From January 1981 to December 1987, 346 children with urinary tract infections, proved by urine culture, were admitted to the Department of Pediatrics, Mackay Memorial Hospital. The ratio of male to female was 3.0 in children below 2 years, and 0.8 in children above 2 years, of age. The urine specimens were collected from suprapubic punctures in 281 cases (81.2%). Fever was the most common clinical manifestation. In children below two years old, other common symptoms and signs were body weight loss or poor gain, feeding problems, diarrhea, irritability, jaundice, and abdominal distension. In older children, urinary frequency,
dysuria
, enuresis, loin and
abdominal pain
were frequently found. Hematuria and edema were occasionally noted in all age groups. Microscopic examination of 329 centrifuged urine specimens revealed: 256 cases (77.8%) had more than 5 leukocytes per high power field, 233 cases (70.8%) had more than 10 leukocytes. Three hundred and seventy positive urine cultures were obtained from these patients. E. coli was isolated in 273 cases (73.6%), followed by Klebsiella spp., 34 cases (9.2%); Proteus spp., 27 cases (7.3%); Enterococcus, 21 cases (5.7%); Enterobacter spp., 9 cases (2.4%); Pseudomonas aeruginosa, 8 cases (2.2%); Citrobacter spp., 7 cases (1.9%); Morganella morganii, 6 cases (1.6%); Acinetobacter spp., 6 cases (1.6%); etc. Candida albicans was isolated from three patients. Two organisms were isolated in 26 cultures; 3 organisms, in 3, and 4 in 1.

The increasing number of sterilizations among young women with few or no children had increased interest in the development of methods which are simple, effective, cosmetically suitable and which involve the fewest possible sick days and which minimize destruction of the Fallopian tube so that refertilization has the best chance for success. In Denmark the method used most often in laparoscopy with electrocoagulation. Since 1983 the Fredericksberg hospital has exclusively used laparoscopic sterilization with the Filshie clip. A total of 194 women were sterilized during the period Aug. 1983-Aug. 1986. The mark V Filshie clip was applied a few cm from the horn of the uterus with an applicator which was introduced intraperitoneally with a special trocar. The majority of the women were in their 30s with 20% in their 20s and 24% in their 40s. The patients were observed for 16-51 months. There were no operative complications. 2 patients complained of
abdominal pain
postoperatively and were treated with antibiotics for suspected perimetritis. 1 patient complained of
dysuria
and pain but recovered spontaneously within 4 weeks. 1 woman became pregnant 11 months after the operation, corresponding to a pregnancy frequency of .5%. Upon resterilization it was found that the left clip had slipped off. None of the patients were refertilized. 13% of the women had had no children; 27%, 1 child; 44%, 2; 13%, 3; and 2%, 4 or more. 52% of the woman had no induced abortions; 26%, 1; 15%, 2; 4%, 3 and 3%, 4 or more.

From January to December 1983, 12,207 specimens of urine were examined for ova of Schistosoma haematobium and 753 (6.17%) were positive. From this group, 44 adult patients were investigated for urinary tract abnormalities. Haematuria was the commonest presenting symptom (81.8%) followed by lower
abdominal pain
(77.3%) and
dysuria
(68.2%). Urinalysis revealed proteinuria in 55.5%, leucocyturia in 90.9% and microhaematuria in 88.6% of patients. Twenty-four hour protein excretion ranged from 230 mg to 2.2 g (mean 960 mg). Serum creatinine was raised in one patient (2.2 mg dl-1), Urological abnormalities included calcification of the bladder in 36.4%, ureteric strictures and dilatations in 65.9%, hydronephrosis in 9.1%, squamous cell carcinoma of the bladder in 4.5%, vesicle calculus in 2.3%, and multiple granulomatas in the bladder in 2.3% of the patients. The results of the study suggest that a sizeable population of this area is at risk of developing urological complications and consequently chronic renal insufficiency.

