The effects of prolonged occlusion on the normal microbial skin flora, pH, transepidermal water loss (TEWL) and carbon dioxide emission rate (CDER) were studied. The total average counts before occlusion were 1.8 X 10(2)/cm(2) and increased to 4.5 X 10(6) on day 5. The highest counts were noted on day 4(9.8 X 10(7)/cm(2)). The composition changed: controls comprised of 63% coagulase negative staphylococci, 6% micrococci, 17% diphtheroids and 6% bacilli. After 5 days of occlusion, the percent compositon was: 63% coagulase negative staphylococci, 11% micrococci, 4% diphtheroids, 19% lipophilic diphtheroids and less than 0.003% gram negative rods. The pH of the skin before occlusion was 4.38 and increased to 7.05 on day 5. After 5 days of occlusion TEWL increased from 0.56 mg/cm2/hr to 1.87 mg/cm2/hr and CO2 emmission increased from 25 nl/cm2/min to 118 nl/cm2/min.

A clinical trial with a "continuous" CO2 laser in the treatment of cutaneous lesions was instituted 2 1/2 years ago. The laser was used as a "light" knife for excision and for gross destruction of some lesions directly. Details of technique and a preliminary report assessing clinical results and general effects are given. Some results thus far have proved satisfactory, and we feel that further investigation of the laser as a surgical modality is warranted.

Thermal effects were achieved from skin biopsies (epidermis, corium, subcutis) measured by batch-microcalorimetry, after exposure to potent inflammatory or antiinflammatory substances (compound 48/80, histamine, serotonine and hydrocortisone phosphate). Exothermic effects were achieved from 48-80, histamine and hydrocortisone phosphate, when exposed to the skin and lasting for varying time. From serotonine initial endothermic signals were achieved but after about 50 min there was stabilizing of the thermal reaction at the baseline. To analyze the nature of these non-specific thermal effects, determinations of O2 consumption and CO2 production were performed, using Warburg technique on exactly matched tissue pieces exposed to the same substances and run parallely with the microcalorimetrical determinations. An increased respiration, probably as an indicator of increased metabolism was registrated for all test substances acting on the skin. It felt, that the thermal effects reflect an increased metabolism of the tissue exposed to the test substances.

This is a retrospective study of the cure rates of forty patients with subungual and periungual viral warts treated with carbon dioxide laser vapourisation (total of 69 lesions). 70.6% (48/68) lesions had failed treatment with cryotherapy and/or electrocautery treatment previously. 20/68 were treated with CO2 laser vapourisation as a first line treatment. The overall cure rate over 10 months follow-up period was 57.4% (39/68). Most recurrences (24/25) occurred within the first 3 months of vapourisation. The carbon dioxide laser vapourisation cure rate for warts in which previous cryotherapy and/or electrocautery had failed was 47.9% whereas those treated with carbon dioxide laser vapourisation as a first line treatment had a cure rate of 80% (p = 0.043). Subungual warts responded slightly better with a cure rate of 64.7% compared with periungual warts (54.9%) (n.s.). The carbon dioxide laser vapourisation cure rate for recurrent subungual and periungual warts (which failed previous carbon dioxide laser vapourisation) was 73.3% (11/15). Our findings appeared to indicate that periungual and subungual warts can be eradicated by CO2 laser vapourisation. Recurrent warts can be effectively eradicated by further vapourisation. Recalcitrant periungual and subungual warts which have previously failed to respond to cryotherapy and/or electrocautery can be effectively eradicated with CO2 laser vapourisation.

