The clinico pathological characteristics of colorectal carcinoma in 127 Black South African patients were studied. The main presenting symptoms were altered bowel habits in 70% of the patients, weight loss in 64.3% and

in 47.1%. Anaemia was present in 75.7%. There was a 31% incidence of mucinous carcinoma with a particular predilection for the younger age groups. Mucinous tumors were found more commonly in Duke stage C and D than in earlier stages. Tumors arising from a pre-existing

constituted to 5.5% of the lot of patients with cancer colon.

Patients with villous tumors of the ampulla Vater usually present with jaundice, intermittent or constant, but may seek care for
abdominal pain
, intestinal hemorrhage, or pancreatitis. Because villous tumors may harbor carcinoma in 30 to 50 per cent of cases, appropriate management may require radical resection. We have managed four patients with villous lesions of the ampulla Vater occurring in 1981, 1992, 1993, and 1995. Three were villous (two with malignant change) and one was a villoglandular

Treatment consisted of local excision with reimplantation of the ducts in one patient, Whipple resection in two patients, and biliopancreatic bypass in one who had distant nodal metastases not resectable for cure. This patient died 18 months after operation of an unrelated disease, but the others were well at last follow-up. The presentation as well as the diagnostic and therapeutic considerations in the management of villous tumors of the ampulla Vater are discussed.

Adrenal cysts are rare (0,064%-0,18% in autopsy series) and less than 500 cases have been reported in the western literature. Incidental diagnosis of adrenal cysts, however, is reported with increasing rates. We observed 12 patients with adrenal cyst. Each of them had a careful laboratory and instrumental evaluation; all the patients were operated. In our series about 67% of the patients were symptomatic (6 patients with

, 1 with palpable mass, 1 with hemorrhagic shock). No biochemical alteration was observed. Conversely we observed an unusual subclinically hyperfunctioning cystic

, potentially progressive to a clinically recognizable endocrine syndrome. US, CT and MRI had a sensitivity of 66,7%, 80% and 100% respectively. Adrenalectomy was performed in all patients. The pathological findings were: 1 epithelial cyst (cystic

), 2 endothelial cysts (vascular cystic ectasia with adenomatous adrenocortical hyperplasia and 1 vascular cyst) and 9 pseudocysts. On the basis of these results, we conclude that a careful hormonal, morpho-functional and instrumental evaluation is indicated in all adrenal cysts, even if the available diagnostic procedures, even when combined, cannot always define their nature. Surgical excision, when possible by laparoscopic approach, is indicated in presence of symptoms, endocrine abnormalities (even when subclinic), complications, suspicion of malignancy and/or large size (>5 cm). Adrenal gland must be excised en bloc, also because of the possible presence of other adrenal lesions.

We report four cases of tubular
adenoma
of the gallbladder with spindle cell metaplasia. Three patients had solitary polyps in the gallbladder and the fourth had multiple (three) polyps. Only one patient who had a polyp of larger size had
abdominal pain
. Histologically, these tubular adenomas had characteristic morular foci composed of short spindle cells. These spindle cell components lacked intercellular bridges or cytoplasmic keratinization. Immunohistochemically, the spindle cells stained positively for one or more cytokeratins and negatively for vimentin, muscle actin and S-100 protein. Spindle cells are considered to represent the metaplastic component of the
adenoma
cells.

at 32 weeks' gestation. Both patients accepted medical therapy initially, which resulted in poor control of hypercalcemia. Patient 1 delayed her parathyroidectomy until the postpartum period; she had maternal hypercalcemia and neonatal hypocalcemia. Patient 2 accepted parathyroidectomy at 32 weeks' gestation with an uneventful outcome for both mother and baby. Patient 3 was asymptomatic; her hyperparathyroidism was diagnosed postpartum after neonatal hypocalcemia and agreed to parathyroidectomy. All 3 patients had a parathyroid

Results of an epidemiological study concerning liver tumors in oral contraceptive users were reported to a recent American College of Obstetricians and Gynecologists meeting. The study involved 76 women with diagnosed cases of hepatocellular
adenoma
(HCA) and 3 age-matched neighborhood control women for each. Results indicated an increased risk of hepatoma of at least 200 times for women 27 or older who had used the pill 4 or more years and an increased risk for women 26 or less of 19 times the normal. The results, which confirmed an earlier case control study from the University of Southern California, showed the association between oral contraceptives and hepatoma to be stronger than the association between cigarettes and lung cancer. As a result of the study, the following recommendations were proposed: 1) all oral contraceptive users should have liver palpation in their physical examinations and be taught to report
abdominal pain
or masses; 2) women with resected HCAs should discontinue use of pills; and 3) women with diagnosed HCA should switch immediately to another form of contraception.

Endoscopic ultrasound (EUS) is proving to be a useful tool for evaluation of clinically suspected pancreatic masses unsatisfactorily evaluated by other means of imaging. We reviewed the records of 19 patients who had CT and EUS performed for clinically suspected pancreatic masses. Each patient had subsequent surgical exploration. Nineteen patients (11 females and 8 males) presenting with symptoms (11 with obstructive jaundice, 6 with
abdominal pain
and weight loss) or incidental CT findings suspicious for pancreatic carcinoma underwent EUS for further pancreatic evaluation. All of these patients had exploratory laparotomies, with 13 pancreaticoduodenectomies, 3 distal pancreatectomies and splenectomies, 1 bypass procedure, 1 open pancreatic and hepatic biopsy showing metastatic disease, and 1 open exploration with negative fine-needle aspiration biopsy. EUS correctly identified pancreatic neoplasms in 17 of 19 cases, with two false positives. The tumors included 15 adenocarcinomas, 1 microcystic

, and 1 lymphoma. Node status was correctly predicted in 9 of 12 specimens. Nine of 12 tumors had accurate tumor staging by EUS. Absence of vascular invasion was accurately predicted in 13 of 14 cases. Two patients had metastatic disease discovered at laparotomy. All 19 patients had preoperative abdominal CT scans, with six of these negative for pancreatic masses. EUS is more sensitive than CT in detecting pancreatic masses and is more accurate than CT in locally staging pancreatic tumors. This higher sensitivity is important because those patients with earlier stage tumors are the most likely to benefit from resection.

Benign tumors of the small bowel are rare. They present with many different manifestations depending on the size and location, and also cause a variety of symptoms that are often nonspecific. These include
abdominal pain
, dyspepsia, nausea, vomiting, and gastrointestinal bleeding that may be melena or hematemesis. Most of the time patients are asymptomatic and the lesions are discovered as an incidental finding. When bleeding occurs, and it may be severe in certain situations, the patient may develop signs of anemia, such as dyspnea, fatigue, and even high-output cardiac failure.
