The levels of
adenosine deaminase
(
ADA
) were determined in the erythrocytes of 10 patients with sexually transmitted HIV-1 infection [five cases with
AIDS-related complex
(
ARC
) and five with AIDS] before and after therapy with zidovudine (azidothymidine; AZT). A linear increase in
ADA
activity was observed during the second and third months of zidovudine treatment, with a final increase of about threefold after 3 months of drug administration. The concentration of adenosine triphosphate (ATP) was significantly lower in the erythrocytes of the same group of patients with respect to healthy controls, and a further decrease was noted after 3 months of zidovudine treatment. The results obtained indicate that treatment of
ARC
/AIDS subjects with zidovudine induces metabolic changes which could be responsible for the development of anaemia, an adverse effect frequently associated with zidovudine therapy.

The levels of purine enzyme activities were studied in 10 patients with acquired immunodeficiency syndrome (AIDS) or
AIDS related complex
(
ARC
) and in 6 healthy individuals with antibodies against human immunodeficiency virus (HIV). All AIDS/
ARC
patients studied had ecto-5'nucleotidase (ecto-5'NUC) activity in B lymphocytes below the normal range and 4 out of 6 clinically healthy HIV-positive likewise had reduced activity. Increased numbers of activated B lymphocytes were found both in the group of healthy HIV positive individuals and in AIDS/
ARC
patients. Further studies are needed to define whether the decrease in ecto-5'NUC activity on the B lymphocytes is a result of increased activation of the cells or of a B cell defect. No significant changes were found in ecto-5'NUC levels in T lymphocytes or mononuclear cells (MNC), neither in the group of AIDS/
ARC
patients nor in the healthy HIV-positive group. Both AIDS/
ARC
patients and healthy individuals with antibodies against HIV had increased levels of
adenosine deaminase
(
ADA
) activity in mononuclear cells, but only in the group of AIDS/
ARC
patients was the increase significant. No changes were found in purine nucleoside phosphorylase (PNP) activity in the two groups tested. From these investigations of purine enzyme levels and other markers of immune function in both sick and healthy HIV infected individuals we conclude that the observed changes in ecto-5'NUC and
ADA
activities in HIV infected patients are not a direct result of the HIV infection but develop early in the course of the disease.

The purine metabolic enzymes
adenosine deaminase

ADA
), purine nucleoside phosphorylase (PNP), and 5'nucleotidase (5NT) have been shown to be important for normal lymphocyte maturation. Abnormalities of these enzymes have been associated with hereditary as well as acquired immunodeficiency states. Enzyme activity was measured in helper (OKT4) and suppressor (OKT8) lymphocyte subsets from 10 homosexuals with
AIDS-related complex
(
ARC
) and in 10 healthy controls. There were no significant differences in either mean
ADA
activity or mean PNP activity between
ARC
OKT4 cells and control OKT4 cells and between
ARC
OKT8 cells and control OKT8 cells. By contrast, mean 5NT activity was slightly decreased in OKT4 cells from
ARC
patients compared with that of controls and more significantly diminished in
ARC
OKT8 cells compared with that of controls. Both deoxyadenosine and deoxyguanosine, when incubated separately with OKT4 and OKT8 cells in the presence of EHNA, an
ADA
inhibitor, did not significantly inhibit lymphocyte blastogenesis to a greater extent in
ARC
patients than in controls. Hence, the decreases in 5NT activity most likely reflect lymphocyte immaturity and are not associated with biochemical abnormalities leading to increased deoxynucleoside toxicity.

The association between human immunodeficiency virus type I (HIV-I) infection and high levels of erythrocyte
adenosine deaminase
(
ADA
) has been suggested by Cowan et al [1986]. We have analyzed the specific activities of the same enzyme during different stages of acquired immunodeficiency syndrome (AIDS), including asymptomatic subjects at high risk and patients with lymphoadenopathy syndrome (LAS),
AIDS-related complex
(
ARC
), full-blown AIDS, and AIDS encephalopathy (AIDS enc). The
ADA
activities were significantly higher (P less than .05) in asymptomatic HIV-I serum-positive individuals (13.1 U +/- 1.1) and in different groups of patients (LAS = 23.6 U +/- 10.2;
ARC
= 23.7 +/- 4.1) than those found in controls (9.5 U +/- 1.8) and in HIV-I serum-negative subjects (10.4 +/- 1.5). In patients with AIDS the mean
ADA
activity was of 32.3 U +/- 7.1, whereas in two cases with AIDS enc it was of 10 U. A tendency to increase in median
ADA
values with the progression of the disease was observed. In LAS patients the
ADA
values presented two distinct subsets falling below and above the cut-off line of 15 U/10(9) erythrocytes, respectively. A specific correlation to drug addition and its duration was observed: LAS subjects who discontinued drug abuse (median addiction time: 3 years) presented
ADA
values (median = 13 U) that are lower than for addicts (median = 27.2 U; median addiction time = 7 years) and are close to those observed for asymptomatic HIV-I serum-positive group. Evidence was also obtained for a progressive increase of
ADA
values of LAS patients with disappearance of the product of gag gene. These results suggest that LAS subjects with elevated
ADA
activities present a longer history of HIV-I infection and a higher probability of developing AIDS.

Adenosine deaminase (ADA), an enzyme involved in purine metabolism, has been shown to be of clinical importance in several diseases in humans. To investigate whether ADA is of any clinical significance in cats, plasma
adenosine deaminase
(P-ADA) and T cell
adenosine deaminase
(T-ADA) activities were measured in feline immunodeficiency virus (FIV) negative and positive cats. The
AIDS-related complex
(
ARC
) group showed a significant elevation in P-ADA activity compared to the asymptomatic carrier (AC), and FIV-negative groups (P<0.005). T-ADA activity was significantly elevated in FIV-positive cats compared to the FIV-negative group (P<0.05) and this elevation was attributed to the increase in the
ARC
group (P<0.01). A correlation was found between P-ADA and T-ADA activities in the FIV-negative group. T-ADA activity and CD4(+)cell number showed a strong negative correlation in FIV-positive cats (P<0.0005). CD4(+) cell numbers were significantly reduced in the
ARC
group compared to the healthy controls (P<0.005). Our results showed that T-ADA is increased in FIV-positive cats during the
ARC
stage. These results also suggest that ADA may be an indicator of T cell activation in the
ARC
stage of FIV infection.