Whole human hair follicles derived from the scalp, chin, axilla, and pubis were organ cultured. The hair follicles were dissected from excised normal skin pieces and placed in 1.0 ml of incubation medium in a closed 5 ml glass tube under an atmosphere of air + 5% CO2. The tube was rolled at 15 rpm and 36 degrees C. The hair follicles from each area of the body grew linearly for about 6 days, both in serum-plus and serum-free media with or without fibroblast growth factor (FGF). The growth rates were slightly slower in serum-free media than in serum-plus media. No significant differences in the growth rates of hair follicles in organ culture were seen in different areas of the body. FGF did not affect the growth rate or the culture term of the hair follicles in vitro. This is the first report of human hair follicles derived from chin, axilla, and pubis growing well for about a week in organ culture.

Experimental results revealed that although [3H]thymidine uptake in the hair bulb increased time dependently for 12 days under normal culture conditions (95% air-5% CO2 at 37 degrees C), striking morphological changes occurred in the hair bulb cells as demonstrated by histological findings. As such, organ culture conditions applicable to human hair follicles were studied utilizing observations from both histology and DNA synthesis. We found that culture conditions of 95% O2-5% CO2 at 31 degrees C were superior when compared to normal culture conditions for cultures of human hair follicles when attempting to maintain the normal morphology of hair germinative cells. The hair bulb and the germinative cells successfully maintained their normal morphology throughout the 96 and 48 h culture period, respectively. Autoradiographs of [3H]thymidine-labeled follicles showed localization in the germinative cells below Auber's critical line. Hair bulb DNA synthesis increased time dependently for 96 h after culture initiation. Under conditions of 95% O2-5% CO2 at 31 degrees C, the synthesis of DNA in hair germinative cells was observed. Such an organ culture method may prove useful for studies on the human hair growth mechanism.

Normal skin and uninvolved and involved psoriatic skin specimens were maintained in vitro in organ culture. The 3-4 mm punch-biopsied skin specimens were put freely into the culture medium with or without fetal calf serum, under an atmosphere of 95% O2 plus 5% CO2, and rotated at 60 rpm at 37 degrees C. In the serum-free culture medium (vitamin A-free) granular layers appeared in the involved psoriatic epidermis in culture. Addition of TGF-alpha caused normal skin and uninvolved and involved psoriatic skin specimens to become acanthotic and to degenerate easily almost to the full thickness of the epidermal layer in proportion to increasing concentrations of TGF-alpha as well as with the duration of the culture, but without disappearance of their granular layers. TGF-beta caused the normal skin and uninvolved psoriatic skin specimens to become thinned without disappearance of granular layers, but caused the involved psoriatic skin specimens to be thinned without appearance of granular layers in serum-containing medium or with their disappearance in the serum-free medium. TGF-beta also antagonized the acanthotic and degenerative effect of TGF-alpha. The results suggest that TGF-alpha and TGF-beta may partially be related to the induction of psoriatic epidermal lesions.

Chemical peeling promotes formation of new epidermis and new dermal collagen, resulting in skin shrinkage, reduction of wrinkling and crepe paper skin, softening of crow's feet, and, when desired, lightened eyelid color. Chemical peeling may be performed as the only eyelid procedure, simultaneously with CO2 laser surgical blepharoplasty, after healing of cold-steel-scalpel or CO2-laser blepharoplasty, and as a repeated procedure to achieve maximal results.

To determine the viability of the immunodeficiency virus in the laser plume after carbon dioxide (CO2) laser irradiation, multiple samples of culture medium containing concentrated simian immunodeficiency virus (SIVMAC at 1000 TCID50 cultured from HUT 78 cells) were irradiated with a continuous-wave CO2 laser at variable irradiances (from 400 W/cm2 for 5 seconds to 1600 W/cm2 for 300 seconds). The resultant plume was collected and cultured for the presence of SIV. A positive control consisted of handling an infected specimen identically to the test specimens, with the exception of CO2 laser irradiation. All test cultures remained negative over an 8 week incubation period, while the control was positive for viable SIV within 7 days. These results suggest that SIV is not viable in the laser plume after CO2 laser irradiation. Further investigation is necessary before extrapolating these results to the human immunodeficiency virus (HIV).
