A 72-year-old male with tuberculous empyema underwent omentopexy following thoracic window. However,
acute abdomen
developed 6 days after the latter surgery. During emergency laparotomy; a large perforation (8.5 x 3cm) was found at the great curvature of the stomach. Histology of the resected specimen showed this was
ischemic ulcer
, which was thought to have been caused by circulatory failure at the areas supplied by the right gastroepiploic arteries after pedicled omental flap. This case suggested that much attention should be paid not only to the blood circulation of the graft but also to that of the stomach.
