Abdominal
tuberculosis
, although rare, occurs mainly in immigrants from the Indian subcontinent. Such people comprise 13.5% of our local population and contributed 90% of a series of 72 patients presenting in the last 16 years; a local disease incidence of 1:6000 for Asian immigrants. Men and women were equally affected, but on average women were much younger. Diagnosis was made from one month to 10 years after immigration. No clinical feature was diagnostic, but
abdominal pain
, night sweats and weight loss occurred in more than half the patients. The erythrocyte sedimentation rate (ESR) was elevated in 95% and no patient tested had a negative Mantoux test. In 20 patients diagnosis was by clinical suspicion and response to therapeutic trial. In 52, including 39 who had a laparotomy, histological and culture material was obtained but these patients fared no better. Only one organism was resistant (to streptomycin) and rapid response to chemotherapy was the rule. Successful outcome was not related to the type of presentation, operative findings or specific chemotherapeutic agents. We would suggest that in Asians presenting with difficult-to-diagnose abdominal symptoms accompanied by malaise, raised ESR and a positive Mantoux test, a therapeutic trial of anti-tuberculous therapy should precede diagnostic laparotomy.

is discussed. Thirty cases diagnosed in an eight-year period are presented. The disease was 2.3 times more common in females. Eighty per cent of the patients were less than 40 years old. Excluding two cases of long duration (one year and three years), mean duration of the symptoms before admission was 2.3 months. Fever,

, swelling and weight loss were the main symptoms. Ascites was clinically detected in 80 per cent of cases. Intermediate strength PPD tuberculin skin test was positive in 61 per cent of cases. In 57.7 per cent there was radiologic evidence of pleuropulmonary disease suggestive of

. Fourteen of 15 ascitic fluids examined were exudative in nature with predominance of lymphocytes. In the majority of patients diagnosis was proven with laparotomy and laparoscopy (12 cases in each group). In the remainder, tissue for diagnosis was obtained through blind peritoneal needle biopsy, percutaneous biopsy of liver and peripheral lymph node biopsy (two cases with each method).

, 91 occurred in immigrants of Asian origin, six were diagnosed at post-mortem, and only 31 had evidence of pulmonary disease.

, weight loss, fever and vomiting were the most common symptoms. Barium contrast studies showed abnormalities in 56 per cent of cases but these could not be distinguished from the changes caused by other chronic inflammatory bowel diseases. Drug treatment was used in all patients, combined with biopsy in 37 and resection in 36. Results of treatment were good with low morbidity and mortality, there were few long-term problems and no relapses have occurred.

and 4 cases of benign hepatic lesions in which percutaneous biopsy had failed to obtain a representative specimen). In 10 patients with abdominal trauma the source of the problem was identified, and in 2 of these successful conservative management was undertaken on the basis of the laparoscopic findings. of unknown cause no lesion could be identified, and the procedure was abandoned in 2 patients because of dense intraperitoneal adhesions. There was no associated mortality and the morbidity rate was 0,5%.

An autopsy case of erythremia with sideroblastic tumor cell proliferation is described. A 60-year-old man was admitted to the hospital due to general fatigue and anorexia. Bone marrow aspiration revealed abnormalities in erythropoiesis (megaloblasts, 4%; sideroblasts, 84%; ring-formed, 39%, and PAS-positive, 5%). Therapy was directed to pulmonary tuberculosis. Anemia was not improved despite repeated whole blood and platelet transfusions. Serum iron and percentage saturation of the total iron-binding capacity rose during the course. Administration of vitamin B12, B6 or folic acid was inefffective. INAH was replaced by its derivative, IHMS, during the course, but the population of sideroblasts especially of ring-sideroblasts was invariably large (78%-100% and 39%-65% for total sideroblasts and ring-sideroblasts, respectively). He died with increasing

and jaundice after three months' hospitalization. Main autopsy findings were: diffuse proliferation of atypical erythroblasts in the bone marrow, systemic lymph nodes, liver, spleen and kidneys. Most of the cells positively stained with iron.

Mycobacterium genavense is a recently defined fastidious organism that has been identified as a cause of disseminated infection in patients with AIDS. We report the cases of two patients who had advanced AIDS and a clinical syndrome of fever, anorexia,

, diarrhea, and weight loss. In addition, splenomegaly and lymphadenopathy were prominent in both cases, and in one patient's case radiographic findings were suggestive of splenic abscesses. Mycobacteria isolated from specimens of blood and bone marrow grew in liquid media but not on solid media. The results of DNA probe tests for Mycobacterium
tuberculosis
and Mycobacterium avium complex were false-positive for both patients. After treatment of the broth cultures to lyse red blood cells, the results of DNA probe tests were negative for these pathogens. Amplification and sequencing of 16S rRNA with use of the polymerase chain reaction indicated that the mycobacterial isolates from both patients had sequences identical to those previously reported for M. genavense. One patient survived 5 months after diagnosis, the other 2 months after diagnosis; only one patient responded (transiently) to antimycobacterial chemotherapy.
