One hundred and eleven cases of syncope or loss of consciousness are analyzed. Most are of obscure nature while some illustrate features of syncope that deserve further scrutiny. The cases are divided into six groups: Resembling cardiac syncope (30 cases); vasovagal syncope (22 cases); features of both cardiac and vasovagal syncope (12 cases); orthostatic hypotensive (29 cases); akinetic seizure? (12 cases); and miscellaneous (5 cases). Some groups are subdivided according to the circumstances surrounding the spells, for example, seated eating, nocturnal, associated with bowel movement, response to anticonvulsant therapy, etc. The following conclusions seem warranted: The evidence favors the existence of a type of akinetic seizure resembling cardiac syncope; loss of consciousness while seated eating (prandial syncope) may comprise a syndrome; syncope related to bowel movement or

The pathogenesis of paradoxical hypertension after resection of coarctation of the aorta was investigated by comparing the course of seven children undergoing repair of coarctation with five acyanotic children undergoing elective cardiovascular surgery. During the first 24 hours after surgery, all coarctation patients demonstrated a rise in systolic blood pressure (35 +/- 15.5 mm Hg; P less than 0.001), a significant depression in
cold
pressor test response, and only a slight elevation in plasma renin activity. In the next 24-72 hours, coarctation patients developed a rise in diastolic blood pressure (26.8 +/- 10.6 mm Hg; P less than 0.001), plasma renin activity (22.9 +/- 10.2/ml/hr; P less than 0.001) and fluid retention. By contrast, control patients had no significant postoperative changes.
Abdominal pain
occurred in five coarctation patients during the period of maximal plasma renin activity. The data suggest that the sympathetic nervous system may be responsible for the initial phase of hypertension after coarctation resection and that the renin angiotension system plays a major role in the second phase of hypertension and in the pathogenesis of mesenteric arteritis.

Of 34 non-bacterial gastroenteritis outbreaks which occurred at day-care centers, kindergartens, elementary and secondary schools in Tokyo during the period from February 1985 to June 1991, 28 outbreaks from which small round structured viruses (SRSV) were detected in the patients' stool specimens by electron microscopy were subjected to an epidemiological investigation. The outbreaks tended to occur frequently in the
cold
season; twenty-two (79%) of these outbreaks from November through April. Though detailed epidemiological informations was not obtained from all outbreaks, the common source of infection were presumed to be present in many of the outbreaks, judged from the incidence as to time course of patients. Food doubted to be incriminated as transmission vehicles in these outbreaks was served at schools, kindergartens, and lodgings. In some outbreaks, SRSV was detected from stool specimens of food handlers, or they were seroconverted to SRSV, suggesting that food was incriminated as a transmission vehicle. The symptoms of patients differ slightly from age to age: in the age range of 0 to 6 years, vomiting 90%, fever 41% and diarrhea 32%; in the 6 to 12 year-olds, nausea 61%, vomiting 48%,

Tuberculosis of the spine or ribs is uncommon, occurring in less than 1% of patients with tuberculosis (TB). 2 women are presented who recently immigrated from Ethiopia and India, respectively. One, aged 55, presented with chest pain, fatigue and several masses under the skin of the scalp. Chest X-ray and CT scan suggested Pancoast tumor, and the lateral parts of the first 2 right ribs were absent. The origin of the patient, clinical findings and positive PPD suggested TB of the ribs and anti-TB therapy resulted in cure. The diagnosis was later confirmed by a positive culture. The other woman, aged 68, presented with fatigue, mild
abdominal pain
and axillary lymphadenopathy. The PPD was positive and X-ray showed widening of the mediastinum. A caseating granuloma with Langhans epithelioid cells was found in a lymph node. Flaccid paraparesis developed before therapy was started. CT scan showed a typical picture of TB affecting the T3-T6 vertebrae. Drainage of a
cold
abscess of the spine via the anterior approach was followed by anti-TB therapy. Culture of a biopsied lymph node and of pus obtained at operation confirmed the diagnosis of TB. The patient died 2 months later from gastrointestinal bleeding. Awareness of the unusual presentations of various forms of TB is mandatory in countries with immigration from countries in which TB is still common.

To assess the relation of menstrual physiologic functions and altitudes, the authors made a comparative study of 240 healthy women with four diagnostic methods on traditional Chinese medicine and obtained their menstrual changes, picture of tongue and type of pulse in Xining (2260m) and Yushu Zhaduo (4300m). The results suggested that the menarche which occurred before 15 years old (P less than 0.005), the menstruation cycle of 16-25 days (P less than 0.05), and moderate quantity menstruation (P less than 0.005) are found more in the Xining group than in the Zhaduo group; the menstrual phase in the Xining group lasted 4-7 days and 1-3 in the Zhaduo group. The incidence of

, and lumbago increased significantly in the Zhaduo group and the symptoms were serious. The age of menostasia after 46 was 100% in the Xining group, whereas 27.3% of the Zhaduo group had menostasia at the age of 36-45. It is concluded that the physiology of menstruation for healthy women has obviously changed under high altitude hypoxic environment. The Chinese traditional drug "Hua Jiao" specified in the Chinese Pharmacopoeia 1990 edition is the dried pericarp of ripe fruit of Zanthoxylum schinifolium Sieb. et Zucc. or Z. bungeanum Maxim., family Rutaceae. It has been used for epigastric pain accompanied by

sensation, vomiting, diarrhea and

due to intestinal parasitosis, ascariasis and used externally for eczema. By the investigation of the drug resources in the main producing areas and distributing regions (Sichuan, Guangxi, Henan, Liaoning, Zhejiang, Anhui, Jiangsu and Shandong Provinces or Autonomous Regions), we found that there are pericarps derived from more than 18 spp. of Zanthoxylum used as drugs in China. In this paper, the morphological and histological characters of crude drugs derived from the following 8 species, viz. Zanthoxylum bungeanum Maxim., Z. schinifolium Sieb. et Zucc., Z. armatum DC., Z. simulans Hance, Z. avicennae (Lam.) DC., Z. ailanthoides Sieb. et Zucc., Z. molle Rehd. and Z. nitidum (Roxb.). DC. were described with illustrations. It was discovered that such features as the external characters of pericarp, the occurrence of hairs on fruit stalk, the presence and location of pigment and crystals of hesperidin, the thickness of the cell walls of endocarp and the presence and shape of nonglandular hairs on fruit stalk were important for the identification of these drugs.

In the attempt to correlate clinical findings with serum levels of aldrin, sixteen patients were followed-up after acute intoxication by this agent. Eight of them, males and females, aged from 1 to 37 years, presented no or light symptoms (some discomfort and nausea). The serum of one of these patients was found to contain 16.6 ppb of aldrin and that of another, 1.41 ppb of dieldrin. A group of five patients, aged from two to 30 years, showed symptoms of moderate severity, reporting nausea, vomiting, drowsiness, dyspnea, sweating, mild jerking, rise in blood pressure and convulsions. Of these cases, two were accidental and three were attempted suicides, the majority achieving complete recovery within 24 hours. Serum levels of aldrin were between 6.98 ppb and 26.3 ppb and of dieldrin between 82.00 and 314.18 ppb. We found three severe cases, aged from 21 to 35 years, two attempted suicides and one occupational case. Two of these patients died and one of them presented hypothermia, coma, absence of reflexes and generalized convulsions, and another presented

reacting Donath-Landsteiner autoantibody. After a few days parameters of hemolysis had normalized, hemoglobin levels were within the normal range after 4 weeks. Eight months later the Donath-Landsteiner autoantibody could not be detected any more, the boy is healthy since then.
