With the purpose of determining the long and short term changes in serum enzyme activities after a marathon race, a survey involving nine healthy male runners was carried out. A basal blood sample was extracted from each 24 hours prior to the race and three further extractions were made immediately after the race, as well as at 1 and a final 24 h after the end of the race. In the enzymes of preferably hepatic origin--alkaline phosphatase (AP), ganna-glutamyltransferase (GGT) and alanine aminotransferase (ALT)--scanty modifications were found and these could be related to the changes observed in the plasma volume. Enzymes such as aspartate aminotransferase (AST) and lactate dehydrogenase (LDH), which are widely distributed in the tissues, were found to have undergone more marked variations and these could not be related to the changes in the volume of the plasma, while in enzymes of muscular origin such as aldolase (ALD), creatine kinase (CK) and its cardiac isoenzyme (CK-MB), notable increases were observed due to the muscular injury suffered. The greatest example of this was the increase found in total CK 24 h after the end of the marathon (414.6%). The high serum percentages found in CK-MB in these endurance-trained runners in relation to total CK activity should be carefully assessed in order to avoid false diagnosis of acute myocardial infarction.

Total creatine kinase measurement in serum has remained the best overall marker for detection and monitoring of skeletal muscle diseases, despite that different human tissues exhibit varying distributions of cytoplasmic and mitochondrial isoenzymes of creatine kinase. Acute myocardial infarction aside, increases in total serum creatine kinase, as reflected by the MM isoenzyme, are most commonly caused by injury or diseases to striated muscle. Enzyme markers of skeletal muscle injury that have been previously used (eg, aldolase, enolase, aspartate aminotransferase, and lactate dehydrogenase isoenzyme 5) are not as specific as creatine kinase and have limited clinical utility. However, new enzyme and protein markers are currently being investigated, eg, troponin and carbonic anhydrase III, which are more specific than creatine kinase toward particular tissues. Moreover, measurement of creatine kinase isoforms may provide information about whether muscle turnover is acute or chronic.

The aim of the present work was to investigate histological localization of newly found human muscle CA-III and its diagnostic value in neuromuscular diseases. The following results were obtained. CA-III was purified as a single band from human skeletal muscle nd specific anti-CA-III antiserum was raised in the rabbits. By the direct immunoperoxidase method in human biceps muscle, CA-III was localized mainly in Type I fibers (red muscle type). A radioimmunoassay was developed for CA-III which can detect 5 ng/ml of sample. Among several human tissues, CA-III was found virtually specific to the skeletal muscles with a level of 5 mg/gm wet tissue. Normal serum CA-III level (n = 20) was 22.5 +/- 15.3 (SD) ng/ml. Among 140 cases of various diseases, elevated serum CA-III levels were found in 29 cases, all of which were only from neuromuscular diseases including 17 various muscular dystrophies, 5 polymyositis, 2 other myopathies and 5 ALS. In acute myocardial infarction with highly elevated CPK, serum CA-III remained normal. In 60 cases of various neuromuscular diseases, serum CA-III, CPK and aldolase were measured. Order of sensitivity (% frequency of elevated serum level) was CPK greater than CA-III greater than aldolase, however, CA-III was most frequently elevated in myotonic dystrophy which predominantly affects Type I fibers. In 15 ALS, raised CA-III was found in 5 cases, which were all in relatively early stages showing rapidly progressive clinical courses. This result raises a possibility to use serum CA-III for evaluation of the prognosis in ALS. It is concluded that CA-III is clinically applicable as a new diagnostic marker for muscle diseases, and probably reflects Type-I fiber abnormalities more sensitively than CPK and aldolase.

Of the 10 chronic haemodialysis patients whose serum TnT levels exceeded the threshold value of 0.1 microg.L(-1) at entry into the study, four were dead at 1 year and three others had a diagnosis of CAD. Of the 20 chronic haemodialysis patients with normal serum TnT levels at entry, one died and none had CAD. All five deaths were cardiac related, either arising from acute myocardial infarction or by sudden death. When serum TnT levels were compared with accepted predictors of death in chronic haemodialysis patients, such as serum creatinine, serum albumin and haematocrit, in the present study serum TnT proved to be more accurate and had excellent sensitivity and specificity. Serum TnT was also superior to serum TnI, which proved to be no more discriminating than the non-specific muscle marker, aldolase.
