The general surgeon has sometimes to face problems arising from an acute abdomen due to gynecologic causes. Such conditions are mainly found in women in reproductive age; the most frequent pathologies are due to complications of ovarian cysts, perlvic inflammatory disease and extrauterine pregnancy. Some short clinical commentaries are herein presented on two cases of gynecologic acute abdomen: the first case reported is related to an intraperitoneal rupture of a large uterine sarcoma and the second an ovarian neoplasm associated with a diffuse peritonitis from perforation of tubo-ovarian abscess.

Ovarian tumour is rarely seen in paediatric age group and primary ovarian non-Hodgkin's lymphoma is very uncommon. A child presenting with acute abdomen was detected to have bilateral solid ovarian tumours. Because of torsion of the right ovarian tumour and complete replacement of the ovarian tissue by the tumour on left side, bilateral salpingo-oophorectomy was done. Bilateral primary ovarian lymphoma was diagnosed on histopathological examination. Immunohistochemistry was suggestive of B-cell lineage. She had eight cycles of chemotherapy and at 28 months follow-up doing well without any sign of any recurrence or any evidence of metastasis.

Mature cystic teratoma is the most common ovarian neoplasm. However, mucinous cystadenoma with teratoma has been very rarely reported in literature. This case report, which is very rare, describes a clinical entity not previously reported in literature. A 34-year-old pregnant woman presented in the 23rd gestational week with severe right lower quadrant pain. She was diagnosed with acute abdomen and was then treated surgically. During the surgical intervention, a spontaneously ruptured mass was detected in the right ovary. This was reported histopathologically as a mature cystic teratoma in collision with mucinous cystadenoma. To the best of our knowledge, this case report is the first to have identified a ruptured mature cystic teratoma in collision with mucinous cystadenoma in a pregnant woman.
