We treated strongyloidiasis patients and obtained the following results: Of the 299 patients (184 males and 115 females), 81 patients (27.1%) had no complaints before treatment, 218 patients complaints of some symptoms, including arthralgia and/or lumbago (28.4%), abdominal pain and/or borborygmus (19.3%), numbness of extremities (18.1%), constipation (16.3%) and itching (15.7). We treated 219 patients with mebendazole and symptoms improved after treatment described below: Thirty-seven of the 63 patients (58.7%) with arthralgia and/or lumbago improved. Twenty-seven of the 36 patients (75.0%) with numbness of extremities improved. Thirty-one of the 32 patients (96.9%) with heartburn improved. We treated 26 patients with mebendazole plus thiabendazole and twelve of 14 patients (85.7%) with abdominal pain and/or borborygmus were improved after treatment. We treated 54 patients with ivermectin and five of 18 patients (27.8%) with arthralgia and/or lumbago were improved after treatment.

This preliminary study represents 10% of the workers in this occupation in Bangalore. The industry is highly commercial in nature and the city of Bangalore; has the largest number of units in Karnataka State. The study provides base line data relating to some morbidity conditions for future comparisons. Low back ache, tiredness, head ache, irritation in the eyes, sleeplessness, joint pains, chest pain, postural giddiness, persistent running nose, abdominal pain, cough and tremors of hands, were some of the common conditions elicited among the workers. Some of the conditions were identified to be possibly related to various processes within the industry. Particularly low back ache and irritation in the eyes (rolling and dipping), tremors of hands (packing). It is very likely that in the final stages when certain chemical ingredients are added which finally give the scent, the chances of pollutants entering either through skin or inhalation is possible. Environmental monitoring has not shown any appreciable dust hazard. The study has helped in identifying some of the areas in the work environment which required improvement. There is a need for social workers, medical personnel and industrial hygienists to explore the possibilities of minimizing possible hazards. A national sampling survey in other parts of the country might also be beneficial.

Bajiaolian (Dysosma pleianthum), one species in the Mayapple family, has been widely used as a general remedy and for the treatment of snake bite, weakness, condyloma accuminata, lymphadenopathy and tumours in China for thousands of years. However, the textbooks of traditional Chinese medicine mention little about the toxicity of Bajiaolian. Within 1 year, the authors saw five people who manifested nausea, vomiting, diarrhoea, abdominal pain, thrombocytopenia, leucopenia, abnormal liver function tests, sensory ataxia, altered consciousness and persistant peripheral tingling or numbness after drinking infusions made with Bajiaolian. The herb was recommended by either traditional Chinese medical doctors or herbal pharmacies for postpartum recovery and treatment of a neck mass, hepatoma, lumbago and dysmenorrhoea. Podophyllotoxin is one of the main ingredients of the Bajiaolian root. The clinical manifestations observed in our patients were consistent with podophyllum intoxication. Podophyllotoxin intoxication usually results from the accidental ingestion or topical application of podophyllum resin. However, these cases of Bajiaolian intoxication were iatrogenic and results from 'therapeutic doses' of Bajiaolian cited in the textbooks of traditional Chinese medicine.

To assess the relation of menstrual physiologic functions and altitudes, the authors made a comparative study of 240 healthy women with four diagnostic methods on traditional Chinese medicine and obtained their menstrual changes, picture of tongue and type of pulse in Xining (2260m) and Yushu Zhaduo (4300m). The results suggested that the menarche which occurred before 15 years old (P less than 0.005), the menstruation cycle of 16-25 days (P less than 0.05), and moderate quantity menstruation (P less than 0.005) are found more in the Xining group than in the Zhaduo group; the menstrual phase in the Xining group lasted 4-7 days and 1-3 in the Zhaduo group. The incidence of abdominal pain, cold, and lumbago increased significantly in the Zhaduo group and the symptoms were serious. The age of menostasia after 46 was 100% in the Xining group, whereas 27.3% of the Zhaduo group had menostasia at the age of 36-45. The colour of tongue varied from darker red to pink with increasing altitudes (P less than 0.005). Meanwhile, cases of rapid pulse increased (P less than 0.005). It is concluded that the physiology of menstruation for healthy women has obviously changed under high altitude hypoxic environment. The mechanism of pathology could be explained by dysfunction of Zang and Fu, deficiency of Qi, stagnation of blood and Qi stasis due to cold. Treatment and prevention of menstrual disorders of women at high altitudes could be carried out by invigorating Qi, regulating blood, promoting the flow of Qi, by warming the channel and regulating Zang and Fu, etc.

