We report on 3 patients who presented with an acute onset of abdominal pain, a palpable abdominal mass and a rapid decrease in hemoglobin. In 2 patients a spontaneously ruptured hypernephroma was found and the other patient had a squamous cell carcinoma of the ureter with bleeding into the tumor. Although in all 3 cases the tumors were at an advanced stage of development, the patients had been entirely free of urological symptoms until shortly before hospitalization. The importance of considering the possibility of spontaneous rupture of such a tumor in the evaluation of cases of an acute abdomen is stressed.

Case histories of dead patients were analyzed in whose life-time errors were made in diagnosis of acute abdomen. It was established that in 90 patients acute abdomen was hypodiagnozed, in 10 cases the abdominal catastrophe was hyperdiagnozed, in 4 patients the localization of the injured organ was not determined properly. The error found mainly resulted from atypical course of the disease due to age of the patients, medication effects and abnormal disposition of the injured organ, as well as to concomitant diseases and complications. The role of the attention factor in the genesis of errors is emphasized.

One hundred cases of acute abdomen in patients over 70 years of age were studied. Mechanical occlusion was the most common cause among these patients, as opposed to acute appendicitis, which is the primary cause of the same condition among young people and adults. Irreducible hernias and visceral neoplasias were the most frequent etiologies among cases of mechanical occlusion. Cholecystitis took second place in our series. Visceral peforations were more often due to localized infections or visceral neoplasias than to gastric or duodenal ulcers. Overall mortality among these patients was 29 percent. Patients with visceral perforations due to localized infections or visceral neoplasias and all of the patients with vascular diseases had a negative prognosis.

This paper concerns blunt injuries of the abdomen in Gorenjsko (a Region of SR Slovenija northern of Ljubljana) treated during the past 3 years. Of the total 131 hospitalized patients, 51 (or 43.5%) were operated on. In 5 (8,8%) an explorative lapartomy was performed in which no pathological substrate was found. The most frequent lesions were found in the spleen, liver and small intestine. The paper stresses the importance of an early and correct diagnosis of such injuries. In making the exact diagnosis aside from the physical evaluation, the most important procedure is lavagage of the abdomen which the authors used in all cases suspect for acute abdomen and obtained excellent results. In 20% of the patients surgery was performed within 2 hours of their of hospital admission. Injuries of the liver were treated with necrectomy, ligature of the bleeding vessels, suturing of the biliary canals, and establishment of drainage. Ligation of the hepatic artery and hepatic resection was performed in two cases. Injuries of the pancreas were also treated with necrectomy, hemostasis and drainage. In one case with a posttraumatic pseudocyst of the pancreas surgery was performed with an isolated intestinal loop according to Roux-Herzen. In conclusion the author insists on a prompt diagnosis in which abdominal lavage plays an important role. Using such an approach the morbidity is significantly reduced.

Acute appendicitis was once thought to be rare among rural blacks. It is now known that appendicitis is relatively common among Africans and in Africa. At the University College Hospital, Ibadan, Nigeria, appendicitis is the most common cause of acute abdomen on the surgical service.One hundred and eighty-one cases of appendicitis were operated on in a two-year period from June 1975 to June 1977. A retrospective analysis of 47 fully documented cases showed that wrong diagnosis occurred more often in females than in males. There was a high incidence of perforation (31.9 percent) in this series. Deaths occurred in perforated cases and cases complicated by typhoid perforation of the terminal ileum. Parasites and their ova were often present in the lumen of some appendices.The age group most affected was 16 to 20 years. This disease affected low, middle and upper income groups in our society. Chronic, recurrent appendicitis was frequently diagnosed (31.9 percent) and history of recurrent right lower quadrant pain as far back as three months was often elicited from these patients.

Volvulus of the spleen is a complication of a previous congenital anomaly consisting basically in the absence of supporting structures of the spleen. Clinically, it is an extremely rare complication. It may present as an acute abdomen. The search of the literature that we conducted does not show any such case being the cause of portal hypertension. This motivated this case report. The patient was treated surgically and this cured the patient.

The clinical picture of solid tumors of the intestinal mesentery is varied and depends on the character of growth (benign, malignant), size and anatomical variants of tumor localization in the mesentery. Sometimes, the tumors concerned can induce pseudosyndrome of acute abdomen or inflammation of the adjacent organ secondary involved by tumor. Among complications induced by tumor are as follows: intestinal hemorrhage, peritonitis and acute or partial intestinal obstruction. The roentgenological picture of intestinal mesentery tumors shows some characteristic features. Eight observations of the author are reported.

A 55-year-old female, previously cholecystectomized, was admitted to hospital with signs of severe acute abdomen. At laparotomy, bile peritonitis was found, but the site of leakage could not be discovered. The biliary tree was otherwise normal and without stones. The treatment was confined to drainage of the common bile duct by inserting a T-tube. In spite of intensive postoperative care the patient died. Autopsy showed a perforation opening in the common hepatic duct on the posterior wall and a solitary stone was detected in the adjacent retroperitoneal tissue. The mechanism to this rare condition is discussed and a review of earlier similar cases is given. The benefit of routine cholegraphy in order to avoid retained stones is briefly mentioned.
