Heterogeneity of synovial fluid fibronectin was studied by means of Laurell cross-immunoelectrophoresis in patients with rheumatoid arthritis, posttraumatic
synovitis
and other arthropathies. Prior to
hyaluronidase
treatment all the synovial fluid samples exhibited the fibronectin heterogeneity, which disappeared after the action of
hyaluronidase
. The data obtained suggest that complexes of fibronectin and hyaluronic acid are responsible for physico-chemical heterogeneity of fibronectin in synovial fluid.

Spontaneous
synovitis
developed in the limb joints and rheumatoid factor-like component appeared in the sera of two rabbits from a pool 36 animals in the course of a long-term immunization with bovine nasal cartilage antigens. A single intra-articular injection of proteoglycan antigens regularly provoked a heavy
synovitis
and cartilage destruction irrespective of whether the booster injections were administered in physiological saline, or in Freund's complete adjuvant. The dose-dependent severity of arthritis suggested that the antibody titre against proteoglycan antigens played an important role in this mechanism. The synovial extract and synovial fluid of knee joints injected with proteoglycan antigens showed an increased enzyme activity concerning the four acid hydrolases (acid phosphatase, cathepsin D,
hyaluronidase
and beta-glucuronidase). The high activity of lysosomal acid hydrolases which persisted for several months can derange the molecular structure of proteoglycans of cartilage. The degraded proteoglycans may trigger autoimmune reactions, and the process eventually leads to chronic inflammation and joint destruction.

A rare case of mucinous nodules on the skin overlying rheumatoid
synovitis
in a 79-year-old man is described. The patient had a five year history of RA. The patient had complained of arthralgia in the left elbow joint, and X-ray demonstrated narrowing of thejoint space along with bone destruction. He underwent an intraarticular injection of dexamethazone and lidocaine. Three weeks later, he noticed two dome-shaped nodules about 5 mm in size developing on the elbow. Histopathological examination demonstrated poorly defined mucinous nodules in the upper dermis. The mucinous material positively stained with alcian blue and colloid iron, and was metachromatic with toluidine blue. These positive stainings disappeared after
hyaluronidase
digestion. Five to six weeks after being resected, both nodules recurred. Lesional injections of triamcinolone were effective. The intraarticular injections preceding the appearance of the nodules might have created channels from the joint space to the skin. Leakage of activated synovial cells, which produced hyaluronic acid, through the channels might have caused the mucinous stroma of the nodules.
