Ovarian cysts are rare in children but can sometimes become extremely large, leading to difficult therapeutic decisions. There is general agreement that the therapeutic attitude for benign serous cysts less than 5 cm in diameter includes echographic monitoring and possible echo-guided punction. For cysts larger than 5 cm, most authors favour cystectomy via the celioscopic route using either celio-surgery or mini-laparotomy. On the contrary, the clinical manifestations of large cysts greater than 15 cm in diameter may vary greatly, presenting as increased abdominal volume, pain, respiratory impairment. The large size of the cyst should not be allowed to mask an associated torsion of the annexes in cases of paroxysmal
abdominal pain
. We report our experience of 6 voluminous ovarian cysts with a largest diameter varying between 15 and 40 cm. The children's age varied from 10 to 15 years. There were 3 dermoid cysts, 2 treated by ovariectomy and 1 by celio-surgery, 1 torsion of the annexes on
dermoid cyst
was treated via laparotomy. There were two cases of mucinous cystadenomas, 1 treated by mini-laparatomy and the other by celio-surgery. Finally one serous cyst with torsion of the annex was treated by annexectomy. The large size of the cysts may impair the surgical approach. Prudent ceilo-surgery should be preferred to wide laparotomy. Introduction should be performed under visual control followed by leak-free punction. The cyst is emptied before exeresis of the ovary via a mini-laparotomy since, in many of these cases of voluminous tumours, cystectomy cannot always be performed and ovariectomy must be preferred.

A case of carcinosarcoma arising within an otherwise benign cystic teratoma is reported. The patient, a 78 year old nulliparous woman, presented with right sided
abdominal pain
of short duration and subsequently underwent a bilateral salphingo-oophorectomy. Slicing of the left ovary revealed a unilocular cyst containing hair admixed with soft yellow material with a thin wall apart from a solid area at one pole. Extensive areas of necrosis and cystic degeneration were present within this mass. Histologically, the large cyst was a typical mature cystic teratoma, containing carcinomatous and sarcomatous elements. Mature cystic teratomas have been reported in association with a variety of malignant ovarian tumours such as mucinous cystadenocarcinoma and malignant germ cell neoplasms. Secondary malignant transformation within a
dermoid cyst
is a much rarer occurrence, estimated as less than 2% of all such lesions. Adenocarcinomas are the second most common malignancies arising within dermoid cysts. Sarcomas alone or in combination with squamous carcinoma have been described arising in a mature cystic teratoma. To the best of our knowledge, no case of sarcoma arising in association with adenocarcinoma has been described before.

We report a case of granulomatous peritonitis which occurred following laparoscopic exeresis of a
dermoid cyst
of the ovary performed together with medically induced abortion. The peritoneal cavity was contaminated when the cyst ruptured during the procedure emptying sebum and hairs in the peritoneum. Abondant washing with aspiration of all the visible particles was done with a 5 mm canula. The diagnosis of granulomatous peritonitis was suggested with the development of persistant fever, degradation of the patient's general health, moderate
abdominal pain
and a nodular image at echography located on the operated ovary. The nodule was found to be an inflammatory granuloma on a foreign body. Two second-look laparotomies, one transrectal and the other via the xypho-pubian route, were required 1 month after the initial operation for complete cure. The diagnosis was confirmed on the pathology report.

Seven primary ovarian angiosarcomas were diagnosed in 7 patients 20 to 32 (mean 26) years of age. The most common clinical presentation was
abdominal pain
and distention. Three tumors were in the right ovary and 3 in the left; one was bilateral. Four tumors were Stage I and 3 Stage III. The tumors were brown and hemorrhagic, ranging in maximum dimension from 6 to 13 (average 10) cm. Microscopically, 5 tumors were pure angiosarcomas and 2 were associated with a

. Follow-up information ranging from 2 months to 9 (average 3.6) years was available for 5 patients. Three patients, all with Stage III disease, died of tumor (1 with a pure angiosarcoma, 2 with tumors arising in a

) 2, 15, and 30 months postoperatively. Two patients with Stage I disease and follow-up data were alive and healthy 5.5 and 9 years postoperatively. The differential diagnosis of these tumors is discussed and the literature reviewed.

