In the last years pentoxifylline (PTXF) has been successfully used in reducing acute injury of the lung parenchyma in ARDS. The authors have studied the effect of PTXF on degree of papain induced lung injury. Papain was administered intratracheally in a dose of 2 mg, 4 mg, 6 mg in 1 ml of PBS. The effect of papain on bronchoalveolar lavage characteristics was also evaluated. Wistar rats of both sexes were used in the experiment. The degree of lung destruction, reflected by interstitial hemorrhage was assessed by measuring hemoglobin content in the fluid of the lavaged lungs. The hemoglobin levels were assessed spectrofluorometrically with the use of the 414 nm wave length. A reduction of hemoglobin content was seen after PTXF administration only in animals receiving the lowest dosage of papain. However in all animals a decrease in the BAL neutrophil count was demonstrated. The protective effect of PTXF on pulmonary tissue in papain induced injury models and situations leading to development of ARDS may suggest a similar pathomechanism in both entities.

The authors discuss the etiology, pathomechanisms, radiological features of lipid pneumonia. The role of bronchoalveolar lavage is stressed in determining this diagnosis. A case is presented of a patient receiving paraffin oil for chronic constipation. The diagnosis was made after identifying the lipid droplets (Sudan III stain) in alveolar macrophages sampled by BAL.

BAL, spirometric evaluation and compliance values were analysed in 51 patients (21 females, 30 males; age range 19-51 years). Basing on BAL criteria, 19 had active sarcoidosis, 32 non-active sarcoidosis. Radiological stages (I, II, III) were found in similar percentages in both forms of the disease. Mean respiratory function parameters were similar in the active and non-active stage of the disease. The most common abnormality found in 33% of the patients was decreased DLCO.

BAL and respiratory function were analysed in 26 patients with sarcoidosis (age range 22-48 years) before and after a 12 month steroid therapy trial. 11 patients were in the active stage according to the BAL criteria. After treatment a significant decrease in BAL lymphocytosis was found (a decrease from 31.4% to 22.8%). A significant increase in TLC, DLCOSB and D/VA mean values was found. The effect of steroid therapy was similar in the active and non-active stage of the disease. The authors found BAL useful in monitoring of therapy.

The aim of the study was to analyse the clinical effectiveness of ofloxacin in treatment of post-infectious exacerbations of endogenous bronchial asthma. Twenty eight patients (20 males, 8 females) were assessed. The following parameters were evaluated: bacteriological examination of the sputum (before and after treatment), morphological analysis of peripheral blood, bronchoalveolar lavage (performed in 11 patients). Additionally in 7 patients lymphocyte subpopulations of the peripheral blood were determined (OKT4/OKT8). Diurnal variations of serum levels of theophylline were assessed in 12 patients. The authors observed a clinical improvement in 26 patients. Following treatment the white cell blood count (WBC) and the CD4/CD8 ratio decreased. The same observations were made analysing the BAL differential count, in which a marked decrease of neutrophils was seen. The authors conclude that ofloxacin is a safe and efficient antimicrobial agent.

In 140 lung cancer patients fiberoptic bronchoscopy with biopsy and BAL procedure were performed in diagnostic process. Obtained results of macroscopic changes were analyzed in relation to histological diagnosis and localization in the bronchial tree (central and peripheral). Squamous cell lung cancer was the most often diagnosed in central localized tumors and adenocarcinoma in peripheral ones. Macroscopic changes were more evident and more characteristic for cancer process in central localized tumors. In peripheral changes non-characteristic, inflammatory-like symptoms were determined.

The study material was made up of 20 patients with stage I pulmonary sarcoidosis in different state of activity. BAL was carried out through a bronchofiberscope using four aliquots 50 ml each, immediately aspirated after introduction into siliconized vials. BAL induced side effects were not seen. The returned fluid was centrifuged and the cell differential count was carried out. 11 patients (55%) demonstrated a high lymphocytic count. The degree of lymphocytosis did not correlate with abnormalities in respiratory function. The degree of lymphocytosis did slightly correlate with disease activity. Thus serial BAL studies might be useful in therapy monitoring. The high lymphocytosis should be carefully interpreted and can not be the only indication for instituting steroid therapy.

The study was set up to evaluate the changes in broncho-alveolar lavage cell-count in asthmatics after bronchial provocation. Similarities and differences between atopic (A) and nonatopic (N) asthma were given a special concern. Twenty mild asthmatics--10 A and 10 N. There was no difference in BAL eosinophila between these groups before provocation. After provocation more eosinophils were found in A group (p = 0.06). Neutrophils were more numerous in N on both occasions--0 = 0.00003 and p = 0.006 respectively). Lymphocytes showed a similar pattern--p = 0.03, p = 0.003. A positive correlation between BAL eosinophilia and bronchial hyperreactivity was shown in both groups. Inflammation, including inflammatory cells plays a major role in bronchial asthma pathophysiology. It seems to be more expressed in nonatopic patients.

In 14 patients with sarcoidosis and in 9 healthy controls the bradykinin level was estimated in broncho-alveolar lavage fluid. The statistically significantly higher bradykinin concentrations was found in sarcoid patients than in controls. No relationship was found between level of bradykinin in BAL and lymphocytes % and no relationship was found between bradykinin level and pulmonary function parameters.

In 29 patients with chronic allergic alveolitis broncho-alveolar lavage BAL and lung function tests were performed before and after one year of Prednisone treatment. We observed significantly (p < 0.001) decrease of BAL lymphocytes from 52% to 32% and significantly (p < 0.001) increase of BAL macrophages from 33% to 54%. We found negative correlation between percentage of BAL neutrophils before treatment and annual changes of DLCO (r = 0.37 p < 0.05). Annual changes of lymphocytes negatively correlated with annual changes of DLCO (r = 0.45 p < 0.02) and D/VA (r = 0.46 p < 0.02).
