Four cases of spontaneous acute hemoperitoneum due to rupture of a liver tumor are presented. The resulting acute abdomen was the first manifestation of the neoplasia. The four tumors corresponded histopathologically to a cavernous
hemangioma
, a bening adenoma related to anabolizing androgens, and two hepatocarcinomas in cirrhotic livers. All of the patients presented
abdominal pain
and shock, the characteristics of which are described in this report. One of the patients died due to cardiac arrest before surgical treatment. Emergency surgery was performed on the other three, consisting of left hepatic lobectomy and ligature of the hepatic artery for the hemengioma, and segmented hepatectomy for the adenoma and the hepatocarcinoma. Only the patient with benign tumor survived. Lastly, the authors review the literature, commenting on the clinical, physiopathologic, therapeutic, and prognostic aspects.

Evidence of a cavernous
hemangioma
of the liver with fistula between hepatic artery and the portal vein of a 52 year-old woman with colickly upper
abdominal pain
and ascites was obtained using Duplex-Doppler-ultrasound. The diagnosis was confirmed by selective arteriography. After ligation of the left hepatic artery clinical symptoms abated. The cavernous
hemangioma
was followed up over 7 1/2 years with ultrasound. Ultrasound has been shown to be a valuable test for diagnosis and follow up of cavernous
hemangioma
.

We present eleven patients diagnosed of giant hepatic
hemangioma
in the last 20 years. The diagnosis was confirmed in all the cases during laparoscopy or laparotomy. The mean age of the patients was 44.9 +/- 8.99; nine of them were women. Only two of the patients complained of
abdominal pain
. Five patients showed abnormal liver function tests; the most common finding was increased levels of alkaline phosphatase. We have reviewed the diagnostic tools employed: isotopic study of the liver with 99Tc, and labeled erythrocytes, abdominal ultrasonography, CAT, hepatic arteriography, laparoscopy, laparotomy and liver biopsy. Usually we employed more than one of these diagnostic methods. In the last years there has been a shift to employ less invasive procedures.

Hemangioma
of the small intestine is a rare disease which is sometimes very difficult to diagnose because of its anatomical location. This report concerns two cases of multiple intestinal hemangiomas treated surgically. The first case was a four-year-old boy who suffered from recurrent
abdominal pain
and progressive hemorrhage from the gastrointestinal tract for four months. The character of his
abdominal pain
was non-specific. On physical examination, whole abdominal tenderness was evident. Serial laboratory and radiological examinations, including abdominal sonogram, barium enema, Tc-99m pertechnetate scan, upper gastrointestinal series failed to reveal abnormality. Multiple intestinal hemangiomas were found at exploratory laparotomy. The second case was a new-born suffering from fetal ascites. Abdominal sonogram, barium enema, voiding cystourethrography were performed without significant findings, and the diagnose of multiple intestinal hemangiomas was proved by laparotomy. In both, symptoms improved after segmental resection of the bowels. The purpose of this paper is to draw attention to the possibility of intestinal
hemangioma
as a source of recurrent
abdominal pain
, tarry stool or ascites in children.

An 11-year-old girl presenting with right upper quadrant
abdominal pain
was found to have a venous
hemangioma
of the posterior wall of the gallbladder. Radiographic, ultrasonographic, and angiographic findings of this entity, along with a review of the literature, are presented.

Angiosarcoma is a rare malignant tumor originating from vascular endothelial cells. We have experienced a case of 17-year-old man, who had angiosarcoma in the pineal region and the liver. Patient's initial symptom was headache and CT scan revealed a high density mass in the pineal region with obstructive hydrocephalus. After the radiation therapy, the tumor was disappeared completely on CT scan. One year later, he developed
abdominal pain
, and CT scan and angiogram revealed multiple
angioma
like lesions in the liver. The mass in the pineal region showed concomitant regrowth, and finally, the patient was died of abdominal hemorrhage. At autopsy, pineal tumor and hepatic tumor were both angiosarcomas, although it was uncertain which was the original tumor.

