Serum levels of amyloid protein A (SAA) have been shown to be elevated in different types of amyloidosis and in rheumatic diseases by radioimmunoassay using 125 iodine labeled AA and anti-AA. SAA levels were elevated in both primary and secondary amyloidosis, but there were highly significant differences between these levels. In heredofamilial amyloid, SAA levels were within normal limits. While the mean SAA level was elevated in persons over 70 years, the fact that some persons in this age group had normal levels suggested that marked elevation after age 70 may be due to occult inflammatory or neoplastic disease. High SAA levels in patients with rheumatoid arthritis correlated, in most cases, with physician evaluation of disease activity and Westergren ESR. SAA levels in patients with systemic lupus erythematosus were lower than those in patients with rheumatoid arthritis, and most patients with degenerative joint disease had normal levels. Very high levels of SAA were found in patients with neoplastic diseases. Patients with carcinoma of the lung and bowel had much higher levels than patients with carcinoma of the breast. Determination of SAA levels may be of value in evaluating different forms of systemic amyloidosis, assessing the activity of rheumatic disease, and screening for occult inflammatory or neoplastic disease.

Data from a prospective study of the clinical course in 223 patients with systemic lupus erythematosus followed for 655 patient-years were analyzed by computer to determine the influence on frequency of infection of 1) corticosteroid dose; 2) azathioprine; 3) active disease, measured by new disease exacerbations, elevated ESR, hypocomplementemia, active urinary sediment, and proteinuria; 4) uremia; and 5) leukopenia. The frequency of all infections, and of bacterial and opportunistic infections specifically, increased progressively with increasing steroid dose. Azathioprine use, independent of steroid dose, did not account for an increased risk of bacterial, opportunistic, or nonspecific viral infections. Leukopenia did not predispose to infection, except possibly when associated with azathioprine-induced bone marrow suppression. Active renal disease, especially when manifested by abnormal urine sediment, was associated with an increase in infection frequency.

The CREST syndrome refers to a disorder comprising the manifestations of calcinosis, Raynaud's phenomenon, esophageal dysfunction, sclerodactyly, and telangiectasia. Thirteen CREST patients (two with CRST) were compared with 26 patients with systemic sclerosis but without the full manifestations of the CRST syndrome. No significant difference was found between the groups in the age of onset of Raynaud's phenomenon, degree of multiphasic digital color changes, ulcerations of fingers, sclerodactyly, or in the frequency of abnormal esophageal peristalsis or dysphagia. Laboratory results were similar, including the frequency of an elevated ESR. However, the CREST patients had a significantly lower frequency of arthralgia (54%) and arthritis (15%) than did those with scleroderma (88% and 65%, respectively). All but one of the CREST patients were women, which was a greater proportion than found among scleroderma cases (69%), and all were white (P less than .05). Most patients with the CREST syndrome had rather severe acrosclerosis. At last evaluation, four patients were chronically ill and three had died. The CREST and CRST syndromes are closely related disorders that seem to be part of the spectrum of systemic sclerosis.

Clinical picture and differential diagnosis of Yersinia arthritis are shown by means of three own observations. It is an acute oligoarthritis affecting especially knee- and ankle-joints. The involved joints are very painful, swollen and warm. There may be a history of enteritis or suspicion of acute appendicitis because of lower abdominal pain, but this is not obligatory. The laboratory parameters of inflammation (ESR, C-reactive protein, white blood count, serumproteinelectrophoresis) are changed significantly. Diagnosis is made by serum agglutination reaction (Widal-reaction) against ceesurface antigens (O-antigens) of Yersinia enterocolitica. Almost only people with the HL-A antigen B27 tend to get arthritis during Yersinia infection. The differential diagnosis has to consider reactive arthritis during Salmonella or Shigella infections, acute sarcoidosis, Reiter's disease and rheumatoid arthritis.

Seven cases with the syndrome of thalassemia minor and pauciarticular, non-erosive, seronegative arthritis are described. Other known causes of seronegative arthritis had been excluded. There was a predilection for middlesized joints such as ankles, wrists and elbows. Usually the synovitis was asymmetrical from onset, with one to eight joints affected. Finger or toe joint were usually spared. Extraarticular synovitis such as tenosynovitis or bursitis were not observed, nor were nodules, signs of vasculitis or visceral involvement. The course of this arthritis showed chronicity and mild, persistent, non-erosive synovitis without joint effusions. X-ray revealed juxtaarticular osteoporosis of the affected joints, characterized by a diminution of the number of trabeculae (hypertrophic atrophy) combined with broadening of the singular trabeculae; this picture is typical of hemoglobinopathies. - Computed tomography showed a probable slight deficit of bone mineralization. Laboratory investigations including ESR, routine immunological tests, blood chemistry, and HLA-tissue typing were all normal. - The combination of this peculiar arthropathy with thalassemia minor would appear to be worthy of note and requires a further search among the forms of arthritis of unknown origin.

