Familial Mediterranean fever is an autosomal recessive disease characterized by recurring inflammatory attacks of synovial membranes. More than 95% of patients show peritoneal involvement which mimics
acute abdomen
and can sometimes cause unnecessary surgical intervention. The authors present two patients with the diagnosis of familial Mediterranean fever who underwent surgery because of rare abdominal complication of the disease. Two patients with the diagnosis of familial Mediterranean fever underwent laparotomy, and segmental small bowel resection was done because of the necrosis. Adhesive intestinal obstruction with associated bowel strangulation and
volvulus
is a rare complication of familial Mediterranean fever, and this life-threatening emergency must be kept in mind.

A case of
acute abdomen
due to diastasic perforation of the right colon by
volvulus
on mesenterium commune with associated agenesis of the left kidney and urogenital malformations in a young woman is reported. The case emphasizes the complexity and the polymorphism of congenital abdominal malformations, particularly predisposing to
volvulus
, that can represent a really challenge for emergency surgeon. The A.A. analyze literature about congenital intestinal anomalies to evaluate clinical, diagnostic and therapeutical problems, and they remark features to take into consideration in emergency: clinical history in non-operated patient of abdominal pain and biliary vomit with spontaneous resolutions; nutritional compromise; hypothetical diagnosis of anorexia or food intolerance; malformations in other organs or systems and, one mostly, young age. The correct interpretation of these symptoms can suggest suspicious of an occlusive complication due to intestinal malformations. Radiological data and the exploratory laparotomy or laparoscopy will confirm our intuition. If otherwise diagnosis of the cause underlying the chronic recurrent abdominal pain are delayed, surgical operation in emergency condition will be more aggressive and anatomical structures sacrificed.

In this case report, we describe a pregnant female patient at 27 weeks' gestation with a rare complication of
volvulus
. Her initial symptom was acute cramping pain of the lower right abdomen. Surgical intervention was performed under the impression of peritonitis. Pathologic diagnosis revealed
volvulus
of the mid ileum. The remainder of her pregnancy was uneventful. The physiologic changes of pregnancy may predispose the parturient to bowel obstruction due to compression of the gravid uterus against the intestine. If
volvulus
is suspected, then emergent surgery should be performed. Delays in treatment may result in septic shock and even death. We present this case to remind obstetricians of such rare causes of
acute abdomen
during pregnancy.

We report a case of a 38-year-old woman who presents at 31 weeks of gestation with acute abdominal pain. Computed tomography (CT) scan of the abdomen and pelvis showed a cecal
volvulus
. Emergency cesarean section was performed for fetal distress. Laparotomy confirmed the CT findings and a right hemicolectomy was performed. While not advocating overutilization, this case demonstrates that helical CT can be, in the correct clinical scenario, an acceptable, useful, and relatively noninvasive test in the accurate assessment of the
acute abdomen
in pregnancy. We review the safety and utility of medical diagnostic imaging in the assessment of the
acute abdomen
in pregnancy.

Cecal
volvulus
is an uncommon cause of intestinal obstruction and is rarely diagnosed correctly at the time of presentation. We report a case in an 86-year-old man presented with an abrupt onset of lower abdominal distension, preoperatively diagnosed as cecal
volvulus
by abdominal CT. Surgery revealed a 20 cm length of the closed-loop that had rotated in a counter-clockwise rotation. A right hemicolectomy with colonic fixation was subsequently performed; early diagnosis enabled this procedure to be performed in a timely fashion. This case demonstrates the need for clinicians to consider cecal
volvulus
as a possible cause of
acute abdomen
. Performing abdominal CT studies in such patients may enable early diagnosis and prompt surgical intervention.

A <<mobile>> liver is an exceptional situation in which the laxness or absence of the ligaments supporting liver allow it to move in the transversal plane so that it is always located on the dependent side. This condition is usually associated to a long and lax mesocolon, which is usually responsible for the symptoms. Very few cases have been described in the literature. To our knowledge, this is the first report of the computed tomography (CT) findings of a mobile liver with
acute abdomen
caused by associated colonic
volvulus
.

Volvulus
of the gallbladder has been reported mostly in elderly women and is rarely reported in children. In this paper, we report a recent case of successful detorsion and removal of the gallbladder via minimal access surgery, which was diagnosed as gallbladder
volvulus
preoperatively by means of magnetic resonance imaging (MRI). An 11-year-old boy presented with an
acute abdomen
, and acute cholecystitis was suspected, based on the findings of ultrasound and computed tomography. However, a diagnosis of gallbladder
volvulus
was established through MRI, and emergency laparoscopic surgery was subsequently performed. Laparoscopy revealed a hemorrhagic, gangrenous gallbladder that floated anteriorly. The gallbladder was rotated twice around its pedicle in a counterclockwise direction. It was untwisted through laparoscopy, and a cholecystectomy was performed without difficulty. To achieve an early, prompt preoperative diagnosis, MRI could be an effective alternative to conventional imaging modalities. The advent of laparoscopic detorsion and cholecystectomy as a treatment for gallbladder
volvulus
allows less invasive treatment, a shorter hospital stay, and better cosmesis, which are highly desirable in children.

A case of acquired megacolon in a 62-year-old man with
acute abdomen
due to sigmoid
volvulus
is reported. The case was associated with the use of psychiatric medications. The aim in this report was to emphasize the differential diagnosis with Chagas megacolon. Anatomopathological examination did not show any evidence of denervation, ganglionitis and/or myositis, and the serological test for Chagas disease was negative.

Acute abdomen
is an emergent condition in the emergency department, and it is mandatory to evaluate it immediately and treat it without delay. Pneumoperitoneum is usually attributed to perforation of the gastrointestinal tract. However, intra-abdominal, gynecologic, urologic, and miscellaneous pathogenesis not related to a perforated gastrointestinal tract had never been described in the past. Approximately 10% of pneumoperitoneum is not associated with hollow organ perforation. There are many imitators of pneumoperitoneum including subphrenic abscess, colon
volvulus
, Chilaiditi syndrome, and so on. In our case, the gas-forming bacterial peritonitis accounted for the pneumoperitoneum. We presented an 85-year-old man who received laparotomy due to peritonitis, and radiographic subphrenic free air was seen. However, a large amount of ascites was found rather than perforated bowels during the surgical exploration, and the culture of ascites was positive for Pseudomonas aeruginosa.

We present a case report involving an unusual type of
volvulus
. A 51-year-old man presented with an
acute abdomen
. He underwent emergency laparotomy at which we identified a
volvulus
involving his caecum to descending colon in continuity. We believe this to be first reported case of total colonic
volvulus
.
