The results of the radiological examination of 330 children following oral administration of barium are reported. It is pointed out that if one just takes into account the undoubted organic diseases, only a few children would benefit from the examination; in our case only 9 of them (a gastric and a duodenal ulcer, a pancreas pseudocyst, 5 hiatal hernias, and a celiac disease), i.e. 3%. This is not very satisfactory from a practical point of view. This situation improves radically when one looks for diseases usually rated as questionable: small hiatal hernia (cardiotuberositary malposition), functional disturbances of the small intestine, reactive hyperplasia of the lymphoid tissue, and chronic appendicitis. Thus we were able to offer 177 children, i.e. 53% of them, an efficacious therapy. The importance of chronic appendicitis and of functional disturbances of the small intestine as a cause of
abdominal pain
in children is pointed out, and their radiological symptoms are discussed. Finally the not uncommon, but not very well known disease of incomplete sigmoid
volvulus
is described.

In a kindred with a familial visceral myopathy, seven patients had operations seeking relief of chronic
abdominal pain
and other symptoms of intestinal obstruction; one patient had an 80% cystectomy and a Y-V-plasty of the bladder neck for urinary retention. Five patients with megaduodenum had bypass operations; a side-to-side duodenojejunostomy was done in four and a retrocolic gastrojejunostomy in one. Two of these died of postoperative complications, and one developed symptomatic adhesions. Two other patients who had duodenojejunostomy have done well for 6 years and 1 1/2 years respectively. One patient with dilation of the distal jejunum and proximal ileum had relief of intestinal obstructive symptoms from jejunostomy to decompress the destal jejunum. One patient who had a resection of the descending and sigmoid colon for sigmoid
volvulus
has done well for four years. Three of these seven patients developed peritonitis postoperatively, and two had symptomatic adhesions after operations. Duodenal aspiration from a patient who developed postoperative peritonitis grew E. coli, 10(13) colonies per ml. After review of the results of operations in other families and in our kindred, we favor side-to-side duodenojejunostomy in megaduodenum. Duodenal aspirate must be cultured before operation. Evidence of bacterial overgrowth in the aspirate should prompt appropriate antibiotic treatment to reduce the likelihood of sepsis.

because her complaints were appreciated too late. At laparotomy, evidence of chronic intermittent volulus secondary to nonfixation from the ligament of Treitz to the transverse colon was found in all patients. A Ladd procedure with appendectomy was performed and immediate resolution of symptoms was noted in each surviving child. Children with a story of chronic
abdominal pain
deserve a carefully interpreted history and radiographic examination before being labeled as chronic complainers.

with mausea, anorexia anemia, and possibly a malabsorption syndrome. An exploratory laparotomy was performed. The surgeon found several previous adhesions, a small intestinal

, and a nodular pancreas. This suggested previous perforation of the small bowel from enteritis, causing a "blind-loop" syndrone. The invilved section of the small bowel was resected. With appropriate treatment, the patient is well three months after operation.

Sigmoid
volvulus
occurs more frequently in the younger patient than is presently thought. It is not infrequent in females. It usually presents with "colicky"
abdominal pain
, long-standing constipation or obstipation, and increasing distention of several days' duration. A history of similar attacks strengthens the suspicion for this diagnosis. Abdominal x-ray films will usually reveal severe colonic dilation down to a low point in the sigmoid, without gas in the rectum. The classic "horseshoe" sign is seldom seen, but when present, strengthens the diagnosis. Surgical manipulation and pregnancy may be contributing factors. Treatment should be surgical unless contraindicated by specific circumstances.

are reviewed. Nearly half of these patients had a history of mental illness and one-third of all patients were chronically constipated. The main clinical features of

and gross abdominal distension had been present for an average of 8 days before presentation of the patient to hospital. The clinical diagnosis of sigmoid

was not difficult but the presence of non-viable bowel was more difficult to establish. A silent abdomen was the most valuable indication of the presence of gangrenous bowel. Conservative measures, including sigmoidoscopy and therapeutic barium enema, successfully reduced the
volvulus
in half of the cases so treated. In those patients undergoing surgery the procedure associated with the lowest mortality was sigmoid resection with end-to-end anastomosis. The overall mortality was 35%.

We describe a patient with symptomatic multiple lipomas with
volvulus
. He complained of gradually increased
abdominal pain
and distention. Preoperative roentgenograms showed many rounded filling defects of the small intestine. Operation was performed to reduce intestinal obstruction and ileal resection was performed. The dilated ileum was rotated 720 degree clockwise and it contained 115 submucosal and 26 subserosal lipomas. Histologically, the lipomas were benign and consisted of well-developed fat tissue. The patient's postoperative course was satisfactory, and neither residual lipoma nor recurrence were found at barium enema examination two years after operation.

A 75-year-old black man came to the emergency room because of nausea, vomiting,

. This was nonoperatively reduced in the emergency room. Following a mechanical and antibiotic bowel preparation, the patient underwent elective exploration. We report, for the first time, operative treatment of sigmoid
volvulus
with a laparoscopic-assisted sigmoid colectomy and primary anastomosis. Because of dense fibrous scarring of the sigmoid mesentery produced by chronic mesosigmoiditis, the redundant sigmoid was exteriorized and resected extracorporeally. A stapled, side-to-side, functional end-to-end anastomosis was constructed. The patient experienced little postoperative pain and virtually no postoperative ileus. We believe that laparoscopic-assisted sigmoid resection may offer distinct advantages for the treatment of the typically elderly, debilitated patient in whom sigmoid
volvulus
develops. Furthermore, because of the characteristic mesosigmoiditis associated with sigmoid
volvulus
, we suspect that exteriorization and extracorporeal resection may prove the easiest and most rapid laparoscopic approach to this disease.
