abdominal pain
, bleeding or a palpable mass. Only one case resembled the pattern of the so-called "Mediterranean lymphoma." The most common primary sites were the stomach and the ileum. One case of colonic lymphoma was associated with

but was probably not etiologically related. The pattern of primary gastrointestinal lymphoma in Chinese corresponds in general to the pattern observed in western countries.

450 patients in Riyadh, Saudi Arabia, complaining of chronic
abdominal pain
and, coming from different countries endemic for
schistosomiasis
, were examined endoscopically using fibre-optic colono- or sigmoidoscopes, and rectal biopsies were examined for Schistosoma mansoni ova. After a preliminary study showed that more than 6 biopsies did not increase the positivity rate, 6 biopsies were taken from the rectum and examined by transparency technique. 280 were positive for S. mansoni, 9 of them having in addition S. haematobium. 4 patients had polyps in the sigmoid colon and rectum. When these positive cases were examined using duplicate 50 mg Kato smears, only 160 (57.1%) were positive. There was a highly positive correlation between the intensity of infection as graded by the 2 techniques. We recommend examination of 6 rectal biopsies using fibre-optic sigmoidoscopy when available in small communities with a patchy distribution of
schistosomiasis
like Saudi Arabia. The method could also be used to exclude
schistosomiasis
in persons who have moved from rural to urban or non-endemic areas and are unlikely to be re-exposed to infection.

A 13-year-old girl presented with fever, night sweat, weight loss,

, haematuria and hepatosplenomegaly. Urinalysis revealed many Schistosoma haematobium ova, but rectal snip examination was negative for schistosomal ova. X-ray and CT scan of the chest revealed enlargement of the anterior, superior, mediastinal and left suprahilar lymph node with an adjacent left pulmonary parenchymal opacity and small peripheral lesions on the right side. Periportal contrast enhancement relative to adjacent liver and portal blood has been reported on CT scans in cases of
schistosomiasis
and hepatic Kaposi sarcoma in AIDS patients. We observed this phenomenon in 10 (8%) of 130 consecutive, contrast-enhanced, nondynamic CT examinations of the abdomen. Thus, the occurrence is more common and less specific than previously reported. Seven of the 10 patients in our series were receiving chemotherapy for malignant disease, and three had

with no definitive diagnosis. In four of the 10 patients, corresponding areas of periportal low attenuation or radiolucency were observed on initial dynamically enhanced scans. Periportal enhancement may be related to late diffusion of contrast material into periportal areas that were initially radiolucent. Such diffusion may occur because of endothelial insult. Periportal contrast enhancement appears to be a nonspecific finding on nondynamic contrast-enhanced CT scans of the abdomen. Periportal enhancement is important to recognize because it can mimic the appearance of portal vein thrombosis and may also be used to differentiate intrahepatic biliary dilatation from periportal radiolucency.

The clinicopathological features of
schistosomiasis
of the appendix are discussed, based on the clinical presentation, operative findings and morphological changes in the specimens of patients seen in Ibadan between 1980 and 1989. Schistosoma haematobium was implicated as the causal agent of a granulomatous inflammatory reaction with eosinophilia and fibrosis. Intramuscular oviposition was associated with frank acute appendicitis, and serosal involvement resulted in peritoneal adhesions, with ileoileal intussusception in one patient. This study evaluates 216 patients with schistosomal colonic disease, diagnosed by endoscopic biopsies at the Armed Forces Hospital, Riyadh. The colonoscopic appearance was suggestive of

in 98 of these patients (45.37%), Schistosoma mansoni ova in stool was detected in only 24 of these 216 patients (11.11%). The most common histopathological finding in colonic biopsies of these patients was Schistosoma mansoni ova in the colonic mucosa with no or mild inflammatory cells infiltrates. , the profiles of these populations were highly similar. The overall frequencies of diarrhoea were 36%, 25%, and 19%, respectively; of

86%, 78%, and 83%; of fatigue 7%, 2%, and 1%; of left lobe hepatomegaly 30%, 18%, and 9%; of right lobe hepatomegaly 18%, 10%, and 5%; of splenomegaly 18%, 10%, and 7%. Organomegaly was generally mild, even in area A.
