ApolipoproteinB 100 (apoB-100) is an important component of atherogenic lipoproteins such as LDL and serves as a ligand for the LDL-receptor. Familial defective apolipoproteinB 100 (FDB) is caused by a R3500Q mutation of the apoB gene and results in decreased binding of LDL to the LDL-receptor. So far FDB is the most frequent and best studied alteration of apoB-100. Apart from this, three other apoB mutations, R3500W, R3531C and R3480W, affecting binding to the LDL-receptor are known to date. We screened the apoB gene segment of codons 3448-3561 by denaturing gradient gel electrophoresis (DGGE) analysis in a total of 853 consecutively sampled German patients undergoing diagnostic coronary angiography for suspected CAD. By this, a new single base mutation was detected and confirmed by DNA sequencing. The mutation, CAC(3543)TAC results in a His3543Tyr substitution in apoB-100 (H3543Y). The prevalence of heterozygotes for H3543Y in the study population was 0.47% compared to 0.12% for the known Arg 3500 Gln (R3500Q) mutation. In conclusion, the new mutation is four times more frequent than "classical" FDB and thus appears to be the most common apoB mutation in Germany.

Diagnostic testing for CAD is aided by the calculation of the pretest probability using either the Diamond-Forrester score or the Morise score. Patients who have a low risk of CAD should not undergo testing. Exercise ECG testing should be reserved for patients who have pretest probabilities lower than 20%, because a negative test does not adequately reduce the posttest probability of significant CAD. For patients who are at intermediate risk, either nuclear perfusion imaging or stress echocardiography is an acceptable choice depending on local availability and practice. Due to its low specificity, CAC scoring is currently limited in its usefulness for the diagnosis of CAD in symptomatic patients. Currently, screening for CAD among patients at low risk should not extend beyond screening for traditional risk factors. Physicians should use the Framingham Risk Score to stratify patients into levels of 10-year risk for cardiac events. Due to its high rate of false positive tests and low sensitivity, exercise ECG is of limited value in screening. Among patients with higher levels of risk, in whom further risk stratification would be of use in making decisions about risk factor management, measurement of CAC either with EBCT or multidetector row CT scanning is a promising option, but more research is required before its use should become widespread. Measures of endothelial function are in development but lack data to support their widespread use currently.

Over the last decade, there has been increased recognition that atherosclerosis imaging adds greatly to the ability to identify patients at high risk for cardiac events. Technologies such as electron beam computed tomography and carotid intimal media thickness have contributed significantly to our understanding of the prevalence of preclinical atherosclerosis and its consequences. Current data suggest that elevated calcium scores are predictive of future cardiac events, independently and incrementally to traditional cardiac risk factors. The approximate predictive power is 10-fold for scores > 100, based upon current studies now reported. Guidelines and policy toward these modalities have shifted, with increased recognition of the importance among experts in cardiology, lipidology, and preventive medicine. Because most adverse events related to atherosclerosis occur in individuals at an intermediate risk, data suggest that it will be most cost-effective to concentrate screening efforts on this group of patients. This article reviews the current understanding of the value of coronary artery calcium screening in asymptomatic and symptomatic patients. Accurate measurement of subclinical coronary atherosclerosis should significantly improve the accuracy of global cardiovascular risk prediction, and allow for tracking of atherosclerosis burden, as well as better prediction of future cardiovascular events. Finally, by identifying high-risk patients, CAC may help select those patients who would benefit most from additional testing (e.g., non-invasive stress imaging) and intensification of medical therapy; CAC should have a significant impact on early detection and management of CAD.

Because T2DM increases the risk of coronary atherosclerosis and CAD and new noninvasive techniques to assess CVD risk have gained considerable popularity, it is important to know how these tools relate to each other. The aim of this study was to evaluate the relationship between the extent of coronary artery calcification measured by MDCT, plasma OPG levels, baPWV and the established cardiovascular risk factors in Korean patients with T2DM. From November 2006 to December 2007, 110 asymptomatic Korean patients with T2DM without prior evidence of CAD were assessed (mean age 57.2 years). CAC imaging was performed using a 40-slice MDCT. Serum OPG levels were measured by an enzyme-linked immunosorbent assay (Oscotec, Korea) from the serum samples of each subject. We measured the baPWV as an index of arterial stiffness. In addition, we measured fasting glucose, HbA(1)C, hsCRP and lipid profiles. A total of 74 patients (67.3%) had minimal or insignificant CAC (<10). The CACS, OPG and baPWV showed significant positive correlations with each other. The CACS was significantly associated with the baPWV, smoking and use of a statin. The baPWV was significantly associated with age, duration of DM, total cholesterol and CACS by multiple linear regression models of the dependent variables of CACS or baPWV. CAC and baPWV were significant predictors of each other (r = 0.359, P = 0.014 and r = 0.361, P = 0.004). The results of this study showed that CAC, baPWV and serum OPG levels were significantly correlated with each other in asymptomatic Korean patients with T2DM. Furthermore, our results suggest that arterial stiffness, as determined by baPWV, may predict the extent of coronary calcification by MDCT.
