Eighty-two persons with chronic
abdominal pain
, submucosal or extrinsic masses, or achalasia and 20 volunteers without symptoms were prospectively examined with endosonography to assess the incidence and features of mediastinal
lymphadenopathy
. In 99 persons without malignant disease, 86% were found to have paraesophageal lymph nodes by endosonography. Overall, 352 lymph nodes were found, with a mean of 3.6 +/- 2.8 SD lymph nodes per person (range, 0 to 14 lymph nodes). Increasing age and chronic pancreatitis were found to correlate with a greater number of lymph nodes. Otherwise, clinical features including quantity of tobacco use, height, weight, sex, and race did not correlate with a greater number of lymph nodes as assessed by regression analysis. The mean width and length among the lymph nodes was 4.9 +/- 2.4 mm (range, 1 to 15 mm) and 9.9 +/- 5.2 mm (range, 1 to 30 mm), respectively, with the largest lymph nodes occurring in the peritracheal and peribronchial regions. Of all subjects, 95% had a maximum lymph node width of 10 mm or less, but only 40% had a maximum lymph node length of 10 mm or less. The most common ultrasound features of these lymph nodes included a triangular or crescent-shaped and homogeneous or centrally echogenic pattern. These ultrasound characteristics were distinct from those previously described in malignant lymph nodes. In summary, a high background prevalence of mediastinal lymph nodes exists as detected by endosonography and must be considered when performing esophageal or lung carcinoma staging examinations.(ABSTRACT TRUNCATED AT 250 WORDS)

Small intestinal lesions may or may not be preceded by other types of intestinal pathology, such as celiac or inflammatory disease. and later nausea and/or vomiting are the most common presenting symptoms of lesions that arise in an already diseased bowel, palpable abdominal masses are present in approximately one third of these cases. and vomiting. A radiograph of the chest revealed widening of the right superior part of the mediastinum, and an abdominal radiograph showed many air-fluid levels. A computed tomographic scan of the chest revealed a solitary nodule in the right anterior lobe of the lung, and right paratracheal
lymphadenopathy
. Ileus was diagnosed and a nasogastric tube was inserted. The patient's condition gradually worsened, and on hospital day 17 a laparotomy was performed. Operative findings were significant for numerous, white nodules all over the peritoneum, omentum, and mesentery, which ranged from miliary to rice grain-sized. Examination of an omental specimen revealed noncaseating granulomas with Lang hans' giant cells. The polymerase chain reaction was used to examine fluid from the nasogastric tube used before surgery, and on hospital day 40 that fluid was found to be positive for Mycobacterium tuberculosis. M. tuberculosis was also cultured from the fluid. From these findings, we concluded that this was a case of pulmonary tuberculosis manifesting predominantly as ileus secondary to tuberculous peritonitis. Anti-tuberculosis therapy consisting of isoniazid, rifampin, and ethambutol was started postoperatively. On repeat laparoscopy 224 days later, no white nodules were seen. A computed tomographic scan of the chest revealed that the right paratracheal
lymphadenopathy
was markedly reduced, and the solitary nodule in the right anterior lobe of the lung was almost gone. Few cases of young people with pulmonary tuberculosis manifesting primarily as ileus have been reported. Tuberculosis should be included in the differential diagnosis in patients presenting with ileus.

Sarcoidosis affects every organ in the body; the most frequently involved structures are the lung, lymph nodes, liver, spleen, eyes, joints and heart. Gastrointestinal system affliction is uncommon. The pancreas is rarely affected by sarcoidosis. A review of the literature revealed only 13 patients with biopsy proven granulomas in pancreas or peripancreatic nodes. In a review of all autopsies performed between 1950 and 1993 at Los Angeles County + University of Southern California School of Medicine, Los Angeles, USA, the authors found one patient with pancreatic and 5 with peripancreatic lymph node granuloma. The authors also describe three previously unreported cases of pancreatic sarcoidosis. From the discussion, a clinically useful and pragmatic profile of pancreatic sarcoidosis emerges. Two thirds of the patients with pancreatic sarcoidosis have

, and three quarters of them have bilateral hilar

. The occurrence of

with or without pulmonary infiltration should lead one to think of pancreatic sarcoidosis. The diagnosis should be established by a tissue biopsy because the laboratory and radiographic techniques do not differentiate pancreatic sarcoidosis from other inflammatory and malignant disorders of the pancreas. . Gastrointestinal investigation revealed no particular findings suggesting a carcinoid syndrome associated with the left kidney tumor. The specimen of the resected tumor showed staining pattern specific to carcinoid tumor; positive staining for chromogranin A and neuron-specific-enolase. Electron microscopy confirmed the presence of dense-core granules in the tumor cells. No evidence of metastasis or recurrence was noted for 13 months after the operation. This was a very rare case of primary renal carcinoid, representing the 26th case in the literature.

A 51-yr-old man, who presented with diabetes mellitus,

, and diffuse enlargement of the whole pancreas on ultrasonography, was at first thought to have pancreatic lymphoma. Chemotherapy including steroid was carried out successfully, although the definitive diagnosis of lymphoma was not made. However, on follow-up, we realized that both laboratory findings, such as hyperglobulinemia and autoantibody-positive results, and radiographic features in this case were very similar to those of a case of autoimmune pancreatitis recently reported by Yoshida et al. (Dig Dis Sci 1995;40:1561-8). When the disease recurred, therefore, treatment with steroid alone was performed with an excellent result. We report a case of autoimmune pancreatitis simulating primary pancreatic lymphoma.
