Seven cases of Diphyllobothrium latum infection were proved by collection of worms after praziquantel treatment between October, 1986 and July, 1987. The patients were all males aged 20-44 years residing in Seoul or Ulungdo, Kyungpook Province. All of them had the history of eating several kinds of raw marine fishes, and they had never been to abroad. One of them experienced abdominal pain and 6 experienced natural discharge of a chain of worm segments, but none revealed any sign of anemia. Total 12 worms (1-3/patient) were collected after praziquantel treatment. The worms were 85-423 cm in length, and revealed the characteristic rosette-shape uterus in their gravid proglottides. The average egg size varied 61.0-65.3 x 41.7-46.1 microns. The eggs were yellowish-brown, and ovoid to elliptical. Including the present 7 cases, the total number of human D. latum infections proven by worms in Korea becomes 28 cases.

This study was based upon deceased patients in a geriatric university hospital with a high autopsy rate (81%). Of 6200 autopsied patients, 333 (5.4%) had had an active peptic ulcer; agonal and other acute erosions were not included. 257 cases were selected for the study (average age 83.8 yr). The diagnostic accuracy, and the symptoms of peptic ulcer in stationary, elderly, chronically ill patients were studied retrospectively. Only 16% of cases with duodenal ulcer and 29% with gastric ulcer had been correctly diagnosed antemortem. The clinical features of ulcer disease in the elderly may often differ from the standard presentation in younger people. Prior to death, appetite and weight loss, nausea/vomiting, anaemia and positive occult blood test had been more common among patients with ulcer, than abdominal pain and heartburn. The predictive values of single symptoms and of combined findings were low (range 2-21%), thus supporting observations from clinical practice that diagnosis is difficult in geriatric medicine. Prospective studies of ulcer disease in living elderly are needed.

A 41-year old man suffered from increasing attacks of colic-like abdominal pain. Laboratory tests revealed only slight anemia. Microscopic evaluation of blood smears was indicative of lead-poisoning. Elevated values for metabolites of heme-synthesis and an elevated lead concentration in blood confirmed the diagnosis. The source of poisoning could be found: lead leaching from a presumed "tin-pitcher" into cherry-brandy. After reduction of the plasmatic lead concentration with sodium-calcium-EDTA the patient became well within days.

Lead poisoning has accompanied the human being throughout history. Owing to the increasing levels of safety at work, the incidence of occupational poisoning has decreased and new forms of non-occupational poisoning have emerged. We present 3 cases of drug addicts, with lead poisoning, as a result of using adulterated drugs. One of them was an intravenous drug addict who had abdominal pain and anemia. The other 2 inhaled heroin, one being slightly anemic and the other without symptoms and with normal hemoglobin levels. The drug adulterated with lead had not been previously recognized as a source of lead poisoning, being likely to cause serious epidemiological effects.

A 65-year-old female with a history of cancer of the uterine body and a strong family history of cancer, was admitted with abdominal pain and anemia. During roentgenographic examination of the small intestine, a tumor was recognized about 25 cm from the ileocecal valve, leading us to suspect a carcinoma of the ileum. A colonoscopy demonstrated a constricting, ulcerated lesion, with a round wall: and a subsequent biopsy proved that the patient had a moderately differentiated adenocarcinoma. At surgery, a tumor, measuring 52 x 50 mm, was found to have infiltrated the serosa with a metastasis to the mesenteric lymph node.

Biliary hemorrhage may occur in a variety of clinical settings, but spontaneous hemobilia has not been reported from a cirrhotic liver. We describe a case of major hepatic hemobilia in a patient with cirrhosis and no history of trauma. A 50-year-old woman had abdominal pain, melena, and profound anemia. An extensive workup did not show the site of bleeding but did show a mass in the gallbladder. Cholecystectomy was performed, and at operation the patient was found to have cirrhosis and portal hypertension. The gallbladder "mass" was simply an organized clot, and hemorrhage recurred postoperatively. On reoperation, bleeding from the ampulla of Vater was observed, confirming the diagnosis of hemobilia. She was treated with angiographic interruption of hepatic arterial flow, at which time bleeding ceased. Her total transfusion requirements included 46 units of blood. Through 16 months of follow-up the patient has had no recurrent bleeding and no evidence of encephalopathy. This case demonstrates that spontaneous hemobilia may indeed arise from a cirrhotic liver. Proximal interruption of arterial flow is usually not recommended for hemobilia, especially in the presence of portal hypertension and cirrhosis, but may be life-saving in selected patients.

