This is a case report documenting the risk in imaging misinterpretation of a pediatric patient that presented with an acute abdomen. Computed Tomography (CT) demonstrated an inflamed blind ending loop of bowel in the pelvis without an obvious cecal connection. The patient was taken to the operative theater, a normal appendix and perforated Meckel's diverticulitis were resected. Meckel's diverticulum is the most common small bowel abnormality and can have complications. We will emphasize that imaging studies in a pediatric patient with Meckel's diverticulum are easily subject to errors radiologists make. This is such a cautionary and learning tale.

Littre's hernia is a rare condition that involves Meckel's diverticulum in the hernia sac. Meckel's diverticulum is the true diverticulum of the small intestine. Neuroendocrine tumors may develop in it; however, there are very few reported cases. In this study, we present a case of neuroendocrine tumor in strangulated Littre's hernia, on which we did not find a study when we reviewed the relevant literature. A 71-year-old male patient presented to our outpatient clinic with complaints of left groin pain and swelling in the groin. Acute abdomen findings were also present in the abdominal examination of the patient. The patient was operated on immediately and it was observed during the operation that the herniated Meckel's diverticulum was perforated. Segmental small intestine resection was performed. The pathology results of the patient revealed a well-differentiated neuroendocrine tumor with mucosal and submucosal localization in Meckel's diverticulum. We believe that if Meckel's diverticulum is found in the hernia sac in incarcerated hernias, it must be completely resected as a neuroendocrine tumor may develop, even if only rarely.

The health emergency linked to the Sars-Cov-2 infection represented an absolutely new problem for all health professionals. In particular, the information regarding the spread of the virus in the pediatric field and its manifestations are still incomplete. In this paper we present a case of neonatal infection which, as far as we know, represents one of the few published cases and which occurred in a patient who came to our attention for acute abdomen from intestinal perforation. The perforation was caused by Meckel's diverticulum, an event considered infrequent in the first year of life and almost exceptional in the neonatal period. This case required particular management, putting pediatric surgeons in front of new and difficult to solve problems. New onset clinical events, such as this one described, represent an opportunity for sharing useful data for the creation of universal protocols for the management of patients with problems that are becoming common and of which little is known.

Spontaneous perforated Meckel's diverticulum is a rare complication and mimics appendicitis or diverticulitis in elderly adults. We report the case of a 51-year-old man who presented to the emergency department with abdominal pain that had progressively increased over the previous two hours. The provisional diagnosis was appendicitis or diverticulitis, and we recommended computed tomography for further evaluation. Computed tomography of the abdomen showed an outpouching, blind-ending, fluidfilled structure in continuity with the distal ileum that depicted wall perforation and extraluminal air bubbles, consistent with perforated Meckel's diverticulum. In this case report, computed tomography provided a clear imaging diagnosis of perforated Meckel's diverticulum preoperatively. Thus, in patients with acute abdomen, complicated Meckel's diverticulum should be included in the differential diagnosis.

Meckel's diverticulum is the most common congenital anomaly of the GIT with a low incidence of 2% and the complication rate is even lower with perforation being the rarest. We report an intriguing case of a 15-year-old male, who presented with one-week history of high-grade fever and diarrhoea followed by acute onset of abdominal pain in the periumbilical region which became generalized. On physical examination his abdomen was distended with guarding and rigidity. A provisional diagnosis of peritonitis secondary to enteric perforation was made and exploratory laparotomy was done which revealed a perforated Meckel's diverticulum and advanced peritonitis. A diverticulectomy with double barrel ileostomy were performed. No heterotopic tissue in the diverticulum was noted on histopathology, nor any other abnormal tissue identified. The patient made an uneventful recovery postoperatively and ileostomy reconstruction was done two months later. This case report is rare case of Meckel's diverticulum complications and highlights the importance of considering Meckel's diverticulum as a differential diagnosis in every patient presenting with acute abdomen, which can aid toward better management through laparoscopy.
