An outbreak of Gnathostoma larva migrans occurred among guests of a New Year's party in Chachoengsao, Thailand. Nine people who consumed a raw fish dish called 'Hu-sae' contracted the disease. Five of them developed gastro-intestinal symptoms consisting of nausea, vomiting,
abdominal cramps
and diarrhea as early as within the first 24 hours, while in the other four, symptoms started on the following day. After the initial symptoms pertaining to the gut, malaise, chest discomfort,

, myalgia, weakness, itching and migratory swellings were experienced. Eosinophilia was demonstrated in every patient with a mean (+/- SE) count of 5,516 +/- 1,010 cells/cu mm. Detection of antibody against aqueous extracts of G. spinigerum adult antigen using an enzyme-linked immunosorbent assay showed a titer of 1:1,600 or greater in every patients except one who had a titer of 1:400 (positive greater than or equal to 1:400). This outbreak illustrates the high attack rate when heavily infected fish are consumed.

Curcumin has been used in traditional medicine as a household remedy for various diseases including biliary diseases,

, hepatic diseases, wound healing. For past few decades, extensive work has been done on biological activities of curcumin. This study was carried out to provide scientific basis for the use of curcumin in gastrointestinal disorders. Animals were divided into 5 groups (Group I--Group V), based on the time interval between administration of curcumin/vehicular fluid to administration of barium sulphate (Group I--1 hr, Group II--8 hrs, Group III--16 hrs, Group IV--24 hrs, Group V--48 hrs). Each group was further divided into two sub-groups, Group A (control) and Group B (experimental), containing 6 rats each. Rats in Group A were given vehicular fluid (0.9% NaCl) while the rats in Group B were administered curcumin intragastrically by the naso-gastric tube reaching up to the lower 1/3rd of esophagus, in the dose of 1 gm/kg body weight, suspended in normal saline. After the intra-gastric administration of single dose of curcumin, there was decrease in length of small intestine traversed by BaSO4 in all the experimental groups as compared to control groups. These data suggests that curcumin decreases intestinal motility in albino rats, and this may partly explain the traditional use of curcumin in different disorders like diarrhea,
abdominal cramps
and irritable bowel syndrome.

Medication-related nausea and vomiting (MRNV) is a significant problem in palliative medicine that is reasonably common likely due to the multiple medications that these patients are often taking.

BACKGROUND Systemic lupus erythematosus (SLE) has myriad manifestations that can affect any organ system in the body. Macrophage activation syndrome (MAS) is a disease of uncontrolled lymphocyte and macrophage proliferation and activation, which has various triggers, including autoimmune disorder, viral infection, and malignancy. We report here on MAS as a complication of adult SLE, a rare association in the literature, in a patient with an unknown past medical history. CASE REPORT A 38-year-old male patient presented with severe muscle weakness, diffuse

, and sweating increasing in severity for about 1 week. He was unable to give a coherent history and according to his family had been released from prison 3 weeks prior, having been in the corrections system for much of his adult life. The diagnosis of new-onset fulminant SLE complicated by MAS was made, noting the profound degree of bone marrow involvement, neuropsychiatric changes, and hyperferritinemia. CONCLUSIONS Many of the symptoms, signs, and laboratory findings of SLE overlap with those of MAS, and concomitant presence of both of these disease poses unique diagnostic challenges as well as extreme risk to the patient. A robust set of criteria for identifying MAS in the setting of a confounding underlying rheumatological illness does not exist in the adult population; this case illustrates the approach taken by our team to come to this diagnosis.
