We report on a case of an atypically located gallstone ileus as a rare complication of cholecystolithiasis. A 61-year old lady with a history of diabetes type II and nephrolitiasis presented with
abdominal pain
lasting for 8 days and with vomiting and diarrhoea. Physical examination revealed a palpable tumour and pain in the left lower abdomen. An extensive elevation of blood sugar, CRP and leukocytosis was found. Initially X-ray of the abdomen and sonography showed signs of a subileus. Additionally a 5 x 2 cm mass with dorsal shadowing was detected by ultrasound. Gallbladder and the biliary system were normal. The sonographic suspicion of a gallstone ileus was confirmed by a subsequent CT scan. Under operation the gallstone was found in the distal

. A gallstone ileus must be included in the differential diagnosis of a tumour in the left lower abdomen. A tumour with dorsal shadowing and signs of a subileus may be the only sonographic findings of a gallstone ileus.

Wegener's granulomatosis (WG) is a disease characterized by a granulomatous necrotizing vasculitis of small vessels. Although any organ systems can be involved, gastrointestinal involvement in WG is notably uncommon. We present a case of 67-year-old man who was admitted with
abdominal pain
and diarrhea lasting for 3 weeks. Colonoscopy and abdominal CT scan revealed vasculitis and multiple mesenteric lymphadenopathy.
Jejunum
and mesenteric lymph nodes biopsies confirmed limited form of WG. The present case indicates that WG might involve only gastrointestinal tract and the histological confirmation is important for diagnosis.

A total number of 72 patients with typical Crohn's disease has been reported for the whole country of Norway during the 8-year period 1956-63. No sex difference was found. The youngest patient was 14 years old. The average age at the time of diagnosis, 40.4 years, was higher than in previously published series of patients, The main symptoms observed in our patients were:
abdominal pain
in 86 per cent, diarrhea in 53 per cent, loss of weight in 40 per cent, fever in 33 per cent, subileus in 34 per cent, and ileus in 21 per cent. Gastrointestinal bleeding occurred in 19 per cent of the cases, an abdominal mass was palpated in 36 per cent, and fistulas occurred in 33 per cent of the patients. The disease was localized to the terminal ileum alone in 58 per cent, to terminal ileum and coecum in 21 per cent, and to proximal ileum and jejunum in 7 per cent of the cases.
Jejunum
alone was affected in 7 per cent and in 7 per cent ileum and segments of colon distal to coecum were affected. Forty-one patients were treated by primary resection, which was followed by good results in 84 per cent of the cases. Improvement occurred less frequently in the 16 patients treated with primary bypass operation.