An epidemiological social-medical study was carried out in the section of IX polyclinic--Sofia, aiming at the establishment of the social risk factors effect upon the origination and course of the rheumatoid arthritis (RA), in view of the particularly social importance of the disease invalidity. The model is based on statistical methods for programmed computor processing. Two hundred and forty six subjects were examined, the basic group of 44 being with diagnosed RA according to the international criteria (0.35% from the population) and 2 control groups--patients with infectional alergic arthritis and healthy subjects. The assessment of the RA severity degree was performed according to the numeric method, elaborated for that purpose, based on the following criteria: number of the joints, kind of the joint involvement, functional capacity, patients' personal assessment of the difficulties due to the disease, disease duration, ESR, rheumatoid factor, X-ray changes according to the adopted four degrees. The following social factors were studied: occupation, kind of work, working conditions, financial status, education, home conditions, mode of life, activity in the leisure time. A statistically significant (correlation C with P less than 0,05--0,001) unfavourable effect of the following complex of risk social factors was established: 1. RA risk factors--unfavourable microclimatic working (C=0,24) and home (C=0.30) conditions: 2. Risk factors for RA invalidity--occupations with heavy physical work (C=0,40), wol income (C=0,31) and unfavourable home conditions (C=0,22). The results obtained will serve as a ground for concrete directed rheumatoid arthritis prophylaxis. That outlines the perspectives of similar studies for assessment of the effect of life environmental factors upon human health state.

The early course of newly diagnosed RA among young adult patients (16-44 yr) is described from results of an ongoing study with a mean follow-up of 3.4 yr. Study diagnosis was based on the judgement of experienced rheumatologists, and data on several hundred variables were obtained on entry and annually for the purpose of defining patterns of onset and course of disease. Race and sex factors, as well as certain entry manifestations, e.g., RF, were found to correlate with onset and course patterns. Females, and especially white females, had significantly greater numbers of swollen upper extremity joints than males at entry and at last observation, with increased likelihood of developing bone erosions. At entry, RF positive patients differed only on few articular manifestations from RF negative patients, but had a higher frequency of positive ANA at entry and more subcutaneous nodules and bone erosions during follow-up. Seropositive white females at entry had significantly more swollen upper joints than their seronegative counterparts, but with no difference found at last follow-up. White females of each serogroup had more joint involvement at last examination than patients of other race-sex groups. Males had more acute onset, especially under age 30, with significantly greater improvement in arthritis and in ESR than did females. The majority (55%) of patients entered as seropositive, converted to seronegative during follow-up, and no correlation of either joint swelling or erosions was noted with this phenomenon. At last visit, RF positively did not correlate with bone erosions, but patients developing bone erosions had higher frequency of ANA and higher mean serum complement levels at last examination. The following entry factors were found to correlate significantly with a better outcome: maleness, acute onset under age 30, less swollen upper joints, and negative RF. Type of drug therapy tended to reflect severity of arthritis, rather than vice versa, and functional capacity improved significantly from entry to last evaluation in both males and females, even though the latter had stable or progressive arthritis. Further study is necessary over long intervals and in wider age range to more adequately interpret the biologic implications of findings of this ongoing study. A better understanding of the pathogenesis of RA may be derived from critical studies of the contribution of host factors, e.g., sex, and other variables predisposing to the development of RF positivity (and ANA), subcutaneous nodules, and bone erosions, particularly in systematic ongoing studies of patients with early diagnosed disease.

Immune arthritis in sensitized rabbits was induced by intraarticular injection of bovine serum albumin. The development of the arthritis was accompanied by an increase in ESR, a rise of the level of serum CRP, caeruloplasmin and CIC. A chemiluminescent response of the whole blood phagocytes to stimulation by barium sulfate crystals, serum beta-glucuronidase and red cell superoxide dismutase activity enhanced, plasma malone dialdehyde content rose, serum SH groups diminished.

Interstitial pneumonia is well known as one of the complications of rheumatoid arthritis (RA). While interleukin-2 (IL-2) regulates the immune response through IL-2 receptor (IL-2R), the exact role of the soluble form of IL-2R (sIL-2R), recognized as a part of the alpha chain or IL-2R, is still obscure. So, the immunological significance of sIL-2R in serum and in bronchoalveolar lavage fluid (BALF) of those of RA patients with or without interstitial pneumonia was studied. The sIL-2R was measured with an ELISA kit (T-Cell Science Ltd). The levels of sIL-2R in the sera of RA patients without interstitial pneumonia were significantly higher than those of normal controls. Furthermore, the levels of sIL-2R showed a statistically significant correlation with ESR and Lansbary's index. The levels of sIL-2R of RA patients with interstitial pneumonia were higher than in those without interstitial pneumonia although the evaluation of class and stage of arthritis in those RA patients with or without interstitial pneumonia revealed no significant difference. A high sIL-2R/albumin ratio in BALF of RA patients with interstitial pneumonia was shown in comparison with those of normal control. These data indicate that the estimation of sIL-2R in RA patients could be useful in estimating the disease activity and that high levels of sIL-2R reflect the active immune response in the lungs of RA patients with interstitial pneumonia.

Joint involvement in giardiasis is reported in children and young persons only with one exception. We present two cases of adult patients (a thirty-year-old woman, 26-year-old man) who developed a slowly aggravating arthritis in PIP-joints, ankles and knees with a tendency to chronification or relapse. Giardiasis was diagnosed by examination of feces. Treatment with ornidazole was effective in arthritic and gastrointestinaly symptoms. The patients had neither ESR elevation nor any other signs of inflammatory processes. The arthritis factor was negative. HLA-typing showed no phenotypes associated with arthritides.
