Herpes simplex hepatitis is a rare disease in adults and with high mortality (81%). Most cases are seen in immunocompromised hosts or in the third trimester of pregnancy. A 43-year-old woman with psoriatic arthritis, treated with prednisone and methotrexate (MTX), presented with an acute abdomen and elevated liver enzymes. The diagnosis was made by liver biopsy. MTX was stopped and acyclovir was started. The patient survived. The diagnosis of Herpes simplex should be suspected in immunocompromised hosts and established by liver biopsy and viral cultures. Treatment includes parenteral acyclovir and discontinuation or reduction of immunosuppressive therapy.

A case of non specific idiopathic ulcer of the small intestine in a 79-year-old patient is reported. On admission he presented with acute abdomen and was submitted to wide intestinal resection. The features of this rare disease (only 370 cases described until 1987), which has an undefined etiopathogenesis, severe symptoms, and poses diagnostic problems, are discussed.

The authors describe a case of acute gangrenous cholecystitis in a 17-month-old boy manifested by symptoms of an inflammatory acute abdomen after several days of a febrile condition associated with diarrhoea. Cholecystectomy was performed and after antibiotic and infusion therapy a noncomplicated postoperative development followed. Five years after operation the boy has no subjective complaints. In the discussion the authors analyze the incidence, aetiopathogenesis, clinical, laboratory and X-ray findings of this rare disease. For the therapy of advanced cholecystitis the authors recommend cholecystectomy.

The rectus sheath hematoma is a generally rare disease, however the frequency of occurrence has increased with introduction of anticoagulant therapy. It almost always mimics the symptoms of acute abdomen. In 7 out of 14 cases, which we observed, an anticoagulant therapy was administered. During this therapy 5 rectus sheath hematomas occurred spontaneously. In the remaining 2 patients severe coughing attacks were reported additionally. The case histories of the other 7 patients included 3 patients with bronchitis, 2 patients with trauma and 2 patients without a relevant history ("spontaneous"). The correct diagnosis of rectus sheath hematoma could primarily be revealed by sonography in 8 of 14 patients, after which an appropriate therapy followed. In 6 patients a laparotomy was performed, because rectus sheath hematoma was not considered. The correct diagnosis was found intraoperatively as a surprising assessment. By inclusion of rectus sheath hematoma in the differential diagnosis of acute abdomen and the verification by sonography an emergency laparotomy because of a false diagnosis in the often severe ill patients can be avoided in favour of a minor and more appropriate procedure.

Primary mesenteric venous thrombosis is a rare disease and it is difficult to diagnose preoperatively. In this report, we present a case with interesting sonographic and computed tomographic findings. On March 15, 1984, a 53-year-old male was admitted to Central Hospital of Social Insurance because of intracranial hematoma, which was removed immediately. On May 17, the patient complained of a vague abdominal pain, in the left upper quadrant. Seven days later, the pain became severe and he was referred to our department. White blood cell count was 19300/mm3. On ultrasound and computed tomographic examination, a rigid thick-walled bowel loop was visualized. Laparotomy was performed and a gangrenous bowel loop (35cm long) was resected. Postoperative course was well. It should be emphasized that ultrasound and computed tomography are noninvasive and may be very helpful for differential diagnosis in the cases of acute abdomen.

The Authors report an exceptional case of Atrophic Malignant Papulosis (P.A.M.) or Degos disease, presenting as acute abdomen with ileal and multiple colic perforations. The disease is due to an occlusive, progressive, thrombotic vascular pathology affecting different parts of the body and may bring to death. Usually this rare disease presents with typical multiple papulous skin lesions, followed by a systemic involvement of the digestive, nervous, and visive tracts. Peritonitis and cerebral infarction are the most frequent causes of death.

Ogilvie's syndrome (acute colonic pseudo-obstruction) is a rare clinical disease characterized by segmental distension of the proximal colon caused by a paralysis without mechanic obstruction. It may be a sequel of underlying neurological, medical or surgical disease. Risk factors are respiratory decompensation, electrolyte disturbances and different drugs. A special kind is the primary idiopathic pseudoobstruction with a high risk of perforation or necrosis. Especially elderly patients (> 70 years) with cardiovascular or neurologic diseases and accordant drugs are concerned. Clinical symptoms are progressive abdominal distension and abdominal pain like an acute abdomen. The differential diagnosis of a mechanic ileus is important for further treatment. This case report should draw attention to this rare disease.

Enteric anisakiasis is a relatively rare disease that is difficult to diagnose preoperatively. We report a case of small bowel obstruction caused by enteric anisakiasis in a 59-year-old Japanese man who presented with abdominal pain a few hours after eating sliced, raw fish. Because of signs of an intestinal obstruction, a laparotomy was performed. Focal thickening and stenosis of the ileocecal region were seen about 100 cm from the end of the ileum and the lesion was excised. We found a moving anisakis thrusting its head into the mucosa of the excised small intestine. Histopathological examination revealed the infiltration of eosinophils in all layers of the intestinal wall and severe edema. Enteric anisakiasis is very rare, and its diagnosis is usually only made after laparotomy. Nevertheless, when signs of acute abdomen develop after the ingestion of raw fish, such as sushi or sashimi, the possibility of enteric anisakiasis should be borne in mind.

The Authors report a case of acute abdomen caused by a perforated solitary diverticulum of the cecum recently treated. This is a rare disease that usually is diagnosed, in the surgical theatre because it is a topic of emergency surgery. From Literature analysis it is obvious that a correct preoperative diagnosis is difficult to make because of the large number of possible abdominal pathologies with similar symptoms. Also the therapeutic approach is so extremely variable that the operation could range from a simple conservative type to right hemicolectomy. A proper approach will be conservative with a simple diverticulectomy when the inflammatory reaction is localized in the colonic wall while a right colectomy could be performed when the inflammatory reaction is more advanced or a large mass suggestive of a carcinoma is present.
