in a 38-week pregnant woman is reported. Ultrasound showed a localized cystic lesion with internal echoes on the right side of the abdomen. It was diagnosed preoperatively as twisted

The authors report their experience with US in gynecologic emergencies through a retrospective study on 105 patients presenting with
acute abdomen
of suspected gynecologic nature. The series included 3 groups of patients: Group I: 59 patients all submitted to immediate surgery. The following pathologic conditions were observed: ectopic pregnancy (23 cases), torsion or hemorrhage from
ovarian cysts
(13 cases), pyosalpinx or tubo-ovarian abscess (9 cases), torsion of pedunculated uterine leiomyoma (7 cases), intraperitoneal bleeding from hemorrhagic corpus luteum (6 cases), hematocolpos and hematometra from imperforate hymen (3 cases). Two false positives, not included in this group, resulting from appendicular abscesses and misinterpreted as ovarian, were submitted to surgery in a gynecologic unit. Group II: 19 patients treated with medical therapy for the following conditions: torsion or hemorrhage from hyperstimulated ovary (10 cases), pyosalpinx or tubo-ovarian abscess (9 cases). Group III: 25 patients in whom neither US nor clinical examination revealed positive gynecologic findings. Both US and clinical follow-up were negative in these patients. The study was aimed at evaluating the role of US in identifying both lesion and peritoneal involvement, and in the diagnosis of nature. US proved a valuable tool in the first two diagnostic steps, allowing to confirm/dismiss active pathologic conditions, to indicate the medical/surgical treatment (immediate or delayed), to detect associated pathologies to study with further examinations. As for lesion nature, US alone proved poorly useful if not correlated with an accurate clinical history.

Seventeen girls were treated following the diagnosis of
ovarian cysts
. Four patients were operated on within the first 6 months of age and the other 13 patients were 10-15 years old at the time of diagnosis. Antenatal diagnosis was made in 3 cases. The presenting symptoms in the infants were distended abdomen in 3 cases, abdominal pain in 1 and vomiting in 1. In the older children the presenting symptoms were abdominal pain in 12, vomiting in 5 and elevated temperature in 6. Preoperative ultrasound was performed in 5 patients, 3 neonates and 2 older children. Sixteen of the 17 girls were operated on. The indication for surgery was an
ovarian cyst
with complication in the infants and in the older children the suspicion of acute appendicitis. The operative procedure was cyst uncapping in 7 cases, salpingo-oophorectomy in 4, ovarian resection in 2, ovarian fixation only in 1 and no ovarian intervention in 2. Asymptomatic infants with an

less than 4 cm in diameter can be managed conservatively. Surgery can be recommended after documented change of the cyst on ultrasound, large cysts giving rise to symptoms or presentation of an

This article describes a case in which tubal torsion occurred as a complication of bipolar electrocoagulation via a laparoscope. The patient, a 37-year old woman who had undergone bipolar electroagulation of her fallopian tubes via a laparoscope 11 months earlier, presented with an
acute abdomen
. A preoperative diagnosis of torsion of an
ovarian cyst
was made. The right adnexa, including the entire fallopian tube and ovary, was infarcted and presented as a 10 x 7 x 4 cm mass that had undergone a torsion of 540 degrees. A right salpingo-oophorectomy and a left salpingectomy were performed, and microscopic examination confirmed the gross finding of infarction of the right adnexa and a left hydrosalpinx. Altogether, 14 case of tubal torsion following tubal sterilization have been reported since 1956. Of these, this is the 6th case associated with electrocoagulation. The interval between the sterilization procedure and presentation with torsion has ranged from 5 months to 8 years. Many patients present at the middle of their menstrual cycle. There is generally a history of acute onset of abdominal pain which may be generalized or localized to the lower abdomen or either iliac fossa (most commonly the right). The pain is usually not associated with gastroin estinal symptoms. Unlike acute salpin gitis, there is no fever or leucocytosis in early cases. There are signs of peritoneal irritation in the involved iliac fossa and an adnexal mass can usually be palpated on vaginal examination. Abnormal mobility and an abnormal bulk of tube appear to be required before the fallopian tube will undergo torsion. Increased mobility is caused by severing or at least constricting the attachment of the tube to the uterus. The increased bulk of the tube is produced when secretions from the tube can no longer travel into the uterus, resulting in a hydrosalpinx.

In a prospective study undertaken at Tygerberg Hospital, Parowvallei, South Africa, six ultrasound criteria were drawn up for the safe, conservative management of these neonates.
