The use of the nitroblue tetrozolium (NRT) dye reduction test as a screening method for bacterial and fungal infections is now widely recognized. Levels have been reported to be falsely elevated in the pregnant state. This report includes a prospective study which showed that the results of this test in 124 pregnant women were indistinguishable from those of a normal control population. Seven of 8 elevated levels were attributable to infection or allergy. With this data, it is concluded that the NBT test can be used to screen for bacterial and fungal illnesses in a febrile pregnant patient as well as in the general population.

The criteria for false positivity of the NBT test were described including absence of classical clinical signs of bacterial infection, negative blood (and, if necessary, other) cultures, and lack of response of antibacterial treatment as the basis for appreciation of positive NBT test result as false-positive. A case of acute lymphoblastic leukaemia with all the criteria being fulfilled was described.

Judging from the spontaneous NBT reduction, the indices of phagocytosis and NBT, there is a moderate, but statistically significant diminution of these parameters in leukaemia and malignant lymphoma including plasmocytoma. Moreover, further diminutions could be identified during the acute stage of the disease (first diagnosis or recidive) in acute leukaemia and lymphogranulomatosis, but not for chronic myeloic leukaemia.

Experiments were performed aiming at establishing optimal conditions for demonstrating the activity of dehydrogenases in Epon-embedded, semithin sections. Small fragments of the nervous tissue were incubated 1, 4, 10 and 24 h 37, 20 and 4 degrees C in the presence of NBT or TC NBT. The incubated fragments were embedded in Epon and semithin sections prepared thereof. The activities of succinate dehydrogenase and of alpha-glycerol dehydrogenase was evaluated. The experiments have shown that semithin, Epon-embedded sections may be used for demonstration and cytological localization of respiratory enzymes. Various experimental conditions were tried. Optimal results obtained when unfixed tissue fragments were incubated for 24 h at 4 degrees C.

15 ditetrazolium salts were examined to prove their qualities for histochemical techniques. The succinate dehydrogenase, the lactate dehydrogenase and the diaphorase I in hearts, muscles, livers, kidneys and brains of rats were demonstrated for it. The results show that NBT is the best allround tetrazolium salt for the histochemical demonstration of dehydrogenases. For the study of special questions it is suitable to use other tetrazolium salts, too.

The nitro-blue tetrazolium dye reduction test according to modified Park's et al. method in asthmatic children with pollen allergy during remission and status asthmaticus was performed. In patients with status asthmaticus high rate of spontaneous NBT reduction by peripheral phagocytes was found. The routine investigations allowed to exclude bacterial infection as a cause of asthma attacks. It is suggested that allergic reaction may change cellular metabolism providing to increase of NBT reduction.

Polymorphonuclear leukocytes of 30 patients suffering from Trisomy 21 between the ages of 3 months and 21 years were examined by means of a modified NBT-test. Ingestion capacity as well as NBT reduction capacity of polymorphonuclear leukocytes showed a decrease in dependence on the age of the patients examined. It is supposed that both disorders are involved in endangering patients with Trisomy 21 by infections.

In 23 subjects of different ages with Down's syndrome a number of parameters of non-specific defense of humoral and cellular immunity were investigated. While in all age groups complement factors C3, C4 and C5 as well as phagocytosis and NBT indices were in the normal range, a dysgammaglobulinaemia increasing with age with a hyperglobulinaemia of the IgG, IgA and IgD types was found, sparing immunoglobulins IgM and IgE. In addition the transformation capacity of peripheral blood lymphocytes decreased with age. This is understood as the consequence of premature aging of the thymus-dependent immune system.

Fifty-six healthy neonates and children as well as 33 patients with juvenile rheumatoid arthritis and other diseases of the locomotor system were examined using a modification of the NBT test. In healthy newborn infants there was observed an increase in the number of NBT-positive cells between the first and tenth days. The test results obtained for the 33 patients with juvenile rheumatoid arthritis were positive in 73%, with positive results being obtained especially in those patients where the disease was in its second stage of activity. Highest values were obtained in patients with a predominantly articular manifestation of the disease. Stimulative effects produced by immune complexes are considered to be chiefly responsible for this. However, this method of examination is of small practical value from a differential diagnosis point of view.

Chemotaxis, random migration, phagocytosis, spontaneous and stimulated NBT reduction were evaluated in neutrophils from the peripheral blood of 8 patients with acne conglobata and were found to be normal. Only 2 patients had an increased spontaneous NBT reduction, and another exhibited a defect in neutrophil adhesiveness. On this basis, intrinsic defects of the major neutrophil functions, as evaluated in vitro, can be excluded in acne conglobata.