Three cases of a primary malignant lymphoma of the small intestine are reported. Case 1: a 62-year-old male who underwent an emergency operation for a jejunal perforation of a malignant lymphoma (aneurysmal type, Stage III, diffuse small cell type). The patient is still alive 16 months after the operation. Case 2: A 54-year-old female with complaints of an abdominal pain and anemia was found to have three tumors (one in the jejunum, two in the ileum) that were determined as being malignant lymphomas (all of the aneurysmal type, Stage III, diffuse medium-sized cell types). This patient is still alive 11 months after the operation. Case 3: A 40-year-old male with a complaint of an abdominal pain was found to have a malignant lymphoma of the ileum (aneurysmal type, Stage III, a diffuse medium-sized cell type). This patient died on the 60th day after the operation.

Cavernous hemangioma of the liver was diagnosed in 12 of 60 patients (20 percent) evaluated for surgery of neoplastic liver disease. All were female, from 29 to 77 years old. Six patients presented with abdominal pain and seven had taken estrogens. Indications for surgery included uncertain diagnosis, symptoms, large lesion greater than or equal to 6 cm, and hypoproliferative anemia. Three right lobectomies, one left lateral segmentectomy, one open biopsy, and one right trisegmentectomy were performed. There were no deaths, one subphrenic abscess, and one bile leak. The remaining seven patients were observed and at 2 to 6 years post operatively had followed a benign course. Resectional therapy may be considered for superficial large or symptomatic lesions in the appropriate patient, but most hepatic hemangiomas follow a benign course.

This is a clinical case report of a 23-year-old female admitted to St Paul's district hospital in North-East Zambia with a chief complaint of abdominal pain. Her past medical history included amenorrhea for 7 weeks, no vaginal blood loss, a previous delivery in 1986, postpartum abdominal pains which lasted for 3 months. On physical examination, she presented with slight anemia, normal temperature, normal blood pressure and a normal pulse. Pressure applied to the lower abdominal area was painful. Vaginal examination revealed an enlarged uterus; the rectovaginal pouch was extremely sensitive to pressure. Hemoglobin level was 8.5% and a pregnancy test was negative. On Laparotomy, a left sided ampullar tubal was found, with the right tube exhibiting an intact elastic swelling in the isthmus. Right salpingostomy was done to shell out the ectopic gestinal tissue and the left salpingectomy was performed to clean the recto vaginal pouch. The diagnosis made was bilateral ectopic pregnancy. The likely etiology was simultaneous ovulation from both ovaries, fertilization and subsequently implantation. The fallopian tubes had been previously affected by postpartum salpingitis.

A total of 126 children with chronic idiopathic thrombocytopenic purpura, including 35 splenectomized cases, were investigated in a long-term follow-up study, with regard to residual hematologic and immunologic abnormalities, complications and physical growth. Such hemorrhagic symptoms as petechiae, ecchymosis and epistaxis were still observed in about 22%-28% of the patients with a period of morbidity ranging from 3 to 15 years after onset. Residual thrombocytopenia below 150,000/microliters was found in 62% of patients within 5 years, 59% within 5 to 9 years and 57% within 10-14 years after onset. Other abnormalities were mild anemia, low serum level of IgA or IgM, positive antinuclear antibody, rheumatoid factor, and positive Coombs test in a small number of patients. Increased platelet-associated IgG was still obtained in patients with subnormal platelet counts whose morbid periods were 6 to 27 years after onset. Investigation of the patients by questionnaire revealed such complications as obesity, striae atrophicae, abdominal pain, headache, cataract, Perthes' disease, and cardiac complication in some patients. No apparent disturbances except for obesity were observed in their physical growth.
