
abdominal pain
. Anterior cutaneous nerve entrapment syndrome causes abdominal wall pain but is frequently overlooked. Correctly diagnosing patients with anterior cutaneous nerve entrapment syndrome is important because nerve block interventions are highly successful in the remittance of pain. Here, we present the case of a pediatric patient who received a diagnosis of functional
abdominal pain
but experienced pain remittance after receiving a trigger-point injection and transverse abdominis plane block.

Abdominal wall pain
(AWP) is a common and underrecognized cause of chronic
abdominal pain
. The etiology of AWP varies. History and physical examination are critical to an accurate diagnosis of AWP. Trigger point injection using either a corticosteroid, a local anesthetic, or a combination of both often gives relief of pain and is of diagnostic and therapeutic value. Increased awareness of AWP as a cause of chronic, nonvisceral
abdominal pain
can prevent fruitless searches for intra-abdominal pathology and reduce medical costs.