The author recently examined four children of clinical macrocephaly. Their occipitofrontal head circumference was exceeded 2 standard deviations above the mean for chronological age. Megalencephaly with normal ventricular system was proved by computerized tomography or pneumoencephalography. The purpose of this paper is to report four cases with various clinical aspects and to discuss the similarity of these patients. Additionally, previously reported syndromes with macrocephaly and multiple hemangiomas were presented in Table 4. Case 1 A 6 year-old was accidentally admitted because of infectious disease. His development was uneventful. Case 2. A 2 year-old boy was admitted because of delayed speech and delayed walking. He had no neonatal complications and no history of convulsions. Case 3. A 4 year-old boy was accidentally admitted because of
abdominal pain
. His physical examination exhibited diffuse hemangiomatous lesions on the right side of face, neck and chest, and congenital glaucoma. He was diagnosed as Sturge-Weber anomalad at the age of 2 years. Case 4. A 3 year-old girl was admitted because of distended abdomen. She had widely spread strawberry
hemangioma
on her right abdominal wall and blue and brownish phacomatosis on her back. GI tract examination showed lymphoid hyperplasia of the colon. Clinical profiles are presented in Table 3. These patients were different from Sotos syndrome (cerebral gigantism), but had the following similar findings besides megalencephaly-1) large birth weight (mean; 3961 g), 2) hypotonic and wasting muscles, 3) clumsy in walking and running, 4) no hereditary tendency. The Cases 1, 3 and 4 had normal mental development. The Case 4 was seemed as a previously undescribed clinical syndrome in which the principal features were megalencephaly, distended abdomen, hypotonic and wasting muscles, lymphoid hyperplasia of the colon, retroperitoneal cavernous
hemangioma
, and cutaneous
hemangioma
and neuroma. The relationship between cutaneous involvement and megalencephaly was unknown.

The association of cavernous
hemangioma
, focal nodular hyperplasia, and hemangiomatosis of the liver has not previously been described in the literature. A case report of a patient who presented with these 3 lesions simultaneously is reviewed. The 32-year-old obese woman presented with upper
abdominal pain
of recent onset. No liver abnormality had been noted at the times of cholecystectomy and cesarean section, each performed some years before. She had taken oral contraceptives (OCs) 6 years earlier for about 6 months. Now the liver was enlarged and contained a firm, nontender mass that extended to 1 handsbreadth below the right costal margin and across the epigastrium. No bruit was noted. Results of hematologic and biochemical investigations were within normal limits. Ultrasonography revealed a solic mass, and selective celiac arteriography showed that the mass occupied the right lobe of the liver and was predominantly avascular, with small areas of pooling of contrast material. An operation revealed, in addition to the mass, many irregular purplish areas 2-4 mm in diameter in both lobes of the liver. Close to the free edge of the left lobe was a yellowish brown and slightly modular firm mass. The pathological report listed a giant cavernous
hemangioma
of the right lobe, several disseminated cavernous hemangiomata, multiple disseminated vascular ectasiae (hemangiomatosis) and 1 lesion of focal nodular hyperplasia in the left lobe. 27 days after the operation, scintiscanning revealed evidence of good liver regeneration. Another scan 3 years later revealed regeneration of the liver to normal size and no focal abnormalities. Due to the fact that increasing numbers of cases of focal nodular hyperplasia and liver cell adenomas have been reported in association with OCs and in view of the possible association between these drugs and
hemangioma
, caution in prescribing these drugs is recommended.

Four cases of primary splenic hemangiosarcoma are described. The most common presenting symptoms were recurrent, fever, malaise, and
abdominal pain
. Three patients also had complaints referable to the urinary tract. Severe hemolytic anemia with bizarre or fragmented red blood cells on peripheral smear was noted in three patients, in two cases, for more than 1 year before diagnosis. Two patients had been treated for mammary carcinoma with ionizing radiation 10 years before developing hemangiosarcoma of the spleen. One patient had multiple congenital hemangiomas present from birth, and is thought to have developed hemangiosarcoma in a previously existing benign
hemangioma
. None of our patients survived more than 7 months after the diagnosis of hemangiosarcoma. The importance of early detection and treatment of this tumor is discussed.

Until now colonoscopies have rarely been carried out in children on contrary to adults. Within two years we have submitted to this method, more than 80 patients between the first and 19th year of their lives, previously those ones who were in preschool- and school-age. The main indication for the endoscopy were bloody stools and mild inconstant
abdominal pain
. In most of the children we found different degrees and extents of an inflammatory process of the colon. The histological diagnosis was in 38 patients an unspecific chronic colitis, in 2 cases a solitary polyp, and only once a
hemangioma
. Despite of the colitis, nearly all children were in a relatively good physical condition. We think that the unknown and untreated colitis in children is very dangerous for them with regard to the develop of colon-carcinoma in their later lives.
