Excretion of chromium-51 was studied in rats poisoned by intramuscular injection of chromium chloride. CaNa2EDTA, BAL, Thioctacid, D-penicillamine and diphenylcarbazide used as detoxicants failed to increase excretion of chromium in the urine or stools.

Cell morphology of BAL- fluid and index bronchitis (by Thompson) were evaluated in 52 patients with acute and chronic bronchitis. A significant increase of index bronchitis and percentage of neutrophils in BAL were observed in acute and chronic bronchitis. There was no correlation between index bronchitis and cell in BAL- fluid.

The lungs have an important role in the synthesis of angiotensin I converting enzyme (ACE). In BAL fluid and serum the ACE activity was determined in 18 patients with sarcoidosis (11 with high intensity and 7 with low intensity alveolitis), 14 patients with lung cancer and 16 with acute bronchitis. The activity of ACE was examined by a reagent set produced by Boehringer Mannheim Biochemica Test-Combination ACE cat. no. 789/011. The ACE activity in the high intensity alveolitis group of sarcoidosis patients was significantly increased in BAL fluid and serum in comparison to other observed patients. On the other hand, in patients with lung cancer the ACE activity was also increased in comparison to acute bronchitis and referred norms, especially in BAL fluid. This findings suggest a role of neoplastic process in ACE secretion in the airways. Very low correlation observed between ACE activity in serum and BAL fluid indicates a separate mechanism of secretion.

The aim of the study was to compare the results of routine counts of percentage of BAL macrophages, using morphological differential staining method--Pappenheim stain (M) and staining for nonspecific esterase activity (E) with those obtained by APAAP technique, using specific for monocytes (macrophages monoclonal antibodies (Ki-M1) and the complex APAAP (alkaline phosphatase--mouse anti alkaline phosphatase complex). The mean counts were comparable, although different criteria of estimation were used. Significant differences were seen in smokers when the criteria were based on morphology and expression of surface antigens. The results indicate that correct estimation of macrophage counts and their characteristics (enzymatic activity, expression of surface antigens) can only be made using many methods based on various criteria of estimation.

The results of 229 routine BAL examinations in patients with different interstitial lung diseases are presented. The authors analyse the diagnostic usefulness of BAL in the differential diagnosis in interstitial lung diseases. It seems that BAL is helpful in making a specific diagnosis of interstitial lung disease.

51 BALFs from HIV positive patients were analysed. In 16 of them Pneumocystis carinii organisms were found. Two methods detecting Pneumocystis carinii were used: Grocott-Gomori, fluorescence Papanicolaou stain. The best staining method turned out to be Grocott-Gomori. The differential count was carried out in 40 BALFs. In patients with Pneumocystis carinii detected by BAL neutrophilia was always present. In some of these patients lymphocytosis was also present. In 4 patients with high lymphocytosis immunocytochemical examinations with monoclonal antibodies against CD4, CD8 T cells showed a marked decrease of the CD4/CD8 ratio.

Immunocytochemical examinations using the PAP method with monoclonal antibodies against lymphocytes T (CD3, CD4, CD8) were carried out in 31 BAL's from patients with different interstitial lung diseases. A distinct increase of the CD4 (T "helper"): CD8 (T "suppressor") ratio was found in patients with sarcoidosis and in a patient with pigeon breeder's disease. A profound decrease of the CD4/CD8 ratio was found in HIV positive patients. The authors also discussed the methodological problems of correct typing of T cells in BAL.

A case of a 34 year old female with clinical and histological confirmed eosinophilic granuloma causing osteolytic lesions of the mandible and skull and diabetes insipidus. Dyspnea, cough, exercise intolerance with ventilatory disturbances and radiological evidence of interstitial lung disease were believed to be in the course of eosinophilic granuloma. The patient refused lung biopsy and BAL. After starting therapy with corticosteroids all respiratory symptoms improved.

Netilmicin - a semisynthetic aminoglycoside - was administered to 33 patients with the acute or chronic lower respiratory tract or pulmonary infections in a daily dose of 5 mg/kg body weight for 10 days. A principle criterium of patients classification to netilmicin therapy were sensitive bacterial strains either in sputum or in BAL liquid. A significant clinical improvement was noted in 88% of the treated patients. However, elimination of pathogens from the sputum was achieved only in 52% of these patients. No improvement was observed in 4% of the treated patients. No adverse reactions were noted. Netilmicin proved safe and effective antibacterial agent in patients with respiratory infections.

The aim of our work was to establish the influence of the HgCl2 poisoning (various doses) on the Fe distribution in the rat organism. 175 Wistar rats were divided into 5 groups: I--control group, II--intoxicated with HgCl2 in the dose of 1 mg/kg of body weight, in the III group the HgCl2 dose was 6 mg/kg, in the IV group--12 mg/kg, in the V--12 mg/kg but rats of this group were given afterwards BAL (2.3-dimercaptopropanol). All rats were given 0.2 ml of 59FeCl3, radioactivity 3.7 kBq. The animals were put to sleep with chloroform after 3 and 6 hours, and 1, 2, 4, 8, 14 days after the isotope administration. For the radiometric assays the following organs or tissues were taken: stomach, small and large intestine, liver, kidneys, lungs, heart, muscles, spleen, blood, brain, testicles. The results are given in the percent of Fe dose in 1 g of wet tissue. The statistical analysis (Students' t test) was performed. The results indicate that in poisoned animals the Fe distribution was different than in controls. Increased concentration of 59Fe was noted after 3 and 6 hours in stomach and after 1 day in large intestine, whereas in other organs the Fe concentration was lower, correlating usually with the Hg dose. One interesting thing was noted: the Fe concentration in almost all organs was higher in rats intoxicated with the 12 mg/kg dose of HgCl2 and given afterwards BAL (IV group), than in rats that were not given BAL. Present results can have some practical significance in therapeutic procedure in mercury poisoning and during the convalescence.