Six cases of lung cancer combined with the disease which has needed semi-emergency operation, two cases of unstable angina, two of ileus due to colon cancer, one of impending rupture of abdominal aortic aneurysm and one of purulent cholecystitis with cholelithiasis, were discussed. Mean age was 62.0 years (range, 36 to 73); four were male and two were female. Case 1 and 2 were admitted with anterior chest pain, Case 3 with lumbago and abdominal pain, Case 4 and 5 with an abnormal shadow on chest x-ray film and Case 6 with abdominal pain. Of the two with unstable angina, one was operated on with right upper lobectomy during the first months after aorto-coronary bypass. Of the two with colon cancer, one was operated on with right upper lobectomy during about 5 weeks after right hemi-colectomy. Case 3 with abdominal aortic aneurysm operated on with left upper lobectomy during 4 weeks after replacement of abdominal aorta. Case 4 with cholecystitis was operated on with left pneumonectomy during about 3 weeks after cholecystectomy. The postoperative course of 4 cases and the post-chemotherapy condition of 2 cases were uneventful.

The increasing use of laparoscopy as a therapeutic method mandates a reappraisal of the risks involved. Complications frequently described include injuries to the large and small bowels, bladder, and blood vessels. The world literature reports only eight cases of ureteral injury at laparoscopy. In this report, we present five additional cases of ureteral injury occurring at laparoscopy, in addition to a summary of those previously reported. Patients tend to present in the early postoperative period (48-72 hours) with low back pain, abdominal pain, leukocytosis, and peritonitis. The diagnosis should be made by intravenous pyelography; if possible, the injury should be stented by either the retrograde or percutaneous route. The injuries, except for one apparent trocar injury, involved the use of electrocoagulation, both unipolar and bipolar. The injuries occurred most commonly near the uterosacral ligaments. In 38% (five of 13) of the cases, the laparoscopy was performed for treatment of endometriosis. Three of the 13 patients eventually lost renal function of the affected side; two of these underwent a nephrectomy. Because visualization of the ureter near the cervix at the time of laparoscopy is difficult, especially in the presence of disease, laparoscopic procedures in this area must be carried out with caution.

Decisional aids are dependent on the accuracy, comprehensiveness and decisional value of the observations in the database and the test subject. Diagnostic data stem from the observations of the patient, the doctor and third party tests. An information-utility index was used to compare the diagnostic contribution of these sources. In a diagnostic database of 314 cases of jaundice, twenty-two findings of high diagnostic usefulness were discovered among 6 diseases. Sixteen of these (73%) were patient's observations, 4 were doctor's (18%), one the patient's age and one a ward test. Five laboratory tests had lesser average scores. In three databases--on jaundice, abdominal pain and low back pain the 7 commonest diseases were examined, totalling 1018 patients. In a simple Bayesian procedure, the patient's observations alone correctly classified 50%, the doctor's observations a further 16%, the patient's age and sex another 4% and the prior probability of the diseases, an additional 4%. These figures varied considerably between the diseases. When all 21 diseases with more than 10 cases in the database were considered, the respective figures were 53, 15, 3 and 3%. A European Community medical research initiative is collecting data on 10,000 cases of jaundice and of acute abdominal pain to establish an adequate diagnostic database for the development of decision support systems.

A case of primary retroperitoneal seminoma is presented and 73 cases from the literature are reviewed. The mean age was 41 years. The presenting symptoms were abdominal pain in 53% of the cases, low back pain in 43% and in 7% the only symptom was a palpable mass. It is concluded that seminomata exist as primary tumors in the retroperitoneum, but thorough examinations of the patients is necessary in order to arrange correct treatment and to exclude a primary testicular focus or CIS. Ultrasound sonography to discover non-palpable testicular tumors is recommended as well as surgical biopsies of the testis to reveal CIS. In patients treated with combined surgical resection and radiotherapy, the success rate was 92%. The overall success rate was 68%. It is expected that the curability for all patients is significantly improved with the introduction of combination of radiation and chemotherapy, as it has been seen in advanced testicular cancers.

The reduction of pain by two antidepressants, clomipramine and mianserin, was, in this study on 253 patients with chronic idiopathic pain syndrome, found to be not better than a placebo when all patients were compared independently of the classification of pain. The improvement rate was around 40% after 6 weeks of treatment when using a 50% or better reduction in pain level. However, in patients who fulfilled a checklist definition of minor to major depression (30% of the total patient material) clomipramine was superior to mianserin and placebo with an improvement rate of 75% after 6 weeks. Using pain curves over time as outcome measure in the various clinical pain categories it was found that both mianserin and clomipramine seemed superior to placebo in patients with tension headache, but in patients with low back pain syndrome placebo was superior to the two antidepressants. No difference among the three treatments was found in patients with burning mouth syndrome or in patients with abdominal pain. These differences underline the importance of studying specific pain syndromes rather than composite groups of patients with idiopathic pain. The clinical significance of these pain curves needs further placebo controlled investigations.

A case of an anterolateral lumbar meningocele in a patient with neurofibromatosis is presented. As anterolateral lumbar meningocele may present itself only by non-specific symptoms like low back pain or abdominal pain, such symptoms require prompt diagnostic work up in patients with neurofibromatosis. The significance of MR imaging in early recognition of this rare anomaly is emphasized.
