Intraabdominal lymphangiomas are rare. Most cases are asymptomatic, except when complicated. This is a case report of a 8-year-old boy with hemorrhage into a mesenteric lymphangioma following trauma, as a cause of acute abdomen. Prompt diagnosis and operation were imperative in the proper management of this child.

The ultrasonic findings of a large mesenteric lymphangioma in a man of 62 years of age are reported. Haemorrhage in some of the cystic spaces of the lymphangioma had caused an acute abdomen.

Cystic lesions of the mesentery, omentum, and retroperitoneum are rare; from 1956 to 1990, 22 patients had operative treatment for such lesions at our institution. They ranged in age from 1 month to 14 years; 75% were younger than 5 years. All had either an acute abdomen or, more commonly, a silent abdominal mass. In all cases, the histologic diagnosis was lymphangioma. Abdominal ultrasonography was done in all cases after 1977 except for two patients who had an acute abdomen requiring emergency exploration. A cystic abdominal mass was diagnosed in 94% of these cases, but the correct diagnosis of lymphangioma was made prior to surgery in only 24%. Mesenteric cysts are most common in the small bowel mesentery. Omental cysts usually occur singly and are easily resected, but multiple cysts predominate in the mesentery and retroperitoneum. Complete resection was accomplished in 82% of our cases. Two patients required partial bowel resection, and four had partial excision with marsupialization of the cysts. With a mean follow-up of 23 months, we have had three recurrences, but none necessitated reexploration. Extra-abdominal lesions, mainly cutaneous lymphangiomas, developed in two cases. Prognosis is good after surgical excision, but long-term follow-up is advisable because of the possibility of recurrence, even during adulthood.

We describe a patient, 54-year-old woman, with lymphangiomas of the jejunum and mesentery presenting as acute abdomen. She had sudden onset of severe abdominal pain with guarding and fever. Physical examination revealed an elastic hard mass, with marked tenderness in the right abdominal region. Ultrasonography and computed tomography revealed a heterogenous mass, 20 x 15 x 8cm in size, in the mesentery. An emergency operation was performed with the diagnosis of diffuse peritonitis due to an inflammatory tumor in the mesentery. At operation, the tumor was found in the mesentery, adhering to a 60-cm segment of the jejunum. Multiple small tumors were seen in the submucosal layer of jejunum. The tumor was excised with a 60cm length of the jejunum. Histologically, the tumors were diagnosed as cavernous lymphangiomas. Although lymphangioma in the abdomen is rare in adults, we should consider it as one of the causes of acute abdomen.

Lymphangiomas are benign tumors, their growth are slow, customarily without symptoms, their location more frequent are in head or neck and they're diagnosed before the two first life years frequently. Their etiology continues uncertain, being accepted the congenital origin. We report on three cases of abdominal lymphangioma beginning as an acute abdomen. The importance of these cases is based mainly in the low frequency of presentation of these tumors in this location (2-5% of the lymphangiomas) and in their initial symptomatology.

Lymphangioma is an uncommon tumor. Lymphangiomatosis, a benign tumor consisting of a cluster of dilated lymphatic channels, is very unusual. Most lymphangiomatoses are found in the neck and head area. Less than 5% are diagnosed intraabdominally and they are very infrequently encountered in the retroperitoneal area. Herein, we report a rare case of a 32 year-old woman who had disseminated intra-abdominal and retroperitoneal cystic lymphangiomatosis, which presented as acute abdomen. She received exploratory laparotomy due to the suspicion of malignancy, which was finally confirmed as cystic lymphangiomatosis. The clinical manifestations, imaging features, and management of this patient are discussed and compared with previous literature.

Cystic lymphangioma is a rare benign tumor that occurs more often in children with different localizations. Abdominal localization and its development may produce symptoms and clinical features of acute abdomen leading to problems in the differential diagnosis. The Authors report a rare case of pancreatic lymphangioma in a nine years old child resolved by surgical treatment. A review of the literature concerning problems of diagnosis and treatment is also presented.

A 6-year-old female was sent to our ER due to nausea, vomiting and abdominal distension for 2 days. This child had a history of constipation and failed intermittent medical treatment for 2 years. Her plain abdominal X-ray showed multiple intestinal loops and under the impression of acute abdomen with mechanical intestinal obstruction, an exploratory laparotomy was performed. A huge mesenteric tumor was discovered to be the cause of the intestinal obstruction; the involved bowel and the mesenteric lymphangioma were resected and primary anastomosis was done. Mesenteric cystic lymphangioma is a rare cause of bowel obstruction; preoperative diagnosis is difficult due to silent clinical course and lack of awareness of the clinical and morphological features of this disease. The case is presented along with a review of literature with the conclusion that a high index of suspicion is recommended. An abdominal ultrasonography may be recommended to evaluate a long-term constipated child to ascertain that any cystic lesion will not be missed.

The case of a 12-year-old boy with hemorrhage into a previously unknown retroperitoneal lesion following blunt abdominal trauma is reported. Diagnostic work-up of a post-traumatic surgical acute abdomen revealed a giant multicystic tumor in the retroperitoneum, which could be completely removed. Histological examination confirmed the suspected diagnosis of congenital lymphangioma. Attention should be drawn to the possible coincidence of trauma and a preexisting asymptomatic lesion. Knowledge of the characteristic ultrasonographic and computer tomographic features is essential in order to make a correct diagnosis.

Five cases of cystic lymphangiomas are described. There were three children and two adult patients. Age ranged from 4 to 38 years. All the patients presented with pain abdomen. On examination, four out of five patients were found to have an ill defined lump abdomen. One patient had no abdominal symptoms. Abdominal ultrasonography done in all the cases revealed the cystic nature of the tumour thereby making the pre operative diagnosis easier. A histological diagnosis of cystic mesenteric lymphangioma was made in each case. Although mesenteric lymphangiomas are rare, they should be considered as a possible cause of acute abdomen, both in children and adults.
