Acute appendicitis is one of the most frequent causes of acute abdomen. Event though its mortality has been significantly reduced, the high percentage of negative laparotomies changed in the last 40 years reaching at present an average of 20-25%. Several radiological methods have being proposed in order to improve a diagnostic effectiveness, however they have showed a limited value. Computerized tomography and ultrasonography has been used, mainly for the diagnosis of complications of acute appendicitis and few studies related to its utility in early diagnosis have been published. The technique of ultrasonographic examination and the characteristics of the normal and inflamed appendix are discussed as well as the sonographic appearance of appendicular mass, sensibility and method controversy.

Cecal diverticulitis is an uncommon entity. Its operative treatment represents 0.2% of procedures performed for an acute abdomen. The clinical presentation is often indistinguishable from acute appendicitis. At operation, it may be confused with cecal carcinoma. The surgeon must be aware of this condition and be prepared to choose the most appropriate treatment. Local excision has been advocated as the treatment of choice. The authors review 18 cases seen over a 10-year period. In no case was the correct diagnosis made preoperatively. Intraoperatively, a correct diagnosis was made in 12 of the 18. Carcinoma was the next most frequent intraoperative diagnosis (four cases). Twelve of the 18 patients were treated by standard or limited right hemicolectomy. One patient died early in the series of sepsis caused by a perforated diverticulum and one patient had a life-threatening complication. Right hemicolectomy appears to be a safe and effective treatment option for cecal diverticulitis.

The authors present the case-history of a six-week old boy born during the 30th week of gestation with the coincidence of an incarcerated right-sided inguinal hernia and acute appendicitis. On operation it was found that the appendix was intra-abdominally fixed. The authors draw attention to the difficulty of diagnosis of an inflammatory acute abdomen in neonates and infants.

A prospective study was conducted of 276 patients with suspected acute appendicitis. Two hundred and eighteen patients had operations and 165 had acute appendicitis. Data from a previous retrospective study were used to develop an eight-item statistical program for a pocket calculator and this was made available to half the surgical registrars as an additional aid to clinical decision making. The other registrars involved in the study used only their clinical skills. There was no difference in diagnostic accuracy or frequency of operations or complications between patients managed by the two groups. Forty items of clinical data on each patient were collected prospectively. These were subsequently analysed on a computer program for the diagnosis of the acute abdomen which was developed in the United Kingdom. The accuracy of diagnosis of the clinicians was superior to the accuracy of the computer diagnosis.

A six years old girl was rushed to hospital with an acute abdomen. Because of the age of the patient, the clinical examination and the usual biology we diagnose an acute appendicitis. The fortuitous measuring out of the pancreatic enzymology allows us to correct our first diagnostic into the one of pancreatitis with angiocholitis. The check-up shows a congenital choledochal cyst with an abnormality of the choledochus-Wirsung junction explaining the physiological pathology presented. The surgical operation was made up of an cystectomy with cholecystectomy and hepatico-jejunostomy according to an Y shaped loop from Roux. Pancreatitis diagnostic is unusual in childhood, there is a good reason to suspect in those circumstances the existence of a choledochal cyst.

Interstitial pregnancy is an infrequent type of ectopic pregnancy. Two cases of acute abdomen due to ruptured interstitial pregnancy following salpingectomy, a rare event, are presented. Both patients were young women who had had a previous salpingo-oophorectomy due to ovarian cyst. The presenting symptoms were in the first case abdominal pain followed by hemorrhagic shock, and in the second a right iliac fossa syndrome simulating acute appendicitis. Surgical intervention consisted of wedge resection of the uterine segment involved. A review of the literature and practical considerations are presented.

Acute suppurative infection of the sacroiliac joint can mimic the acute abdomen. Experience with a patient who had exploratory laparotomy for acute appendicitis prompted a review of the literature, which indicated that 12.6% of patients with pyogenic sacroiliitis have acute abdominal pain. A high index of suspicion is necessary to establish this diagnosis.

We report a case of eleven years old child with an acute lymphoblastic leukemia who developed an acute abdomen while in haematological relapse. He had an acute appendicitis with leukemic infiltration of the coecum. The early surgical intervention and the coecum irradiation were successful: the following chemiotherapy achieved remission. A more aggressive surgical approach to the acute abdomen, combined with careful supportive measures, might improve the usually grim prognosis in patient whose leukemia offers a chance for remission.

The authors report a case of a primary torsion of the omentum in an eleven-year old child who had been operated with a diagnosis of acute appendicitis. They take the opportunity to re-examine this idiopathic lesion with a review of world literature and case reports and they discuss the etiology, pathogenesis and pathology. The importance for the surgeon to inspect the omentum in all the so-called "negative" acute abdomen is enhanced.