Two cases of a parasitic omental teratoma which originated from an ovarian dermoid that underwent torsion, autoamputation and omental reimplantation are presented. A review of the literature revealed 23 additional cases of omental teratoma which occurred mostly in females. In some cases, the mature teratoma of the omentum showed histological evidence of ovarian stroma, and was associated with a dermoid tumor of the remaining contralateral ovary. It is generally believed that autoamputation and reimplantation of an ovarian

is the most common etiology of omental teratomas.
Abdominal pain
is the main presenting symptom of these tumors, and on physical examination a mobile abdominal or pelvic mass is often found. Both ultrasonography with colour flow Doppler and CT-scan are helpful in the diagnosis of dermoid tumors, but the correct diagnosis of omental localisation is extremely difficult. Mature omental teratomas may be treated by simple resection. The immature teratomas of the greater omentum, however, are potentially malignant tumors requiring postoperative chemotherapy and radiotherapy.

Inside the germinal neoplasms of the ovary, the monodermic or specializad teratomas constitute a not very frequent variety. The struma ovarii represents 2.7% as the form more common of this teratomas type, constituted by thyroid tissue. Struma ovarii can associate to other tumors of the ovary of germinal type as

Most of the struma ovarii are benign and 5-10% only are malignant, of these group 40% has only associated to extraovaric dissemination. The objective of the present work is to analyze the clinical, epidemical and biological characteristics of the struma ovarii. Eight cases of struma ovarii are presented, that occupy 4% inside the ovary tumors diagnosed and treated in our hospital. The clinical charac-teristics of these tumors were: age average 45.5 tears, pelvic mass accompanied by distension and

. They were managed as anyone ovary tumor, in not any case the diagnosis it was suspected, 4 were presented of the left side and 4 in the right, the weight average was 726.2 g. Only in 4 cases was carried out the transoperative diagnosis and the eight cases confirmed in the postoperative. In not any case hyperthyroid symptoms, neither morphological criteria of malignancy were presented. Only in two cases the struma ovarii was reported as pure, in the 6 remaining it is accompanied with another type of ovarian neoplasm.

. The lesion was demonstrated by ultrasonography, computed tomography, and magnetic resonance imaging to be a cystic lesion containing serous fluid. A splenectomy was performed. Both histopathological and immunohistochemical findings demonstrated the cyst to be a

of the spleen. To avoid the risk of infection following splenectomy, polyvalent pneumococcus vaccine was administered. The patient recovered uneventfully. We performed immunohistochemical studies with cytokeratin and electron microscopy to determine the origin of the cyst.

Primary ovarian carcinoid tumors are rare. A 47-year-old woman presented to our emergency room with lower
abdominal pain
. Physical examination, pelvic ultrasonographic evaluation and abdominal computed tomography revealed a 10-cm mass in the right ovary containing cystic and solid components, as well as calcifications typical of a
dermoid cyst
. At laparotomy, a smooth-surfaced, firm and mobile right adnexal mass with solid and cystic portions was detected. Initially, right salpingo-oophorectomy was performed. Frozen-section examination identified the mass as a sex cord stromal tumor containing a mature cystic teratoma. Based on this finding, total abdominal hysterectomy, left salpingo-oophorectomy, omentectomy, appendectomy were performed, and the pelvic-paraaortic lymph nodes were also removed. All histological findings in the right ovary were similar to the features of cystic teratoma and trabecular carcinoid tumor. Examination of the resected lymphatic, omental, and appendiceal tissue indicated no tumoral invasion. The diagnosis was ovarian carcinoid Stage IA. Serum testing post-surgery revealed that the levels of cancer antigen (CA) 19-9 and CA125 were 18.5 u/ml and 10.5 u/ml, respectively. The patient was discharged on postoperative day 5. The report describes the clinicopathologic and immunohistochemical features of a primary ovarian carcinoid that contained a mature cystic teratoma.
