The patient presented with acute and constant
abdominal pain
. He had had a lobectomy of the left lung three months before. On the 4th day in hospital the pain increased and he went into temporary shock. The next day a hydropneumothorax and incarcerated stomach were revealed by chest X-ray and computed tomography. He was transferred to the University Hospital immediately and underwent an operation. The diagnosis was an incarcerated para-oesophageal
hernia
with hydropneumothorax and perforation of the stomach. As a para-oesophageal
hernia
may be fatal, it is important to diagnose and treat it early.

We present a case report of a young man found to have Crohn's appendicitis in an incarcerated spigelian

, initially diagnosed via laparoscopy. We have found two published cases of incarcerated appendicitis in spigelian hernias; however, only one of them also showed histologic evidence of Crohn's disease, and neither was discovered during laparoscopy. We present two cases of catastrophic intestinal obstruction from to strangulation of a left paraduodenal
hernia
(PDH). PDH is the most common internal
hernia
. Early diagnosis and treatment are essential because of the high morbidity and mortality associated with strangulation. Early involvement of the surgical team without an overly extensive evaluation in the emergency department is important if this diagnostic suspicion exists. PDH should also be considered when there is a history of chronic, intermittent
abdominal pain
of unclear cause. To our knowledge, these cases are the first described cases of sudden demise within a few hours of onset of symptoms of a left PDH.

Pre- and postsurgery examinations were compared, and relevant radiological findings were identified. A series of 13 patients (12 males, 1 female, aged 5-57) operated between 1990 and 1997 is presented. In 12 cases the greater omentum was used to cover tissue defect within upper extremity, in 1 case--lower extremity. Complications at donor site and within extremity were assessed in two groups (group I--5 cases of free omentum graft, group II--8 cases of pedicled omentum graft). Two acceptor site complications (flap necrosis and osteomyelitis) and no donor site complications were observed in group I. In group II 2 similar donor site complications occurred but 5 patients from this group complained of strong

due to excessively short pedicle of the graft and 1 case of abdominal

A 9-year-old Thoroughbred was evaluated for clinical signs of acute abdominal pain. During laparotomy, a rent in the diaphragm and herniation of the small intestine into the thoracic cavity were detected. Because edges of the rent were smooth and fibrous, which suggested the defect was chronic, the abdomen was closed without repairing the rent. After recurrence of clinical signs, a second laparotomy was performed, during which the intestines were found to have reherniated. Ultrasound examination is a valuable diagnostic tool both in palpable and nonpalpable Spigelian hernias. The treatment is surgical, with excellent results and low risk of recurrence.
