A retrospective review was completed on 43 patients with metastatic lesions to the gastrointestinal tract between 1980 and 1993. The most commonly involved organs were stomach and small intestine. The main features were abdominal pain (70%), acute abdomen (42%), alimentary bleeding (21%), massive in 11.6%). Operations were performed on 42 patients (palliative resection in 40 patients, enteric bypass in 2). The results support surgical intervention for patients with complaints of gastrointestinal metastases, especially in emergency patients.

Mesenteric venous thrombosis is a rather rare condition posing diagnostic problems and very often with an obscure etiology. Recently, various coagulation deficiencies were found as the main etiologic factor. A case of a 75-year-old man with acute abdomen is presented. Exploratory laparotomy revealed a 250 cm necrosis of the small intestine. Patient was successfully treated by resecting the entire involved segment of the bowel. After a complete coagulation profile examination, deficiencies of protein C and anti-thrombin III were found and were considered as the main cause of the disease. In cases with known coagulation deficiencies the presence of an acute abdomen should raise suspicion of mesenteric venous thrombosis.

We report a case of acute abdomen due to puncture of a dermoid cyst during oocyte aspiration, which required laparotomy. A woman who had undergone an in-vitro fertilization and embryo transfer required hospitalization due to onset of an acute abdomen. An ultrasonographic scan showed a pelvic mass with the features of dermoid cyst. The patient required diagnostic laparotomy which confirmed the presence of a ruptured dermoid cyst with subsequent peritonitis.

We report a case of high 67Ga-citrate accumulation with myocardial metastasis in the right ventricle from uterine cervical carcinoma. A 41-year-old woman was admitted to our hospital because of acute abdomen after operation and radiotherapy for uterine cervical carcinoma. On performing 67Ga-scintigraphy for differentiation from fever of unknown origin, we found significant 67Ga-citrate accumulation in the right ventricle. To the best of our knowledge, this is a rare case.

From 1987 to 1991 a total of 5035 pelviscopies were done at the Kiel University Hospital of Gynaecology. More than 90% of these were surgical pelviscopies, whereas less than 10% were diagnostic procedures only. Serious complications requiring laparotomy or repeat pelviscopy or laparotomy within 4 weeks after the original procedure, occurred in 2.0% of the cases and slight complications were seen in 2.9%. Procedures for acute abdomen or adhesiolysis after multiple laparotomies bore the greatest risk of serious complications. Low risk was associated with sterilisations, procedures for female sterility, endometriosis or tubal pregnancy.

Case report of a giant retroperitoneal liposarcoma which presented with a picture of acute abdomen resulting from a massive tumoral haemorrhage. Diagnosis was achieved by ECO and CT, and was confirmed by Pathological Anatomy. Treatment was surgical, performing tumour removal plus polar nephrectomy of the affected side. The patient evolution was favourable. The standard methods of diagnosis and prognostic factors are commented.

A rare case of acute abdomen caused by the rupture of a solitary, non-parasitic cyst of the liver is reported. Considering the rarity of the event, it was considered interesting to examine the anatomo-pathological, clinical and diagnostic aspects as well as the various surgical and other treatment possibilities for these conditions. Stress is also laid on the fact that surgical treatment is only indicated in the case of complications, while in uncomplicated forms the treatment of choice of alcoholisation.

A 31-year-old pregnant woman who was an active cocaine abuser presented to our emergency department five times in 1 week for abdominal pain and vomiting. She continued to use cocaine regularly despite having abdominal pain. Her fifth admission was for seizures. There were no objective signs of peritoneal inflammation and the rectal guiac examination was repeatedly negative. The patient progressed to severe septic shock. Intraabdominal sepsis and/or bowel perforation was suspected. Exploratory laporatomy revealed gangrene and perforation of the small intestine and fecal peritonitis. She rapidly developed multiorgan failure and died. Gastrointestinal complications resulting from cocaine use are uncommon. Our case is unique in that the patient was pregnant, used cocaine by the intranasal route, and lacked objective signs of acute abdomen. Emergency physicians should be aware of the morbidity associated with the use of cocaine.

Angiostrongylus costaricensis (classified under the more specific genus Parastrongylus by different authors) is a filiform intestinal nematode endemic to Central and South America, which afflicts primarily the pediatric population 1 to 13 years of age. Only three cases of adult infections have been previously documented outside the endemic region. To our knowledge, we report the first such case in the western United States, as well as the oldest patient yet recorded. Our patient is a 73-year-old woman living in Los Angeles who presented with an acute abdomen 4 weeks following a 5-month visit to El Salvador. Examination of the gross ileum removed at exploratory laparotomy revealed a perforation associated with a segmentally thickened intestinal wall. Microscopic examination showed eosinophilic ileitis, arterial lumens containing nematodes morphologically consistent with A costaricensis, eggs in the submucosa, and arteriolitis with thrombosis.

A 5-year-old boy presented with episodic, postprandial abdominal pain and hypertension. A few days after the onset of symptoms, the pain became more severe, and progressed into a picture of acute abdomen and intestinal obstruction. Urgent laparotomy findings showed the presence of small bowel gangrene. Pathology findings of the superior mesenteric artery (SMA), which was found to be occluded, showed intimal fibroplasia. The patient died 7 weeks after the onset of symptoms.