We report a case of spontaneous intraperitoneal rupture of the bladder. A 54-year-old woman was admitted to our hospital with the chief complaints of severe lower

dysuria
and macroscopic hematuria in October, 1985. In 1969, she had had a radical hysterectomy and postoperative irradiation for cancer of the uterus. Two years later she had undergone additional irradiation. On physical examination, the abdomen was tender with guarding and signs of peritonitis. Laboratory data revealed a blood urea nitrogen of 32.8 mg/dl and all electrolytes were normal. Excretory urogram showed normal upper urinary tract but irregularity of the bladder dome. Cystoscopy revealed acute inflammation of the bladder mucosa. Consequently, we made a presumptive diagnosis of radiation cystitis and she was treated with antibiotics and drip infusion. Within a week her general condition was improved and she had discharged. In June, 1986 she was admitted again with the same chief complaints as at her first admission. Cystoscopic findings showed a hole on the postero-superior wall and retrograde cystogram revealed an intraperitoneal rupture of the bladder. At exploration a necrotic bladder wall was resected and closed in 3 layers. The post operative course was uneventful.

Testicular torsion is characterized by the sudden onset of testicular pain associated with
abdominal pain
, nausea and vomiting. Fever is unusual and urinalysis is often normal. Fever, pyuria,
dysuria
and urethral discharge are characteristic of epididymitis. Radionuclide scanning and Doppler ultrasound are helpful in establishing the diagnosis. If the diagnosis is uncertain, the patient should be considered to have testicular torsion until it is proved otherwise. Undiagnosed torsion leads to testicular necrosis.

A chart review and telephone interview of 72 sexual abuse victims was conducted to determine if children and adolescents who are victims of sexual abuse suffer from symptoms similar to the "rape trauma syndrome," which has been reported in adults. Symptoms similar to the rape trauma syndrome were found in 48 of the 72 abused children and only 26 of the matched control group, p less than .01. Common somatic complaints in the sexual abuse patients included

. Some of the emotional and behavioral problems noted during the follow-up period among the sexual abuse patients included sleep problems, runaway behavior, and suicide attempts. The duration of abuse and age of the victim at the time of abuse significantly affected the frequency of reported somatic symptoms, but the type of abuse and type of assailant did not significantly affect the frequency of reported somatic and emotional reactions. There was no difference in the occurrence of school problems and early pregnancy between sexually abused patients and controls. Since 67% of all sexually abused patients suffered from emotional and somatic reactions, close follow-up of these patients is indicated.

Disorders of urachal remnants are uncommon. While a urachal cyst usually is asymptomatic, infection may mimic a variety of acute intra-abdominal or pelvic processes. We describe 10 patients in 2 distinct age groups (the young child and the young adult) with an infected urachal cyst. The presenting symptoms and signs in most patients included
dysuria
, severe lower
abdominal pain
and fever. In 7 patients the correct preoperative diagnosis was made. Diagnoses at referral included Crohn's disease, bladder carcinoma and pelvic inflammatory disease. A single procedure was performed in 7 cases and a staged technique was used in 3. The differential diagnosis of acute abdominal and pelvic pain or a midline lower abdominal mass in the pediatric or young adult age group should include infection of a urachal remnant.

A 19-year-old male was admitted with left lower
abdominal pain
and
dysuria
. Excretory urograms showed only a right kidney. V.C.G. showed a cystic mass lesion over the bladder. Computerized tomography revealed a huge retroperitoneal cystic mass located near the bladder. It seemed to be a huge bladder diverticulum, but the diagnosis was not clearly confirmed before operation. Through a lower abdominal midline incision, the cystic mass structure was removed. The specimen was a left ectopic pelvic kidney with non-functioning hydronephrosis. The removed mass was 91 X 85 X 78 mm in size, and 120 g in weight. The intrarenal fluid was 350 ml. A brief discussion and review are made. To our knowledge, only 3 such case have been described in the Japanese literature.
